1 PART 1 - GENERAL 1.01 RELATED DOCUMENTS A. Drawings and general provisions of the Contract, including the supplemental documents as indicated in the Site Work General Provisions, apply to work of this section. B. Provisions of Division-3 Section "Concrete Work" prepared by others, with the modifications contained in this section, apply to work of this section. C. Concrete Formwork shall be in accordance with the ACI-347 "Recommended Practice for Concrete Formwork", American Concrete Institute DESCRIPTION OF WORK A. Extent of the concrete work is shown on the Contract Drawings and by the requirements specified in this section. Concrete work includes but is not limited to, the following: 1. Concrete slabs on grade, outside the limits of the building. 2. Concrete sidewalks and handicap ramps outside the limits of the building. 3. Concrete curb including drop curbs for handicap access. 4. Excavation, backfilling and compaction required for the installation and construction of the concrete work. 5. Saw Cutting existing concrete where new concrete work is to abut. 6. Maintenance and Protection of new and existing concrete work. 7. Concrete shock slabs over utility, drainage, and sanitary piping where there is less than 2 feet of cover from the top of pipe to the finished grade. 8. Concrete lighting, foundations and handicap sign foundations. 9. Concrete Formwork, furnishing, placement and removal of formwork for cast-in-place concrete. 10. Reinforced Concrete Retaining Walls QUALITY ASSURANCE A. Codes and Standards: Work, and types or methods of construction, shall in no event be less than that necessary to conform to all the provisions of the Town, and local law, ordinances and regulations of the Town, County or any of the jurisdictional departments, applicable to the work included in this section, including but not limited to: 1. 2 2. Town Planning Board Site Plan Requirements. 3. "Standard Specifications Construction and Materials, and Details" of the New York State Department of Transportation, latest edition. 4. Title 29 Code of Federal Regulations, Part 1926, Safety, and Health Regulations for Construction, (OSHA), latest edition. 5. Industrial Code Rules of the New York State Department of Labor Board of Standards and Appeals, latest edition. 6. New York State Labor Law sections pertaining to safety and health of employees, latest edition. 7. "Recommended Practice for Concrete Formwork", American Concrete Institute, publication No. ACI-347, latest edition. B. Minimum Requirements: Concrete work shall be accomplished in accordance with the requirements and regulations of the Town. 1. Work within the Roadway ROW shall conform to the requirements specified in the road opening permit of the appropriate municipal agency having jurisdiction. 2. Access: Provide continuous access to the site. Coordinate work with other contractors and subcontractors working on the site, adjacent roadways, or adjacent properties. The Contractor shall not obstruct access to and from the adjacent properties from adjacent roadways and driveways. 3. Safety: Observe applicable local and/or State requirements. Take immediate steps to rectify hazardous or unsafe conditions. Conform to the maintenance and protection of traffic requirements of the municipal authorities and County agencies having jurisdiction and the N.Y.S.D.O.T. M.U.T.C.D. C. Permits and Bonds: Purchase and submit copies of permits and bonds necessary in connection with the performance of the work specified in this section. At the job site, post notices and copies of permits necessary to the proper and lawful performance of the work, in accordance with such permits. Contractor shall obtain releases from the municipal agencies having jurisdiction approving the concrete work and submit copies to the Owner and its authorized representatives. D. Construction Observation: The Contractor shall provide access to the Owner, their representatives, representatives of the municipal agencies having jurisdiction and testing laboratories engaged by the Owner for purposes of observing and testing the work, at any time during the progress of the work, for purposes of determining the conformance to the specifications. Subsequent, observations and testing of redone work (of work that was determined to be unacceptable) will be at the Contractor's expense. TALL OAKS STP

3 1.04 SUBMITTALS A. Material Certifications: The Contractor shall submit to the Owner and representatives of the municipal agency having jurisdiction delivery tickets showing but not limited to the material type, date, time, project, amount and source and supplier certifications of the materials delivered and utilized on the site, to show compliance with the specifications JOB CONDITIONS A. Protection of Existing and New Improvements against damage and maintain said existing and new improvements, etc. in a working or specified condition. The Contractor shall repair or replace damaged existing or new improvements on this site or adjacent properties or ROW's, which are damaged by construction operations, in conformance to the contract drawings and specifications to the satisfaction of the Owner and the representatives of the municipal agency having jurisdiction at no additional compensation. B. Traffic: Conduct concrete work so as not to interfere with other contractors and subcontractors working on the site, and adjacent roadways or properties and the public utilizing the adjacent roadways and driveways. Do not close or obstruct streets, driveways, walks or other occupied or used facilities without written permission from authorities having jurisdiction. 1. Maintenance/Protection of Traffic: Maintain traffic and protect contiguous roadways and driveways during progress of the work. Materials, warning signs and traffic control devices used shall be acceptable to the State, County or municipal authorities having jurisdiction and in accordance to the N.Y.S.D.O.T. M.U.T.C.D. PART 2 - PRODUCTS 2.01 A. All materials required for the preparation of Portland cement concrete shall conform to either of the following requirements: 1. All Portland cement concrete materials shall conform to the New York State Department of Transportation Standard Specifications Section 501 Portland Cement Concrete, for Class A concrete with a minimum 28 day compressive strength of 4000 psi unless specified or shown otherwise on the contract drawings PREMOLDED BITUMINOUS EXPANSION JOINTS A. Premold Expansion Joints shall conform to New York State Department of Transportation Standard Specifications. TALL OAKS STP

4 2.03 REINFORCEMENT A. Reinforcement shall conform to New York State Department of Transportation Standard Specifications. B. Reinforcing bar reinforcement shall conform to New York State Department of Transportation Standard Specifications FORMWORK A. Form materials shall be as follows: 1. Unless otherwise indicated, forms shall be of wood, plywood, metal, or other similar material acceptable to the Owner or his authorized representative. 2. Forms in contact with exposed concrete shall have a smooth plywood surface or be lined with "Masonite" or similar type material. The plywood for these forms shall be DFPA "Plyform" - B-B Class I or II, or equal as approved by the Owner or his authorized representative. 3. Re-use of forms for exposed concrete shall be subject to approval by the Owner or his authorized representative. B. Form sealers shall be a non-staining, mineral oil or other approved material having no adverse affect on the concrete surface. The material shall conform to the New York State Department of Environmental Control guidelines and regulations. C. Form ties and spreaders for exposed work shall be "snap back" type, which do not leave an open hole through the concrete and shall leave no metal closer than one (1) inch from the concrete surface. Cutting ties back from the surface and the use of wire ties shall not be permitted. Use of wood spreaders shall be subject to the approval of the Owner or by authorized representative. If approval is granted for the use of wood spreaders, the spreaders shall be removed from the concrete at the time of pouring. Concrete improvements in which wood spreaders have been left shall not be acceptable, and shall be removed and reconstructed QUALITY CONTROL A. Contractor shall submit delivery tickets to the Owner and its authorized representatives for all Portland cement concrete, expansion joints, and reinforcement supplied and used clearly identifying the delivery date, supplier, material type and size/mix, and amount. The Contractor shall also submit the suppliers certification that the materials supplied conform to the specifications. TALL OAKS STP

5 PART 3 - EXECUTION 3.01 SURFACE PREPARATION A. General: Prepare soil base and subbase under the concrete as specified in Section Earthwork. The soil base and subbase shall be undisturbed or compacted immediately prior to the placement of Portland cement concrete to 95% of the Maximum Modified Proctor Density at the optimum moisture content. Damaged or unacceptable surfaces shall be removed and replaced in accordance to the specifications at no additional cost to the Owner FORMWORK A. General: Construct all required forms to be substantially sufficiently tight to prevent leakage; and, able to resist excessive deflection when filled with wet concrete. 1. Form all required cast-in-place concrete to the shapes, sizes, lines, and dimensions indicated on the contract drawings. 2. Coordinate with other trades, as required, relative to providing openings, anchorage, blocking, depressions, inserts, sleeves, and other items as shown or required, and shall be provided at time of concrete installation by the Contractor. B. Bracing: Construct all bracing, supporting members, and centering of ample size and strength to safely carry, without excessive deflection, all dead and live loads to which they may be subjected. C. Removal: Forms shall not be removed until the concrete has acquired sufficient strength to safely support its own weight and all loads upon it. This Contractor shall assume full responsibility for any damage to the work due to premature removal of the formwork. Tie hole rods shall be grouted solid. D. Curing Compound: Shall be placed on all exposed concrete immediately after removal of forms. All exposed vertical concrete surfaces shall have a high rubbing for uniform appearance unless specified or shown on the contract drawings otherwise curing compound shall be as specified in Division 3 concrete, prepared by others, or conform to the materials specified in the New York State Department of Transportation Standard Specifications PLACEMENT A. General: Portland Cement Concrete shall be of the size and type shown on the Contract Drawings, identified by permit and as specified herein. Removable form work shall be utilized for the installation of the concrete facilities specified herein. B. ROW Concrete Work: Concrete work within ROW's shall be accomplished in accordance with the appropriate road opening permit. TALL OAKS STP

6 C. Joints: 1/2 inch premolded expansion joints shall be provided at a maximum of 16 feet on center and at all points of curvature, tangentency and other points of direction change unless specified or detailed otherwise on the contract drawings. The joints shall be the shape and size to match the improvements. D. Construction Control Joints: Construction control joints shall be provided at 4 feet on center, unless specified or shown on the plans otherwise. Construction control joints shall be 1/4 inch in thickness by 3/4 inch in depth formed using a steel edger. E. Saw Cutting: Saw cutting of existing concrete improvements shall be to full depth or thickness. Surface to which new concrete improvements are to abut shall be smooth and straight. A 1/2 inch premolded bituminous expansion joint shall be provided at joint. F. Backfilling and Compaction: Backfilling and compaction around the Portland cement concrete improvements shall be as specified in Section Earthwork. G. Protection and Maintenance: Upon the completion of the Portland cement concrete improvements, the Contractor shall maintain and protect the improvements. Any damaged or otherwise unacceptable Portland cement concrete improvements shall be removed and replaced in accordance with the specifications herein at no additional compensation. H. Concrete Slabs and etc.: Shall have reinforcing bars and mesh continuous between joints as shown on the contract drawings. I. Concrete Shock Slabs: Shall conform to the slab detail on the contract drawings. The slab shall be placed so as to permit asphalt paving over the slab. The slab surface shall be treated with a liquid asphalt tack coat immediately prior to placement of asphalt in conformance to New York State Department of Transportation standards FIELD QUALITY CONTROL A. Construction Observations: Field Quality Control shall be as described in Section 1.03 D of this specification. B. Finished Grade Tolerances: The finished elevations of the Portland cement concrete work specified herein shall be 0.01 feet of the proposed finished elevation shown on the contract drawings or the established elevation. Ponding of surface water greater than 0.01 feet in depth will not be acceptable. Concrete thickness tolerance shall be 0.02 feet less than the design thickness. The face of the curb shall not vary more than 0.03 feet along the flow line, unless shown otherwise on the contract drawings. Concrete improvements not meeting these tolerances shall be removed and replaced by the Contractor to meet the required specification requirements, at no additional compensation. Actual elevations shall be determined as specified herein and/or using field survey methods. Measurements shall be based on field surveying and use of a 10 foot straight edge placed longitudinally along the flow line. C. Slump, Air Entrainment and Compressive Strength: The concrete slump for work shall be 2-1/2 to 3-1/2 inches and air content shall be 6.5% + 1%. The minimum 28 day compressive strength shall be 4000 psi unless otherwise shown on the contract drawings. TALL OAKS STP

7 The slump compressive strength and air content shall conform to the applicable latest A.S.T.M. (American Society of Testing and Materials) Designations, to include but not be limited to C-143, C-31, C-39 and C-231. D. Submittals: Tickets and supplier certifications shall be as specified in 1.04 of this specification. END OF SECTION TALL OAKS STP
