Locality L.B.Nagar X Roads or within 1.5 Kms radius of L.B.Nagar having accesability to Bus Stop / Metro Station. Intersted parties may quote the per sq.ft ( carpet area ) rate in a sealed cover and send it to Addl.Commissioner, on the address noted above on or before by A.M. Tender opening date : at 3.00 P.M. 2 Important Instructions for Bidders regarding Online Payment All bidders/contractors are required to procure Class-IIIB Digital Signature Certificate (DSC) with Both DSC Components i.e. Signing & Encryption to participate in the ETenders. 3 TERMS AND CONDITIONS OF E-TENDER IN SINGLE BID SYSTEM FOR HIRING OF ACCOMODATION ON RENT FOR BRANCH OFFICE -L.B.NAGAR EMPLOYEES' STATE INSURANCE CORPORATION, TELANGANA Quotation should inter-alia contain the details as follows: a. The location and address of accommodation. b. Detailed approved plan of the building as per building by laws of the Municipal Corporation. c. Clearance/ No Objection Certificate from all the relevant Central/ State/ Municipal Authorities and fire department for use as an office/dispensaries premises conforming to the Municipal Laws. d. Facilities and amenities available with the building. e. The rent offered per sq.ft.(carpet area) basis only. The rent offered should be inclusive of property tax or any other tax required to be paid by the property owners. Electricity Charges to be paid seperately by ESIC. f. Facilities required to be provided by the owner. I) Built up area around 1500 sq.ft.(carpet area) preferably on ground floor as specified for above location. II) The building should be fit for use of office use and Dispensary. III) Easy accessibility the approach road of premises offered should be sufficiently wide. IV) The building should have adequate windows & doors and be properly ventilated. V) Assured free parking space be provided/ made available for vehicles. VI) The premises offered should have electrical fittings and fixtures such as switches, power points etc. VII) There should be a provision for 24 hrs electricity supply preferably with power backup. VIII) The area proposed to be given on rent should be preferably on ground floor. However, preference would be given to exclusive building where only our dispensary and office would be housed with a boundary wall of reasonable height. IX) The maintenance Civil, Electrical, Mechanical, Plumbing including consumable etc. shall be provided by the owner. X) Owner will also undertake to carry out annual repair and maintenance as and when required. XI) The building should be in a ready to use condition with facilities like electricity, potable water and sewage disposal etc. XII) The Building should have Gents / Ladies toilets for Staff & Visitors seperately and proper waiting space. XIII) The Landlord should do any minor alteration if required,by this office at his/her own cost, which includes civil & electrical works. XVI) Owner of the building will bear the stamp duty charges to execute rent agreement and registration of the agreement on equal cost basis. XVI) The owner of the building should allow for display of Branch Office Sign/Name Board in the front of

4 the building in a prominent place, and also installation of anteena for network signals on the top of the building as ESIC is a computerized office. 2. The period of lease should be for a minimum of 5 years duration and extendable on mutual consent under agreement. Rent enhancement after expiry of agreement period is subject to mutual consent of land lord and ESIC as per guidelines of ESIC applicable at that time. 3. All entries in tender form should be legible & filled clearly. In case any overwriting/cutting in entries/rates quoted in figures & words in tender form, entries/rate quoted in words will be considered. There should not be any use of white fluid in tender form. 4. The Tenderers should ensure that no corrupt file/page is uploaded failing which the tender shall be outrightly rejected. 5. Submission of bids: Proposals should be submitted in single bid namely, a. Those applying online through e-tender has to upload the following documents i. Annexure A & Annexure B ii. Approved plan of the building iii. Clearance / NOC from Muncipal authorities of the building. iv. Latest tax receipt. b. Those not applying through e-tender has to submit their offer in the drop box available at ESIC Regional Office, Adarshnagar Hyderabad. c. Last date to upload e-tender / drop box : upto A.M. d. Both e-tender and sealed tenders will be opened on at 3.00 P.M. The tenderer or his authorised representative may remain present at the time of opening of tender. 6.
