1 Ms. Elaine Camia Project Manager City of Calabasas Mureau Road Calabasas, CA Subject: DRAFT Proposal to Provide Public Works Observation Services for the City of Calabasas Overlay Project Dear Ms. Camia: Willdan is pleased to submit this proposal to the City of Calabasas to provide public works observation services for the Calabasas Overlay Project. As requested, this proposal includes two levels of service; one limited to providing public works inspection of the paving portion of the work with an option for addition of construction contract administration services. It is understood that the City will provide administration and construction inspection for all aspects of the work, except in the area of ARHM materials quality assurance and construction inspection. The Construction Observation Services and Asphalt-Rubber Materials Quality Assurance outlined below will be performed, only as apply to ARHM construction inspection. A Willdan Senior Public Works Observer will provide the full-time paving inspection, BTC Laboratories will provide batch plant inspection, compaction and materials testing, and a Willdan Supervising Public Works Observer will provide overall coordination and consultation for all services. In addition, Ms. Roxanne Hughes, Supervising Engineer, will be present at the batch plant and visit the job site on the first day of production and provide oversight for all services rendered. CONSTRUCTION CONTRACT ADMINISTRATION 1. Maintain a project construction file for the City, beginning at project award and continuing until final close-out. The project will be coordinated with a goal of completing the work and filing NOC no later than June 30, Conduct the preconstruction meeting, review and approve all submittals and issue Notice to Proceed. 3. Work with the Director of Public Works to coordinate public works observation services to be provided by City staff. 4. Process all necessary change orders, requests for information, and related construction related paperwork required to administer the construction contract. 5. Review progress invoices with appropriate inspection staff, prepare progress payment request in City format and submit for Council approval.

Coordinate final acceptance of project with appropriate inspection staff, file Notice of Completion and prepare release of retention for City issuance at end of the required 35 day period. CONSTRUCTION OBSERVATION SERVICES The construction observation services are generally performed in accordance with the provisions of the latest editions of the Standard for Public Works Construction Green Book, City of Calabasas Standard Plans, Caltrans Standard Specifications, County of Los Angeles Field Inspection Work Book for ARHM, and the Public Work Inspector s Manual. Following is a listing of the inspection services that will be applied to the City of Calabasas Overlay Project: 7. Conduct a field investigation of the project area to become familiar with the existing facilities and the project environment. 8. Become familiar with traffic control plans, construction schedule, construction sequence, and permit requirements from other agencies. 9. Meet with City staff to review the scope of work and establish project schedules. 10. Attend preconstruction meeting. 11. Provide full-time construction observation of the work to monitor materials and methods towards compliance with plans, specifications, and contract documents, and address and document non-conforming items as they are discovered. 12. Monitor compliance with Cal OSHA requirements and compliance with all local, state, and federal regulations, including but not limited to, Clean Air Act and Clean Water Act (NPDES). 13. Meet with the contractor at the beginning of each day and review proposed work plans, including specific details that may affect progress. 14. Review actual contractor performance throughout the day and discuss discrepancies with the contractor as they occur. 15. Coordinate activities of engineering support, surveying, and testing. 16. Coordinate contractor s fieldwork with utility companies and other agencies. 17. Attend weekly jobsite meetings and other meetings with City staff, merchants, engineer, public relations personnel, contractor, and construction administrator, as needed. 18. Observe construction safety and public safety and convenience, and report discovered discrepancies to construction administrator and City. 19. Evaluate Contractor s operation and production with respect to quality and progress, and report to construction administrator and City.

3 Page Maintain copies of all permits needed to construct the project and enforce special requirements of each. 21. Maintain detailed daily diary on construction progress showing site conditions, weather conditions, traffic control measures taken by the construction contractor, labor, equipment, and materials used during the day, quantity of work performed during the day, and major incidences occurring each day. 22. Prepare weekly statement of working days, and submit to Contractor and City. 23. Prepare clear and concise letters and memorandums, as needed. 24. Maintain field file bound workbooks during construction, including a cumulative record of quantities constructed, daily and weekly reports, working day reports, change order documentation, photographs, and other documentation. 25. Analyze delays and review claims on a timely basis and make recommendations to the City. 26. Review, evaluate, and discuss with the City, all requests for changes of work, including costs involved; prepare change orders, including provision of cover memorandum of explanation. 27. Maintain all data for change orders, and record information with regards to the time of dispute, time of notification by the contractor, and action taken by the inspector. 28. Provide complete measurements and calculations documented to administer progress payments and make recommendations for payments. 29. Prepare punch list at substantial completion and follow up. 30. Upon project completion, provide the finished set of project workbooks to the City. ASPHALT-RUBBER MATERIALS QUALITY ASSURANCE 1. Provide any assistance requested in relation to qualifying the equipment and process of asphalt-rubber production for the low bidder prior to construction. 2. At the preconstruction conference, provide consultation, such that contractor, subcontractors, suppliers, and inspectors have a full understanding of their roles or obligations over the course of the project related to ARHM construction. 3. Provide assistance prior to construction, as requested by the City, in preparation for start of work, including notification, in writing, of required material, related submittals, and required record of production tests. 4. Provide asphalt-rubber binder plant inspection at outset of project, as necessary, to establish proper constituents and formulation in production and that proper reaction time and temperature parameters are confirmed with the manufacturer at the plant.

4 Page 4 5. Provide coordination of ARHM plant inspections by materials testing consultant, and provide necessary directions to facilitate asphalt-rubber binder monitoring by ARHM plant inspector. 6. Monitor ARHM plant inspection through review of test results and consultation with materials testing consultant. 7. Be present at the project site on the first day of construction to assist and provide consultation for inspection, and as necessary to obtain full compliance with specifications as normal procedures on the job. 8. On the first day, also work with the materials testing consultant at the construction site to establish field sampling parameters, proper rolling pattern and nuclear testing requirements. 9. Provide any requested miscellaneous consultation during the project, including discussions with the contractor to emphasize proper treatment of asphalt-rubber materials. 10. Coordinate corrective measures and prepare reports for clarification of corrections of deficient test results. 11. Compile documentation from the asphalt concrete (AC) plant inspection consultant, including any field test results on the material from the contractor, and generally all materials-related documentation and submit to the City. KEY PERSONNEL Willdan offers the City of Calabasas a complete staff of experienced public works observers and construction managers with demonstrated capabilities in municipal public works construction projects. We propose to assign the following personnel to your project: TBA FEE Willdan will be compensated on an hourly basis for providing professional public works inspection services. Our not-to-exceed fee is based on a 25 working-day construction contract, which will require part-time public works observation at 4 hours per day. The batch plant inspection/material-testing fee is based based on a 5 paving-day construction observation schedule, which will require full-time public works observation at 8 hours per day plus full time supervision on the first day of paving with as-needed supervising consultation on the remaining days.

The above schedule is for straight time. Overtime will be charged at 1.25 times the standard hourly rates. Sundays and holidays will be charged at 1.70 times the standard hourly rates. Blueprinting, reproduction, messenger services, and printing will be invoiced at cost plus 15 percent. A subconsultant management fee of 15 percent will be added to the direct cost of all subconsultant services to provide for the cost of administration, consultation, and coordination. These hourly rates are effective through December 31, 2003, and may be adjusted after that date to compensate for labor adjustments and other increases in costs. Our inspection staff is available to perform inspection services (per the above hourly rates) for City of Calabasas on either of the following work schedules: 9-80 flexible work schedule (9-hours per day, with a day off every other week) from 6:30 a.m. to 4:00 p.m. Normal workweek of five, 8-hour days per week (7:00 a.m. to 3:30 p.m.).
