1 4501 West DeYoung Street, Suite 100B Marion, Illinois Ph Fax Addendum No. 1 to the Bidding Documents For: Toilet Partition Replacement Massac County High School Massac CUSD No. 1 Metropolis, Massac County, Illinois Project No Date: May 7, 2014 To: Prospective Bidders This addendum forms a part of the bidding and contract documents and modifies the original bidding documents, dated April 20, Acknowledge receipt of this addendum in space provided on the bid form. FAILURE TO DO SO MAY SUBJECT BIDDER TO DISQUALIFICATION. PROJECT MANUAL: 1. PRE-BID MEETING NOTES: Pre-bid meeting notes and attendance record as attached. Items discussed in the pre-bid notes shall be part of the said contractor s contract. All items discussed shall be included in the said contractor s bid. CLARIFICATIONS: 1. Under Alternate Bid No. G-2, contractor shall remove and replace both grab bars in toilet stall. 2. Alternate Bids will be accepted in order that is most beneficial to owner. SPECIFICTIONS: 1. SECTION STAINLESS STEEL TOILET COMPARTMENTS Add the following paragraph: 2.1.C.5 Door sizes shall be per manufacturers recommendations.

2 2. SECTION PLASTIC TOILET COMPARTMENTS Add the following paragraph: 2.1.C.6 Door sizes shall be per manufacturers recommendations. DRAWINGS: 1. A-1.0 OVERALL FLOOR PLAN, ENLARGED FLOOR PLAN, DETAILS AND NOTES Clarification: At Boys Locker 179, contractor shall provide a 7 floor mounted pilaster at end of privacy panel. Alternate Bid G-2 shall include both grab bars in stall. 2. A-1.1 ENLARGED FLOOR PLANS AND NOTES Clarification: At Boys 112 and Faculty Boys 121, contractor shall provide a 7 floor mounted pilaster at end of privacy panel. Project Description: Base Bid - Removal of existing toilet partitions and installation of new solid plastic (HDPE) toilet partitions throughout facility. Base bid includes removing and replacing existing toilet accessories. Alternate G-1: Provide all materials and labor required to install stainless steel toilet partitions in lieu of solid plastic toilet partitions. Alternate G-2: Provide all materials and labor required to install new grab bars in lieu of reusing existing grab bars. Alternate G-3: Provide all materials and labor required to install new toilet tissue dispenser units in lieu of reusing existing dispensers. Alternate G-4: Provide all material and labor required to install new sanitary napkin disposal units in lieu of reusing existing disposal units.

4 3. Schedule: Work to commence on June 1, 2014 (after last day of school) and be completed no later than August 1, 2014 per Section Construction Schedule. 4. See Section Supplemental Conditions for specific insurance requirements. Prevailing Wage: This project will require payment of prevailing wage. Supervision: At all stages of the project, competent, experienced supervisory personnel familiar with the project requirements will be required on site to supervise the work. 5. Discussion: 6.
