FS LTR001-JH Rev A Richard Baker Energex Ltd Property Services GPO Box 1461 Brisbane QLD 4001 Dear Richard, Re: Energex Leased Residential Property 2417 Beaudesert-Nerang Road, Canungra, QLD Asbestos containing materials register COHLABS is pleased to provide the asbestos containing materials (ACM) register for the following Energex leased residential property as follows: 2417 Beaudesert-Nerang Road, Canungra, QLD Project details COHLABS was engaged by Mr Richard Baker of Energex Ltd to undertake an ACM survey of the residential leased property listed above. The surveys were conducted by Stuart Rowley representing COHLABS on Friday 07 July Scope of services In accordance with the requirements of the Client, an inspection of all above ground accessible internal and external areas within the nominated properties was required to identify all visible ACM situations and as far as reasonably practicable. Where required, samples were collected from suspect materials identified to confirm the presence or non-presence of asbestos fibres by NATA accredited laboratory analysis. The ACM registers (generated from the Energex Site Hazard Register database) are provided in Attachment A with the laboratory analysis certificates of analysis. Limitations Scope of services The ACM register (the report has been prepared in accordance with the scope of services specified in the contract, or as otherwise agreed, between the Client and COHLABS. The scope of services may have been limited due to various factors such as time, budget, access and/or site disturbance constraints. COHLABS Pty Ltd ABN Page 1 of 4

Unless stated otherwise, COHLABS has not verified the accuracy or completeness of the information provided. COHLABS will not be liable in relation to incorrect conclusions and assessments should any information provided be incorrect or have been concealed, withheld, misrepresented or otherwise not fully disclosed to COHLABS. Dimensions included in the ACM register are estimates only and contractors and other parties should perform their own measurements to determine the quantities of any materials to be removed or repaired during future works. Access restrictions It may not be possible to identify the presence of all asbestos containing materials during the course of an inspection. This is due to various factors including, but not limited to: restrictions on access to certain areas such as blind service risers, plant and equipment, sub-floor voids, ceiling spaces, crawl spaces, wall cavities, etc. live plant and equipment availability of building/plant drawings and plans While the inspection of the nominated buildings included access to as much of the buildings as possible, certain areas of the buildings remained inaccessible, including those areas that could not be accessed due to health and safety concerns. Please note that no areas were inspected at a height of 3 metres above the ground. Please also refer to the specific areas not accessed for the property identified in the attached ACM register. Identification of asbestos in bulk samples Samples of materials suspected of containing asbestos collected during the inspection are typically submitted to a NATA accredited laboratory to determine the presence or absence of asbestos fibres in the sample. The ability of the laboratory to identify asbestos in samples can be impacted by the presence of binding agents such as adhesive, vinyl or bitumen in the sampled material, or if the material has been heat affected. Reliance on an inspection The client must not rely upon an inspection or report as indicating that a site or building is asbestos free. All that the report can be relied upon to show is that no asbestos containing materials were found (or that only such asbestos containing materials were found as was reported to have been found) in the course of the inspection. The findings of the report must be considered together with the specific scope of services and limitations on the type of inspection undertaken. Third party reliance The report has been prepared for the Client and is not to be relied upon by any other third parties. COHLABS will not be liable to any third party for any loss or damaged suffered by third parties arising from their reliance on information contained within or any assumptions inferred by the report.
