2 SUMMARY First wear limited factory building was constructed in 2002 and started production in The six (G+5) storied building was constructed as an industrial structure with total floor area of sq. meter and 70 ft in height. The building is also occupied by another two factory named Style Wash and Vertex Apparels Ltd from ground floor to roof. Extended shed from main building is being used as generator room. Reportedly there here were 480 workers in the factory during the inspection. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The Specific timing of improvements, including any requested extensions due to design / installation constraints shall be submitted to the Accord for approval. Page 2 of 11

3 FINDINGS AND RECOMMENDATIONS FINDING NO: E- 1 CATEGORY: DESIGN, DRAWINGS & RECORDS 1. Thermo graphic scanning of the entire electrical system has not been performed 2. Insulation resistance test of electrical equipment is not performed 3. Electric safety program is not initiated 4. As-built electrical SLD, wiring layout designs and drawings, machine layouts are not prepared. 1. Thermo graphic scanning of the entire electrical system must be performed on tri-annual basis and recorded 2. Insulation resistant test of all the cables must be performed once every 5 year cycle and recorded 3. Electrical safety training and awareness program for the electrical personal and workers must be initiated and recorded 4. The factory must have As-built electrical SLD with electrical wiring layout designs and drawings. Any changes in load, protection system, conductors, Generation and supply system must be reflected in the As-built SLD and drawings. PRIORITY: P1 REMEDIATION TIME FRAME: 25 WEEKS FINDING NO: E- 2 CATEGORY: SERVICE LINE Service line crossed to water pipe line. Cables must be kept minimum 0.9 meter distance from water supply line. 4 FINDING NO: E- 3 CATEGORY: SERVICE LINE 11 kv Service line entering building without support. The cables must be weather resistant and supported with steel wire. It is recommended that the cables be drawn underground from pole to the factory building. Replace silica gel and must include in routine maintenance to check and maintain. 5 FINDING NO: E- 6 CATEGORY: TRANSFORMER ROOM Excessive dust and lint deposit in transformer room. Establish a routine cleaning program to keep neat and clean the transformer room. Shut the power of the transformer and clean the exterior of the transformer at scheduled period. PRIORITY: P3 REMEDIATION TIME FRAME: 1 WEEK Power transformer frame FINDING NO: E- 7 CATEGORY: EARTHING SYSTEM Earthing wires are not protected and spliced improperly. Earth conductor should be laid through PVC or steel pipe (on wall/floor at safe location).it should be supported or clammed at regular interval and Mid-length joints in earth conductor must be avoided. PRIORITY: P3 Earth conductor spliced improperly FINDING NO: E-8 CATEGORY: EARTHING SYSTEM Earth conductor drawn from panel enclosure has leakage current. (8.07 Amps) Earth and neutral point should be terminated at separate point and insulation must be checked again. 6 FINDING NO: E- 9 CATEGORY: EARTHING SYSTEM Open earth loop measured. Check for loose earth-connection. Reconnect wire and check the continuity of earth wire (if the value is within required level). Cable tray or conduit must be passed across the wall to support and protect the cables. The openings after the passage of cable tray or conduit (the wiring system) should be sealed with the fire rated materials. REMEDIATION TIME FRAME: 12 WEEKS. 7 FINDING NO: E-12 Panel base plate not provided and lint and dust deposited in the panel. Cables entering through base plate must enter from cable entry openings guarded with rubber bushings/glands and the remaining gaps in the opening must be sealed with noncombustible materials. Bottom of HT switchgear FINDING NO: E- 13 CATEGORY: CABLE & SUPPORTS Cables in flexible PVC are not supported. Cables must be supported by covered cable tray/ladder, fixed on wall, up to the top plate of the panel. Distribution board in Electrical room without base plate and lint and dust deposited in it. FINDING NO: E- 14 CATEGORY: GENERATOR ROOM Storage in generator room i.e. oil drum kept inside the generator room. Remove all the oil drum from generator room, Any kind of combustible materials cannot be stored inside the generator room and near any electrical panel. Establish a routine cleaning program to keep the generator room neat, clean and dry. REMEDIATION TIME FRAME: 1 WEEK Storage(oil drum) near control panel of generator Page 7 of 11

8 FINDING NO: E-15 CATEGORY: GENERATOR ROOM Oil spilled in generator room floor. Generator room floor must be kept free from oil spillage to avoid fire hazard. Establish a routine cleaning program to keep the generator room neat and clean. Generator battery placed on wet floor FINDING NO: E-16 Panel base plate removed to allow cable entry Install base plate of the panel and make hole into it then fit cable gland (required sized) for cable entry and exit to the panel and seal all the unused openings by suitable means to make the panel dust and vermin proof. 9 FINDING NO: E-18 CATEGORY: WIRINGS The opening of the permanent wall after passing cable is not sealed. Cable tray or conduit must be passed across the wall to support and protect the cables. The openings after the passage of cable tray or conduit (the wiring system) should be sealed with the fire rated materials. REMEDIATION TIME FRAME: 5 WEEKS Cables entering production floor FINDING NO: E- 19 Openings in the panel top cover plate. Make circular hole at the top plate of panels and provide cable gland according to the respective cable size for cable entry and exit so that the cables are not stressed on the sharp edges of the hole of panels. Arrange periodic inspection & thermal scan to identify the overloading, loose connection, unbalanced load which may cause the excessive heat-rise and take action accordingly. 11 FINDING NO: E- 24 CATEGORY: LIGHTNING PROTECTION & EARTH The factory building(s) not installed with lightning protection systems. Lightning arrestor must be installed (according to BNBC Part 8, section 2.9.) with proper size air termination network, down conductors and earth termination.
