1 T-2822 TARGET BOSTON / FENWAY ALL UNITS OTIS PUNCH LIST The Target Store was reviewed and checked by Thomas Branham of Elevator Consulting Services, Inc. on December 8 th, My QEI number is 1339 and I am certified by NAESA. NAESA is the National Association of Elevator Safety Authorities. We have identified corrective items that in our opinion need to be addressed by the Elevator Contractor and the General Contractor. They are free to offer an explanation or dispute any individual item within the report but we can offer a reasonable reason for each and every negative evaluation. They are evaluated on and for the ASME 17.1 and 17.2 Code for Inspectors Manual along with industry standards for acceptable installation and maintenance procedures. 1 OF 16 T-2822 FENWAY

2 T-2822 TARGET BOSTON / FENWAY FREIGHTS 1, 2, 3 1. The door controller for elevator 2 does not have legal clearance under the electrical code. The clearance is 34 inches and 36 is required. May notch the wall in front to gain clearance because it is so close or relocate the controller. 2. The door controller for elevator 3 does not have legal clearance under the electrical code. The clearance is only 28 inches. Relocate the controller to have proper clearance. 3. There is a conflict with the code data tag. The controller tag shows 2004, current adopted code is Verify the contract was signed prior to the new code being adopted or comply with prevailing code. 4. Provide a roller work platform. Due to the height of the machines, a work platform is required. A roller unit is a good option rather than a permanent platform. A roller version can accommodate all three machines. 5. Stencil the car top crosshead, toe guard, controller, machine unit and pit equipment with the complex unit numbers. 6. Install the crosshead data tag. 7. Phone- The phone wires shall be terminated in a x4 electrical box on top of the controller or on the duct. The cover shall be identified as phone and the associated dedicated phone number shall be identified with a sharpie or label. Confirm a sharing device is available from Otis and intended to be used. Only one emergency phone in machine room. If not a sharing device, the phone must be portable or additional phone lines per elevator must be provided. 8. The phone shall be programmed with the appropriate store number, T-2822, location and elevator number. The phones shall auto dial Otis s 24 hour number at no additional cost. 9. Provide 10 keys per key switch device per elevator. Includes all key switches. 10. Provide tasking logs or log books, a copy of the full load test report in a file folder per elevator in the machine room. MSDS sheets only on gear oil for the machine. Target requires mandated hours of PM during warranty and extended maintenance. Visitations are required and documented. Critically important for compliance. 2 OF 16 T-2822 FENWAY

3 11. Install the fire box in each elevator bank vestibule. Provide and tag all keys. 12. Install identification 4 x 4 tactile on the main floor egress. Goes on the entrance jamb 13. Camera- bring the coax cable and 2-14 gauge wires to the corner of the canopy opposite the car operating paned and put in the pre-installed electrical box or a new x 4 electrical box. Label it Camera with a sharpie or label. The other end in the machine room has to be brought to a x4 box mounted on top of the controller or the duct. The cover of the box shall be labeled or identified as camera. Both the coax and 4 gauge wires shall be terminated at this box. 14. All gate drive assemblies (front and rear) need stop collars on the drive three sprocket shaft Provide low overhead signage on the car top due to the deflector sheave assemblies. 16. Remove all temporary light cords and patch the holes. 17. Straighten the fan shroud on the car top due to being crushed when somebody stepped on it. 18. Define load weighing. Need to know if load shedding or bypass. If load shedding, advise of what it will be set at. 19. Once the code version is established. 5 32. Once the code version is confirmed. Provide the proper main egress hall fixture if phone monitoring is required. 33. Relocate the selector wiring to eliminate a trip hazard on the car top. Run the wire under the mounting plate rather than on top. Secure with straps or ty wraps along the edge. 34. The counterweight top left side throat is rubbing on the rail. Redo the roller guide mountings to center the counterweight. 35. The RFI about the SEES Enforcer gibs evidently was not resolved on this elevator. I highly recommend these special door gibs get installed on this elevator (at least on the fast speed doors) due to being the cart retrieval elevator. 36. Pin the clutch release assemblies with a tek screw at each landing once they are set by the adjuster. Will accept spray painting the perimeter so the final set of the plate is identified for future use by techs on call backs. 37. Provide and install the Essex key pad for the elevator. Incorporate the interface with the controller software. (Eventually will provide training to Target personnel on how it works, functions and advantages. 38. Confirm the installation of the emergency power panel is not needed. May be part of larger panel in fire command. Coordinate the emergency power testing. 51. 8 52. Replace the left side jack packing. T-2822 TARGET BOSTON / FENWAY ELEVATORS 8 & Provide oil return lines from each jack head to a pit bucket or scavenger pump. 54. Pin the clutch release plates on each landing with a tek screw. Will allow spray painting around the perimeter once they are set by the adjuster. 55. There is a conflict with the code data tag. Install the crosshead data tag. 58. Lube the bottom roll pin of each sill mounted spring closure device. 59. The cover shall be identified as phone and the associated dedicated phone number shall be identified with a sharpie or label. Confirm a sharing device is available from Otis and intended to be used. Only one emergency phone in machine room. If not a sharing device, the phone must be portable or additional phones per elevator must be provided. 60. Stencil the complex unit number on the top and bottom handrail inlet shroud. The up should be number 1, the down unit should be 2. Use a standard 4 inch self-adhesive number where it will be visible. 70. Provide barricades on site that will seal off both ends per escalator. Total of four for this set of escalators. Provide in the closest passenger or freight machine room to ease the burden on the field techs. 71. Provide training on escalator starting, comb teeth observation, etc. 72. Provide 20 keys to Target that are used to start, control or operate the escalators. Spares after the LOD s have their own key, shall be placed in the machine room for future distribution. 73. 12 T-2822 TARGET BOSTON / FENWAY CARTVEYORS 5 & Stencil the bottom and right side balustrade with the complex unit number. Can be sequencial with the escalators. Provide a 4 inch label at the bottom of the balustrade to identify 5 as the up unit, 6 as the down unit. 81. Install the correct landing plate at both the top and bottom. Needs to match the style and format of the adjacent escalator. Reinforce the leading edge to the unit with a flat stock to strengthen the spongy transition. 82. Install the missing top stainless steel panel on the right side curve. 83. Install the anti-skid tape when you know where it should be utilizing the cart wheels as a guide. 84. Schedule Pflow into finish the installation. GC Must have at least 10 carts of each kind on site prior to scheduling. 12 OF 16 T-2822 FENWAY

13 T 2822 Target Fenway Boston Fenway GC and Target Requirements General items 85. Provide permanent power to each bank, each elevator. Check and coordinate with Otis for proper phasing before turning on per bank. 86. Complete low voltage fire modules. 87. Complete the emergency power panel. Pretest the transfer switch. Normally ithere is a 15 second delay in providing the elevators emergency power and going back to normal power. Coordinate with Otis so the elevators can carry a full load as well. Freight Elevators 1, 2, 3: 88. Add lights in the machine room. Needs to be 19 foot candles. As a rule of thumb, no shadows is an indication there is enough light. May warrant some on wast wall depending on the relocation of freight door controllers. Lights over the elevator one machine and controller are the minimum needed. 89. Provide hot telephone lines per machine room. Confirm Otis has line sharing devices so it is only one line per machine room or a per line per phone. (4 in machine room, // 3 elevators and one emergency phone. 90. Add lights in each elevator pit to meet the 10 foot candles required by code. 91. Fill in the hole in the floor of the machine room. 92. The electrician has to provide and install NEC labels or tags on each disconnect identifying the distribution source location. 93. Fire caulk the machine room. 94. Finish the air conditioning for the room. Make sure it is operational prior to final inspection. 95. The small pocket at the ceiling level on the west machine room wall warrants a smoke detector or soffit it off to eliminate the pocket all together. 96. Ceiling will require a minimum of six smoke heads due to the pockets created by the structural beams. 13 OF 16 T-2822 FENWAY

14 97. Identify the disconnect on the south wall. We believe it to be the emergency generator transfer but warrants identification early on. Use signage to identify. 98. Finish installing the sill threasholds on floors 2, 3, Provide the walkies or temp wiring for construction use activity. Elevator Add lights in the machine room. Needs to be 19 foot candles. As a rule of thumb, no shadows at the floor is an indication there is enough light Replace the existing receptacle in the machine room with a GFI per code. Remove temp GFI that is no longer used in the room including the temporary wiring going to it Provide a closure device for the machine room door Remove temporary wiring in the machine room and patch the hole where it enters the room Provide hot telephone lines per machine room. Confirm Otis has line sharing devices so it is only one line per machine room or a per line per phone. Must use Epoxy adhesive per specs to maintain the intended life cycle of the floor due to cleaning parameters. Elevators 6 & Grout sills on all floors Change primary walk in pit door. 15 112. Provide 20 amp fuses in the 110 power disconnects. And provide the 110 power, currently off Add pit lighting to meet 10 foot candles of light per pit Electrician needs to provide the NEC data tags identifying the distribution panel location Provide plastic tubes on all the machine room florescent lighting or provide a guard over them Finish the air conditioning to proper working order Provide hot telephone lines per machine room. Elevators 8 & Fire caulk machine room. Includes the #9 oil line penetration into the hoistway Lower lights in machine room. Add as needed to get to a 19 foot candle level. Provide plastic tubes over the light tubes or provide a guard over the tubes The 110 power was not hot. Provide the 110 power, currently off Add pit lighting to meet 10 foot candles of light per pit Electrician needs to provide the NEC data tags identifying the distribution panel location Provide plastic tubes on all the machine room florescent lighting or provide a guard over them The oil cooler has a toggle switch but it needs a lock out tag out collar Finish the air conditioning to proper working order Provide hot telephone lines per machine room. Currently terminated by the machine room door, has to go to the controllers. 16 130. Put a switch on the walk in pit door to stop the elevator when opened. Otis needs to interface with safety circuit. Needed due to toe guard extending down into door way when car comes into lowest landing. Escalators Grout in and around the trusses at the top ends Grout in and around the trusses at the bottom ends Wrap underbelly of the units, do NOT drill into the bottom pan of the truss There is a missing small piece of sheetrock on both sides at the bottom newell. Must be enclosed prior to any final inspection Install the bollards at the top of the down unit number 4 to prevent carts from accidently being pushed down the escalator. Carteyors 5 & Schedule Pflow to finish the installation. They will need a minimum of ten steps to set up the units Provide the light and GFI in each pit.
