FTN Associates, Ltd. Monitoring Well Installation Report Entergy Arkansas, Inc., Entergy Independence Plant Class 3N Landfill Permit No S3N-R1, AFIN: FTN No. R On behalf of Entergy Arkansas, Inc. (Entergy), FTN Associates, Ltd. (FTN) is providing this memorandum report for the Entergy Independence Plant Class 3N Landfill in Newark, Arkansas. The memorandum presents a summary of field activities associated with the installation of five groundwater monitoring wells as required by Arkansas Pollution Control and Ecology Commission (APCEC) Regulation No. 22, (f), to improve the existing monitoring well network. 1.0 BACKGROUND FTN submitted a revised work plan dated December 21, 2012 (Arkansas Department of Environmental Quality [ADEQ] Document Identification Number [DIN] 63675), to install five monitoring wells that are generally evenly distributed around the perimeter of Phases 1 and 2 of the landfill and that represent groundwater quality at the relevant point of compliance as directed by APCEC Regulation No (a)(2). In a letter to Entergy dated March 6, 2013, the Solid Waste Management District (SWMD) of ADEQ granted conditional approval of the work plan for the new well installation (ADEQ DIN 64017). 2.0 FIELD ACTIVITIES Five new monitoring wells were installed during July 15-24, Depths of these wells range between 41 and 42 ft below ground surface (bgs). Drilling and well construction was performed by Tri-State Testing Services (Tri-State), an Arkansas-licensed driller, with an FTN registered

4 Mr. Bill Sadler, P.G. August 30, 2013 Page 2 professional geologist onsite to document field activities. Procedures adhered to methods described in Handbook of Suggested Practices for the Design and Installation of Ground-Water Monitoring Wells (US Environmental Protection Agency [EPA], 1991) and follow the requirements of the Arkansas Water Well Construction Commission (AWWCC). Boreholes were advanced through subsurface materials using an 8 ¼-inch (OD), continuous flight hollow-stem auger (HSA). Continuous samples were collected with a 5-ft continuous sampler, and soils were logged and classified according to the Unified Soils Classification System (USCS). A site map showing monitoring well locations is included as Attachment 1, and boring logs are included in Attachment 2. Wells were constructed using 2-inch diameter, flush-threaded, solid Schedule 40 PVC riser and a 10-foot, inch slotted, flush-threaded well screens. Well construction details are included as Attachment 3. Heaving sands and gravels were encountered during well installations and caused natural formation material to cover portions of the well screens. The annular space around unaffected portions of well screens were subsequently filled with silica sand during HSA, casing removal and the filter pack was tamped into place to verify proper compaction. A bentonite pellet seal was placed on top of the filter pack followed by a well-mixed, bentonite-cement grout. Monitoring wells were secured at the surface with 4-inch, locking, protective casings and concrete pads, and four protective bollards. An alpha-numeric label was assigned to each well installation. FTN developed the wells on July 17-24, 2013, in general accordance with EPA guidance (EPA 1991). Wells were mechanically surged and over pumped, which is typically sufficient to repair aquifer damage from the drilling operations and to restore the basic physical characteristics of aquifer. However, due to the natural filter pack material, a greater well development effort was necessary to restore formation hydraulic conductivity and yield relatively representative samples having low turbidity values with readings of 10 NTUs or less. The only well installation location that needed adjustment due to field conditions was well location 703S. The proposed well location 703S was located on the west side of the railroad tracks. Due to difficult access to the location, the well had to be installed to the east of the railroad tracks. The installation location for 703S was less than ft from the proposed well installation location, as required by SWMD conditional approval letter. 3.0 MONITORING WELL SURVEY Existing monitoring wells were resurveyed and new wells were located horizontally and vertically following completion by Harmon Surveying Inc. out of Conway, Arkansas. Horizontal coordinates are based on Arkansas State Plane North American Datum (NAD) of 1983 North and vertical control is based on Entergy s site referenced coordinate system, which is tied to the North American Vertical Datum (NAVD) of There is a ft conversion
