Landlords (lessors) and sellers are also required to distribute an educational pamphlet and include standard warning language in their leases or sales contracts to ensure that parents have the information they need to protect their children from lead-based paint hazards. This report should be read in its entirety to fully understand the scope of this survey.

2 July 17, 2018 Montrose County 949 N. 2 nd Street Montrose, CO Attn: Mr. Kenneth Norris, PE, MBA Re: HUD Lead-Based Paint Inspection LaSalle Road Montrose, Colorado Project Number: Dear Mr. Norris: Empirical Environmental, LLC (Empirical) is pleased to submit this Lead-Based Paint (LBP) Inspection Report relative to the above-referenced site. This LBP inspection was performed on June 20, 2018 at the residence located at LaSalle Road in Montrose, Colorado, (subject site) by Ms. Sheila D. Sealander of Empirical (Colorado Lead Risk Assessor Certification number 8950). According to the Montrose County Assessor s records, the original structure was estimated as being built in Between 1950 and 1976 the porches were enclosed and an addition was added and brick veneer was placed over the exterior wood siding. The residence is reportedly owned by Montrose County. This inspection was performed in accordance with the U.S. Department of Housing and Urban Development s (HUD s) Guidelines for the Evaluation and Control of Lead-Based Paint Hazard s in Housing, Chapter 7, dated June 1995, revised 2012 and the State of Colorado s Department of Public Health and Environment Regulation 19. LBP is defined in the Residential Lead-Based Paint Hazard Reduction Act (also known as Title X) as paint, varnish, shellac, or other coatings on surfaces that contain more than 1.0 mg/cm 2 (milligrams per square centimeter) of lead or more than 0.5 percent lead by weight. Lead can be present in paint at lower levels and not be defined as lead-based paint. Generally, leaded paint or paint containing lead is present if surfaces contain more than 0.06 percent lead by weight, but less than 0.5 percent lead by weight. For this type of measurement, percent lead by weight, paint chip sampling is required. Paint chip sampling is destructive and performed only at the client s request.

Project Number July 17, 2018 Paint conditions have been included and are defined by HUD as: (1) Intact: Entire surface area is intact; (2) Fair: Less than or equal to 10 percent of the total painted surface area of the component is deteriorated; or less than or equal to 2 square feet of the total surface area has deteriorated paint; or less than or equal to 20 square feet of the total surface area on large exterior surfaces and (3) Poor: More than 10 percent of the total painted surface area of the component is deteriorated; or more than 2 square feet of the total surface area has deteriorated paint; or more than 20 square feet of the total surface area on large exterior surfaces. LBP with a fair condition should be repaired and/or monitored, but is not considered to be a lead-based paint hazard as defined by Title X. LBP surfaces in poor condition are considered to be lead-based paint hazards as defined by Title X and should be addressed through abatement or interim controls. Sampling was performed using a Niton XLi-702a Series Lead Analyzer (Serial Number 14719) that utilizes X-Ray Fluorescence (XRF) technology. The XRF s testing mode was set at K & L Spectra Mode during this inspection and the assay (source) date was February 15, This type of sampling is non-destructive. Empirical calibrated the XRF according to manufacturer s directions prior to and at the end of the inspection. Substrate correction is not recommended for this XRF instrument. Empirical was requested to sample the painted surfaces on the interior and exterior of the subject site by Mr. Keith Laube, Montrose County Public Works Director. The subject site consisted of a single level residence with a cellar and small basement utility room. Surfaces included in this survey were walls, door jambs, doors, baseboards, window components, and other painted surfaces. The observed painted surfaces at the subject site appeared to be in intact or fair condition. Untested surface conditions are not included in this report. Paint conditions for tested surfaces are listed in Table 1, Appendix A. Onehundred and forty-nine XRF readings were taken for this inspection; six (6) were for calibration purposes, 20 were null or incomplete readings, and 123 were at selected locations at the subject site. XRF testing started at approximately 0840 and ended at approximately 1040 on June 20, At the time of this inspection the sky was clear and the temperature was approximately 75 Fahrenheit. One-hundred and nine XRF readings were taken on the interior of the residence. Onehundred of these XRF readings registered below 1.0 mg/cm 2 and would not be considered LBP as defined by Title X. Nine (9) of these readings registered above 1.0 mg/cm 2 and would be considered LBP. These readings were located on the following locations: closet wall in the Laundry; the walls in Stair 2; the door at the bottom of Stair 2; the Cellar ceiling; and the wood wall in stair 1. Areas and components not specifically tested may contain Empirical Environmental, LLC 2

4 Montrose County LaSalle Rd. Project Number July 17, 2018 LBP. The XRF readings, locations, and paint conditions are enclosed in Table 1, Appendix A. A site diagram is enclosed as Figure 1 in Appendix B. Fourteen XRF readings were taken on the exterior of the residence. These XRF readings registered below 1.0 mg/cm 2 and would not be considered LBP as defined by Title X. It should be noted that beneath the brick it appeared there was a wood siding. This siding should be presumed to have LBP. The XRF readings, locations, and paint conditions are enclosed in Table 1, Appendix A. A site diagram is enclosed as Figure 1 in Appendix B. The Lead Risk Assessor s certification and Empirical s Lead Firm certification are included in Appendix C. A copy of this report must be provided to new lessees (tenants) and purchasers of this property under Federal law (24 CFR part 35 and 40 CFR part 745) before they become obligated under a lease or sales contract. The complete report must also be provided to new purchasers and it must be made available to new tenants. Landlords (lessors) and sellers are also required to distribute an educational pamphlet and include standard warning language in their leases or sales contracts to ensure that parents have the information they need to protect their children from lead-based paint hazards. Contractors performing renovations that impact LBP will need to be certified through the EPA s Renovation, Repair and Painting (RRP) program. In addition to this certification, the renovation firm will need to be certified as well. If federal money is used to renovate this house, a HUD Clearance test will need to be performed after renovations are complete and before residents are allowed occupancy in the work areas. Please note, the Occupational Safety and Health Administration s (OSHA s) regulation 29 CFR applies to construction work where an employee may be occupationally exposed to lead. OSHA does not recognize lead levels in paint, but focuses on lead levels in the ambient air during construction. Therefore, OSHA applies during construction activities that impact Lead-Containing Paint (LCP) as well as LBP. Paint chip samples should be collected and analyzed to provide information regarding LCP. The analyses and opinions expressed in this report are based upon data collected along with other information described in this report. This report does not reflect variation, which may occur across the site. Actual conditions may vary and may not become evident without further sampling. Additionally, lead has been reported in toys, jewelry, home remedies, tile and bathroom fixtures such as tubs and sinks. The HUD Guidelines do not require testing of these items; therefore, they were not included in this inspection. It should Empirical Environmental, LLC 3

This report was prepared for the exclusive use of Montrose County with specific application to the site and it has been prepared in accordance with generally accepted environmental engineering practices.
