1 Addendum No. TWO Date: Project: HICKORY GROVE RE-PAVING OF DRIVES & PARKING FOR THE JEFFERSON COUNTY HOUSING AUTHORITY, BIRMINGHAM, ALABAMA MCKEE PROJECT NO The following modifications/ changes and/or substitutions to the plans and specifications are hereby made a part of same and are incorporated in full force as part of the contract documents. Bidders shall acknowledge receipt of this Addendum in writing on their Proposal Form. A1.1 GENERAL MODIFICATIONS: A. Upon review of the Pre-Qualification Packages, the Following General Contractors are approved for bidding the project. 1. The Pennington Group 2. Southeastern Sealcoating B. Refer to Project Manual Advertisement, Form: Bid for Lump Sum Contracts and other locations project allowable time. Note that the allowable time for completion of work is hereby changed to 100 consecutive calendar days from date of the Notice to Proceed. The Owner will take weather delays into consideration, upon receipt of documentation by the General Contractor. Documentation is due no later than 10 days following the weather event. A1.2 SPECIFICATION MODIFICATIONS: A. Bid for Lump Sum Contracts: See the Attached revised document This version is to be used for bidding. 1. Note the revised description of Unit Price #2. 2. Note the revised allowable complete time, increased to 100 days. A1.3 DRAWING MODIFICATIONS: A. The Following Drawings are hereby replaced with the attached revised drawings. 1. Civil Drawing C2.0. with revision date A1.4 CLARIFICATIONS: A. Weather Delays: The Owner will take weather delays into consideration, upon receipt of documentation by the General Contractor. Documentation is due no later than 10 days following the weather event. The below shall define the determination of a weather delay day. Other guidelines of work, found with in the specifications, shall also be adhered to. Page 1 of 2

2 Asphalt Work shall be considered delayed if the Temperature falls below 40 degree Fahrenheit. Concrete work shall be considered delayed if the Temperature falls below 35 degrees. Each day of a rain event shall be considered a weather day. Following a rain event day or series of days, only one additional day (with no rain) shall be considered a delay. Following the completion of a rain event the contractor shall make efforts to drain standing water by pumping or other means, to allow soils to dry. 3 U.S. Department of Housing and Urban Development Place Date Project No. HUD-4238-C (8-66) Proposal of (hereinafter called Bidder ) (a (State) corporation/ a partnership/ an individual doing business as ) (STRIKE OUT INAPPLICABLE TERMS) To The Jefferson County Housing Authority, Birmingham, Alabama, (hereinafter called Owner Gentleman: The Bidder, in compliance with your invitation for bids for the construction of: REPAVING PROJECT AT THE HICKORY GROVE PUBLIC HOUSING COMMUNITY (RE-BID NOVEMBER 2018) FOR THE JEFFERSON COUNTY HOUSING AUTHORITY, BIRMINGHAM, ALABAMA PROJECT NO Having examined the plans and specifications with related documents and the site of the proposed work, and being familiar with all of the conditions surrounding the construction of the proposed project including the availability of materials and labor, hereby proposes to furnish all labor, materials, and supplies, and to construct the project in accordance with the Contract Documents, within the time set fourth therein, and at the prices stated below. These prices are to cover all expenses incurred in performing the work required under the Contract Documents, of which this proposal is a part. Bidder hereby agrees to commence work under this contract on or before a date to be specified in written Notice to Proceed of the Owner and to fully complete the project within 100 consecutive calendar days thereafter per project as stipulated in the specifications. Work shall only occur Monday thru Friday 8:00 am to 5:00 pm. No work is to occur on U.S. holidays. Note: Nonworking days are to be counted in the total 90 consecutive calendar day limit. Work shall proceed as indicated in the Phasing Plan as indicated on sheet C1.0. Bidder further agrees to pay as liqui damages, the sum of $ for each consecutive calendar day thereafter as hereinafter provided in Paragraph 33 of the General Conditions. Bidder acknowledges receipt of the following addendum: UNIT PRICES: The below Unit Prices establishe Unit Prices that the Owner can delete or add quantities from the Contract as needed. However, all of the quantities listed below should be included in the base bid, and if the quantities are not used, payment for the quantities specified will not be issued to the contractor, and will be deducted from the base bid amount. UNIT PRICE #1 CURB & GUTTER: For the removal and installation of 200 Linear Feet (LF) of new concrete curb and gutters and valley gutters as detailed on the Civil Drawings and at locations selected by the Owner. ( LF of curb replacement identified in the plans is included within this Unit Pricing total of 200 LF): 200 per LF equals total dollars of $ (Included in Base Bid) MCKEE PROJECT # PAGE 1 OF 2

4 HUD-4238-C (8-66) UNIT PRICE #2 - UNDERCUTTING: 700 Cubic Yards measured in-place (CYMIP) of undercutting. Undercutting includes the removal of existing material, haul off and disposal and replacement with ALDOT #57 STONE, #67 STONE, Structural Fill, Dense Grade Base Or as directed by the Geotechnical Engineer. No additional payment is allowed for differing replacement materials. The establishment of need for undercut and the measurement of undercut must be performed by the Owner's Geotechnical representative. ( CYMIP of undercutting identified in the plans is included within this Unit Pricing total of 700 CYMIP, refer to Civil Drawings.): 700 per CYMIP equals total dollars of $ (Included in Base Bid) UNIT PRICE #3 BIAXIAL GEOGRID: 270 Square Yards of biaxial geogrid, such as Tensar BX1100, installed in place. Locate per the Geo-Technical Engineers recommendation. This Unit Price does not include any areas indicated on the plans. 270 per SYMIP equals total dollars of $ (Included in Base Bid) BASE BID PROPOSAL: Bidder agrees to perform all of the work described in the specifications and shown on the plans including the Unit Prices listed above for the sum of: Dollars $ WORDS FIGURES (Amount shall be shown in both words and figures. In case of discrepancy the amount shown in words will govern). Bidder understands that the Owner reserves the right to reject any or all bids and to waive any informalities in the bidding. The bidder agrees that this bid shall be good and may not be withdrawn for a period of 60 calendar days after the scheduled closing time for receiving bids. Upon receipt of written notice of the acceptance of this bid, Bidder will execute the formal contract attached within 10 days and deliver a Surety Bond or Bonds as required by Paragraph 10 of instructions to Bidders. The bid security attached in the sum of ($ ) is to become the property of the Owner in the event the contract and bond are not executed within the time above set forth, as liqui damages for the delay and additional expense to the Owner caused thereby.
