Included in the attachment is an adendum notification which details the changes the respective proposal pages which were added / or changed. Except for new bid insert pages, it is unnecessary to return any of the pages attached. Bid insert pages must be returned with the bid proposal submitted to the Department, unless your firm is submitting a bid using a computer print out. The computer print out must be changed to reflect the new bid item information. Contractors material suppliers, etc. who have previously been furnished informational proposals are not being furnished a copy of the addendum. If you have a subcontractor on the above project, please advise them of this addendum. Item 7, Legal Relations Responsibilities All motor vehicle equipment having an obstructed view to the rear shall have a reverse signal alarm audible above the surrounding noise level. Item 8, Prosecution Progress Submit the sequence of work estimated progress schedule on paper or as a Portable Document Format (PDF) electronic file compatible with Adobe Systems Incorporated Acrobat Reader X. Upon completion of pavement repair operations, the Engineer may suspend work working day charges under the following circumstances: Until completion of crushing operations. From November, 205 to March 3, 206. Item 9, Measurement Payment Provide a conversion rate for units of payment for work subcontracted to disadvantaged business enterprises if units of payments differ from those shown on the plans. The progress payment period shall end two working days before the last working day of the month. Deliver invoices to be paid as material on h on or before the end of the progress payment period. The target value for the desired percent by weight of residual bitumen coating for virgin limestone aggregate is.2%. If using aggregate other than virgin limestone, notify the Engineer prior to pre-coating. The Engineer will determine the target value for the percent residual bitumen coating for non-limestone aggregate. Item 320, Equipment for Asphalt Concrete Pavement Provide production equipment that ensures a uniform continuous production rate of at least 50 tons per hour. A Type D Structure is required. Item 344, Superpave Mixtures Longitudinal drop-offs will not be allowed when work is suspended outside of the paving season. Liquid antistripping agents are not allowed. Do not dump reload hot mix asphalt. Should the paving operation stop three times in one day due to equipment malfunction or mixture flow interruption, the Engineer may require the Contractor to immediately suspend operations until the next working day. Hauling equipment is subject to weight verification. If a limestone material source is used, add a minimum of % lime conforming to the requirements of Item 30, Asphalt Antistripping Agents. Testing in accordance with method Tex-530-C is waived. Design a mixture with a gradation that has stone-on-stone contact passes below the reference zone shown in Table 9. Verify stone-on-stone contact using the method given in the Superpave design procedure in Tex-204-F, Part IV. Unless otherwise directed by the Engineer, a tack coat is required. Item 35, Flexible Pavement Structure Repair, Item 354, Planing Texturing Pavement Remove dispose of existing raised pavement markers, jiggle bars, traffic buttons before beginning work. Take measures to prevent reclaimed asphalt pavement (RAP) from entering storm drain grates, inlets waterways. The Department will retain ownership of all removed asphalt concrete pavement. Stockpile reclaimed asphalt pavement at the intersection of SH 58 US 87. Maintain approved sediment control measures around the stockpile of reclaimed asphalt pavement (RAP) material at all times. This shall not be paid for directly but shall be considered as included in payment for this item. Item 502, Barricades, Signs Traffic Hling The contractor force account Safety Contingency that has been established for this project is for work zone enhancements that were unforeseen in the project planning design stage, but would improve the effectiveness of the traffic control plan. These enhancements will be mutually agreed upon by the Engineer the Contractor s Responsible Person based on weekly or more frequent traffic management reviews on the project. The Engineer may choose to use existing bid items if doing so does not slow implementation of work zone enhancements. Furnish regulatory speed limit signs. The Engineer will determine placement locations will provide supervision to the Contractor in placing, removing replacing these signs. When flaggers are used to control traffic, furnish install signs CW20-7 FLAGGER SYMBOL, CW20-7aD "FLAGGER AHEAD", CW3-4 BE PREPARED TO STOP. Flaggers shall use 24 in. STOP/SLOW paddles. Remove Type III barricades plastic drums upon depletion of a stockpile. At Contractor s option, place these items near the fence line. Warning reflectors mounted on plastic drums may be substituted in place of Type C steady burn warning lights. Prior to each work day, make provisions to exclude vehicles from parking within work areas. Omit advance warning signs furnish install reduced size signs CW20- ROAD WORK AHEAD mounted back to back with reduced size signs G20-2 END ROAD WORK signs at low volume crossroads, which are: Allen Rd., Flowers Rd., Overton Rd., McDonald Rd., County Road 4-P, Parramore Lane, County Road 20-P, Kinnebrew L, County Road 403-P, Hunt Rd. 4 th Street. Furnish install signs CW20-D ROAD WORK AHEAD, G20-aT ROAD WORK NEXT X MILES, NEXT X MILES, G20-2 END ROAD WORK at high volume crossroads, which are: FM 82, SH 63, SH 58 Use a shadow vehicle with a truck-mounted attenuator in advance of work areas when installing short line specialty markings (yield triangles, stop bars, arrows, words, numbers, diagonals on isls). Place cones around the perimeter of the work area, from the shadow vehicle to the end of work area. Provide duplicate warning signs on the median side of divided roadways. In addition to providing a Contractor s Responsible Person a phone number for emergency contact, have employee(s) available to respond on the project for emergencies for taking corrective measures within 30 minutes. Cones may be used as the typical channelizing device. Item 504, Field Office Laboratory Furnish one Type D structure. Asphalt content will be determined by the ignition method. A nuclear gauge will be used for moisture /or density determination. Item 506, Temporary Erosion, Sedimentation, Environmental Controls General Notes Sheet E

Exempt projects are those that disturb less than one acre or routine maintenance activities that maintain the original line grade, hydraulic capacity, or original purposes of the site. No temporary erosion control measures or Storm Water Pollution Prevention Plan (SW3P) have been included in the plans. Item 533, Shoulder Texturing Perform shoulder texturing prior to placement of final edge line pavement markings. Item 585, Ride Quality for Pavement Surfaces Schedule 3 from Table will be used to determine the level of bonus for each 0.-mi. section on the project. Item 3004, Diamond Grinding Grooving Pavement Perform full-width diamond grinding for payment at the beginning ending points of paving, where abutting concrete bridge decks or approach slabs, where abutting ends of concrete pavement, other locations as directed. Typical diamond grinding length shall be 00 ft. This work will not be required at those locations where Engineer determines that ride quality is acceptable.
