1 Project: DIALYSIS RENOVATION - 2 NORTH Baptist Beaumont Hospital Addendum No. 2 Date: All bidders are herewith notified of the following additions, deletions, changes or clarifications to the drawings dated and shall be acknowledged as received on the proposal. 1. Baptist Hospital has added Construction Zone as an approved General Contractor to bid this project (409) Baptist Hospital has pre-qualified the following subcontractors to bid on this project: a. Electrical Contractors i. Crown Electric (409) ii. Jefferson Electric (409) iii. Miller Electric (409) b. Plumbing Contractors i. Plumbing Specialties (409) ii. Premium Plumbing (409) c. Mechanical Contractors i. Calcasieu Mechanical (337) ii. HB Mechanical (281) iii. Metal Industries (409) The third floor roof is a modified bitumen membrane. The roofing contractor responsible for warranty work is Pyramid Roofing. 4. HVAC Controls will be furnished and installed by Johnson Controls (JCI) under direct contract with Baptist Hospital. Contractor shall coordinate with JCI. 5. Nurse Call will be furnished and installed by Executone under direct contract with Baptist Hospital. 2 6. Fire Alarm is Edwards, and shall be modified as required under the electrical contractor s work. The fire alarm system is maintained by Southwest Building Systems. 7. Refer to attached supplemental drawings SKE-1 and SKE-2 for additional electrical work required to provide the additional critical circuits required. Electrical contractors shall field verify with Facilities Department the location to tie into the 480V feed. 8. Remove lay-in ceiling in Electrical Closet. Seal all penetrations in gypsum board fire-proofing mounted on bottom of floor joists above, including new supports for suspended transformer noted in item 7. Seal all penetrations through surrounding partitions to deck. This electrical room will not be receiving a new ceiling. Relocate sprinkler head(s) and fire alarm devices as required to meet code. Existing wall mounted light fixture is to remain. 9. The Owner will purchase and furnish to Contractor to install eight (8) black-out roller shades. Purchasing of shades is not in contract. 10.See attached Infection Control Risk Assessment (5 pages) for ICRA requirements associated with this project. 11.Floor in Soiled Utility Room in NICU Nursery on Floor 3 will be replaced by others under a separate contract after the ducts have been installed and the walls furred. Floor replacement for the Soiled Utility is not in this contract. All other patch work is in this contract. 12.The existing basket weave vinyl wall covering shall be removed and walls skimmed and painted on existing partitions noted to remain within the Dialysis Suite. Contractor shall make a clean break in the existing Corridor 219 / Existing Nurse Station 200 between the vinyl wall covering to remain and the painted drywall partitions at an interior corner. 13.Electronic Keypad Locks shall be Trilogy Electronics Digital Locks, DL2700 Series with Best core to match building standard. 14.Mechanical Contractor shall furnish and install a Lamiflow Technologies Model LN102 airflow indicator adjacent to the Isolation Room Door at 84 AFF to the bottom of the device Pine Street, Suite Ronald M.
