2 Page 1 of 8 RFI QUESTIONS AND ANSWERS RFP no /BD Project: Primart Vault Renovation Addendum 2, February 24, 2017 RFI No Questions Question 1: Is there an estimate of hours for UTPD officers the GC should carry in their bid proposal and please confirm the current hourly rate for UTPD officers. Please advise. Answer 1: Contractor should include 80 $42/hr in the bid for UTPD officer support. Upon conclusion of the activities requiring the officers, the actual time will be reconciled against the hours specified herein, and an appropriate change order issued for the difference. Question 2: During the pre-bid 2/16/17 it was discussed that noisy work including hammer drilling, coring, and concrete cutting needs to take place after hours and weekend. Please advise what times during the week and during weekends this work can take place. Also advise if any other work needs to take place after hours. Answer 2: Noisy work, or work which affects public areas utilized by staff and patients may be conducted between the hours of 7 PM and 6 AM, Monday through Friday. Such work may be performed continuously from 7 PM on Friday until 6 AM on Monday. Areas in which such work is performed must be clean and ready for patients and staff at the start of business each day. Question 3: Please confirm that traffic control will be handled by the GC. How many deliveries will the contractor need to provide assistance for traffic control and floor protection? Answer 3: Traffic control will be the responsibility of the General Contractor. Three setups are anticipated one to receive the piece of equipment itself, one to rig in medical sources, and one to rig out medical source equipment.

3 Page 2 of 8 Question 4: Will floor protection need to be carried by the GC for the rigging path? What floor protection materials would need to be utilized and how many deliveries will the contractor need to provide assistance for floor protection? Answer 4: The rigging contractor will be responsible for providing floor protection during all rigging activities. Question 5: Sheet A-600 calls for Roppe straight Mauve base (RBS). The Mauve has been discontinued for years and the hospital rarely uses straight base, especially over sheet vinyl. Would it be acceptable to substitute the color Fawn and coved base? Answer 5: Roppe Fawn coved base should be substituted for the straight Mauve base. Question 6: Sheet A-600 calls for the color of SVF1 to be Bouvardia. This color has been discontinued in the Armstrong Medintech line. Would it be acceptable to substitute Freesia Pearl, which is the newer building standard? Answer 6: Armstrong Medintech Freesia Pearl should be used in place of Bouvardia. Question 7: Sheet A-600 calls for TLW-42 to be Legacy Glass Mosaics, which has been discontinued. Please advise on a substitute. Answer 7: Use American Tile Supply Micron Porcelain tile, #904 Jade Polished in lieu of the Legacy Glass Mosaics product. Question 8: Please confirm lifting, hoisting, and rigging will be by Owner. Answer 8: All lifting, rigging and hoisting will be the responsibility of the Owner. Question 9: The HW and CW tie-in points that serve the new exam rooms is located in Corridor BB.4858 and runs above the existing waiting area that has to stay operational during Phase 1. In order to get HW and CW to Exam

4 Page 3 of 8 Room BB.4974 during Phase 1, please confirm that this above ceiling work will need to occur on nights/weekends. Answer 9: Because this work would affect a public area utilized by patients and staff, and because such areas must remain operational during construction, any above ceiling work would have to be conducted after hours on weekends. Refer to question #2, herein, for clarification of such hours. Question 10: Elevation 1/E2 on drawings E2 of the Elekta drawings show FB3 (Floor Box). Being that conduits are being run to control console overhead, is FB3 really needed at this location (i.e. should this be changed to a wall box/duct?) The Elekta drawings also do not define the WR designation which we are assuming is a wall duct. Please clarify if this wall duct should actually be coming from above ceiling down to below the countertop. Answer 10: The assertion that FB3 should be a wall box in lieu of a floor box is correct inasmuch as all cabling will run overhead instead of under the slab. The WR designation is intended to represent a wall duct which would extend to the ceiling plenum from the wall box referred to herein. Question 11: Note 10/E5.1 says to provide (1) 3 conduit from pull box to FB3 for Item 8 on Elekta drawing E1. Item 7 on Elekta drawing E1 says to also provide (1) 5 conduit to FB3. Does the (1) 3 conduit suffice for both of these or should note 10/E5.1 also include (1) 5 conduit and core through the vault wall? Answer 11: In lieu of the 5 conduit, provide for a 4 conduit. Depending on final agreement for conduit routing in the field, the size of the 4 conduit may be reduced. Question 12: Can the cores through vault walls be straight cores or do they need be cored at angles for shielding requirements? Please advise. Answer 12: Cores through vault walls can be straight in lieu of angles as long as the core is not located in the west wall of the vault. Any cores in the west wall of the vault will have to be angled. Question 13: There is ductwork, plumbing and electrical lines that will pass through the same wall the concrete is being cut to widen existing openings

5 Page 4 of 8 as shown on drawing A-101. Please advise if cores need to be provided at these locations. Answer 13: Additional coring is required for ductwork, plumbing and electrical lines that must pass through the vault walls. The steel plate that is scheduled to close the opening cannot be cut and used for passage of utilities. Question 14: Drawing P2.0 shows a new 3 sanitary line running down the middle of the new corridor. Due to the phasing plan and needing to keep access to Vault BB.4934 during the phase 1 work, we propose to move the sanitary line to be located within the exam rooms so that Exam Room BB.4974 can be operational after Phase 1. See below sketch and advise.

6 Page 5 of 8 Answer 14: This solution is acceptable. Question 15: All the HVAC work in the Vault BB.4942 is grayed out on M2.1. Please confirm there is no HVAC work in the vault with the exception of replacing the grilles.

7 Page 6 of 8 Answer 15: Correct. Existing ductwork is acceptable to be re-used. Existing fan coil unit will need to be re-balanced for updated airflow quantities. Question 16: What is the weight of the heaviest piece of equipment and what measures does the contractor need to include for protecting the flooring during transporting equipment to the vault. Answer 16: The heaviest piece of equipment weighs 40,000 pounds. The rigging contractor will be responsible for floor protection. Any floor damage that does occur will be repaired by the Contractor through an appropriately executed change order. Question 17: S-200 shows the approximate sawcutting and slab removal extent for new utilities. There is a random section cut (4/S-200) that is not defined by any lines. Is there additional sawcutting and slab removal in the vault for existing equipment removal? If so, can a rough area be defined for bidding purposes? Answer 17: Refer to Sheet S200, revision date 2/23/27, included with this document by reference, for extent of sawcutting required to remove the existing base frame. Question 18: FCU-2R looks to be installed within Phase 2 however serves grills installed in Phase 1. Please advise if FCU-2R shall be installed with in phase one. If the box remains where it is shown, will this work need to be during afterhours or on weekends? Answer 18: The fan coil unit would need to be installed in Phase 1, and the work associated with that installation would have to occur outside normal hours. See question #2 herein for a definition of the hours when such work can be performed. Question 19: Note 1 on sheet A-111 indicates for existing door to remain, and to refinish as required to like new condition. Please advise if the pump room BB.4866 door needs to be laminated and if the vault door just needs to be re-painted? Answer 19: The pump room door is to remain as is. The vault door shall be repainted.

8 Page 7 of 8 Question 20: If the existing equipment cables for the existing equipment do not reach the new location of the control console, please advise who will provide the new cables. Answer 20: If existing equipment cabling is of insufficient length to reach the new control console, MD Anderson will provide new cabling. Question 21: Temporary partitions are not shown on the documents but due to the phasing, could be quite extensive. Would the below sketches be acceptable? Answer 21: Temporary partitions proposed in the sketches below are acceptable.
