2 SUMMARY AKH Apparels Limited is established in two buildings. One is a six storied and the other is a four storied building belonging to the factory owner. Six storied building was constructed in 2008 and utilization was made from It has the floor area of 107,000 sq.ft and the height of the building is 65 ft. Four storied building was constructed in 2013 and still under finishing stage. The total floor are of the building is 40,000 sq.ft. and the height of the building is 40 ft. In a small shed housing, the transformer is attached to the six storied building. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. AKH Apparels Limited Page 2 of 7

3 FINDINGS AND RECOMMENDATION: Finding No. E-1 Category: Documentation SLD diagram do not match with the existing site condition. Recommendation: Management must have clear SLD and other drawings for understanding the total Electrical system of the factory. Reportedly drawings are under preparation by consultancy firm. 4 Finding No. E-2 Category: Transformer Room Transformer guarded with wire mesh fencing. Recommendation: Transformer may be separated from panels by constructing barrier walls. Remediation Timeframe: 3 Month Transformer room Finding No. E-3 Category: Transformer Room Oil cup below transformer breather is empty. Recommendation: Breather oil cup must be filled with transformer oil to required level as instructed by the manufacturer. Remediation Timeframe: immediate Transformer room. Finding No. E-4 Category: Transformer Room Cables from Secondary of transformer going to changeover are laid on concrete floor without protection. Recommendation: Construct a raised cable tray/duct to terminate the transformer output cables to changeover switch and provide covers made of non-combustible material to protect cables insulation from physical damage. 5 Finding No. E-5 Category: switch board and panels SPNMBC with live supply connection on the floor without enclosure Recommendation: SPNMCBs must be fixed on the MCB enclosure (DB) and enclosure should be fixed at a height of 1.8m from the finished floor level. Remediation Timeframe: Immediately. Live MCB on the floor Finding No. E-6 Category: Cables and cable support Wiring in flexible PVC pipes laid inside cable trench. (Typical) Recommendation: Wiring laid in cable trench must be protected in rigid conduits with bends and accessories. If flexible conduits are used for some specific reasons, it must be of industrial grade and should bear ample strength to withstand physical impacts. Remediation Timeframe: 1 Month Electrical room Finding No. E-7 Category: cables and cable support. Cable trench partially covered. (Typical) Recommendation: The cable trench must be tightly covered throughout its length to avoid physical damage to the cables from falling objects. 6 Finding No. E-8 Category: Generator Room Cables terminating to/from MCCB of generator and entering cable trench not protected. Recommendation: Cables terminating from MCCB of Generator and entering the trench must be protected by providing the raised duct with cover. Remediation Timeframe: 1 Month Generator Room Finding No. E-9 Category: Switch board and panels. Cable terminating from Bus Bar Trunking (BBT) in electrical shaft, extended to different levels (floors) are not supported (Typical). Recommendation: Cables extended from BBT breaker to distribution boards in various floors must be supported on trays/risers. Cables termination in BBT Finding No. E-10 Category: wiring Wires not protected Recommendation: wires passing through the wall or slab must be through the industrial graded conduit and where ever require junction box must be provided. (Typical) Recommendation: Wires drawn through flexible PVC conduit must be replaced with industrial graded conduit and fix with saddles to prevent possible physical damages of the wiring installation. Recommendation: Clear transparent sheet must be used to cover the bus bar of the distribution board to protect the possible physical contact of the electrician during maintenance. Ebonite sheet loosely fixed. Finding No. E-13 Category: wiring. Wiring exposed in the junction area. Recommendation: Wherever there is a crossing of wiring, junction boxes must be provided.
