2 SUMMARY The is housed in a nine-storied (G+8) building and one shed. The factory building, reportedly, constructed in The factory started its production in the same year. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Page 2 of 7

3 FINDINGS AND RECOMMENDATIONS Finding #: E- 1 TRANSFORMER ROOM Transformer not separated from other occupancy i.e. no separate fire rated room for the transformer. Construct a fire rated room for the transformers. Assign a qualified engineer to design a required transformer room according to BNBC, Section The transformer must be installed with barrier walls between transformer and other panels. The walls must be fire resistant and should have height up to the ceiling. The wall should have the provision for fitting necessary louvers and exhaust fans for ventilation and fire rated on required side. No dedicated transformer room Finding #: E- 2 Transformer LT cables laid on floor without protection. Install cable tray with protective cover to route and protect the LT cables safely i.e. protect the cable insulation from physical damage due to falling objects and stepping of maintenance personnel. 4 Finding #: E- 3 SERVICE LINE Excessive HT cable with acute bend not supported and protected; Both the HT and laid cables passed together. Install separate cable tray with protective cover for HT and LV cables; Latch the HT cable properly avoiding acute bend. HT and LT cables laid together not protected Finding #: E- 4 TRANSFORMER ROOM Silica gel in transformer breather contaminated with moisture as a result it s color has been changed. Isolate the transformer from the feeder and replace the silica gel or perform maintenance to remove moisture from it. Establish a routine maintenance checklist for the transformer to keep all the accessories of the transformer operable. Silica gel get moisturized Finding #: E- 5 Barrier/separators not installed between different phases of MCCB. Install separators between different phases of MCCB to avert flashover. 5 Finding #: E- 6 Panel base plates not installed i.e. panel not dust and vermin proof. Collect base plate for the panel. Make circular hole at the base plate of panel and provide cable gland according to the respective cable size for cable entry and exit so that the cables are not stressed on the sharp edges of the hole of panels. Provide covers (of noncombustible material) if any additional gap remains after installing cable glands to make the panel dust and vermin proof. 6 months Panel without base plate Finding #: E- 7 Electrical panel enclosure including it s door not connected to earth. Panel enclosure including its door must be connected to earth using green cables preferably earth braid so that the metallic door remains at zero potential all the time. Practice earth continuity test to insure earth continuity to panel and loads enclosure and keep record. Panel enclosure not connected to earth Finding #: E- 8 Higher rated protective device (MCCB) with lower rated output cable. Select the protective devices according to the connected cable size to be protected i.e. The rated current of protective devices (ACB, MCCB, MCB) does not exceed the current carrying capacities of the conductors. Use single (individual) cables from the bus bar to MCB input or use plug-in bus bar for MCB input to avoid loose connection and ease of maintenance work.. Loop connection among MCB input Finding #: E- 10 Protective device(s) mounted on wall without enclosures. Protective devices should be encased in metal casing made of 20 SWG thickness metal sheets. Cables must be protected from possible damage by panel edges or sharp objects by suitable means i.e. electrical graded rubber may be placed between the cables and panel sharp edge. 7 Finding #: E- 12 Combustible materials are kept close to tube light fitting attached to channel. Keep necessary clearance between the fabrics stored in the production floor and lighting fittings. Necessary steps should be taken by the authority to aware the workers to keep all kind of flammable materials and substances away from the electrical fittings and equipment to prevent spreading of fire hazard.
