Smith & Associates Date: April 17, 2015 Project: Spring Lake Improvement District Stormwater Treatment Area Subject: ADDENDUM NO. 002 Bidders are required to acknowledge receipt of this ADDENDUM on Page of the Contract Documents and by returning the acknowledgment on Page of the Contract Documents to our office immediately via to or via fax at Failure to do so may cause the bid to be considered informal or irregular and subject to rejection. The Contract Documents, Specifications and Drawings are hereby amended as follows: A. CONTRACT DOCUMENTS AND SPECIFICATIONS: 1. This addendum will be ed to pre-bid attendees and can be downloaded at the same CAS link where the bid documents were obtained as shown below In order to keep additional fill onsite, the access shell road will be raised by 2 feet. The slope from the shell road to the control elevation will be modified from 8:1 to 4:1 (Hor:Ver). The 8:1 slope below the control elevation shall remain. An estimated volume of 24,500 CY may be kept on site from this plan adjustment that would otherwise been hauled away from the project. 3. Bahia sod is to cover 3 of lake & marsh bank above the control elevation and the south berm. Hydroseeding is to be used beyond the 3 mark. 4. Any remaining muck not used for the marshes can be used for banks and berm. SLID Stormwater Treatment Area Addendum No / 2015

2 5. Bid prices for the Base Bid will be maintained for the duration of the Base Bid regardless of fuel cost increases. However, SLID will entertain a modification in the contract prices should a significant change in fuel prices exist between the time the Base Bid was submitted and when an Additive Alternate Bid (AAB) or a portion thereof is awarded using a FDOT indexes as a reference. 6. In order to minimize costs, impacts to County and State roads, and to help expedite the project, suggested placement of fill will be allowed in areas as designated by the attached Exhibits. The contractor is not obligated to use these properties. These areas where fill can be placed are SLID owned lands, private land within SLID boundaries which is adjacent to the project, unimproved section of the Sebring Regional Airport via access through the Davis property/sebring Ranches. The contractor is to incorporate this into the bid prices and is to assume all responsibility when performing this task which may include but not limited to construction of temporary haul road(s), culvert crossings, installation best management practices for sediment and erosion control purposes, coordination with SRA & private land owners (SLID will provide contact information), restoration of lands and impacted roads, etc. Should the contractor choose alternate locations other than the ones suggested by SLID, a description of these locations must be provided in the bid submittal. The material to be deposited on Airport property and on the Sebring Developers property is to be free of organics or muck. The material must be suitable for site development and meet the classification of select or plastic soil based upon FDOT specifications. Highly plastic and/or muck material will not be allowed. The Airport reserves the right to have the contractor remove unsuitable material, at his own expense. The Airport will coordinate through CAS field representative who will oversee this effort. The Airport reserves the right to visit the stockpile site. A haul route to the Airport has been provided in the attachment and is subject to change. Limitations on all of these locations may be encountered and SLID will reasonably work with the contractor to find a suitable solution of fill placement. 7. CAS has received confirmation that the County landfill will accept all uncontaminated fill as well. The contractor shall be responsible for obtaining testing parameters from the County and costs for testing of this material. No additional compensation shall be made should the Contractor choose this option. 8. The additive alternate bid schedule for FY2017 has bid items for wetland plants to be used as directed by SLID in the event marshes are not sufficiently colonized by the anticipated native wetland vegetative species. The suggested plant species are shown below. An equal number of each species will be planted in those areas not colonized by the native wetland vegetative species. The following questions were submitted by a bidder and responses are provided in italics as follows: Could you please provide a detailed description (coordinates, owner, and amount of fill to be stock piled) for the stock pile locations? General locations of placement of fill material has been provided via this addendum. Will any dewatering and/or de-mucking be necessary to transport the material to the designated stock pile locations? Means and methods has been and will be left up to the contractor so as long we have compliance with the SFWMD permit and FDEP NPDES requirements Will the stock pile locations be available for inspection prior to bid due date? Bidders can, within their means, inspect locations prior to the bid date. What is the average water elevation on the west side of the pump station? The design water elevation is at 37.0 NGVD and is controlled by the pump station. Will any silt fence be required for this project? Please refer to bid item no. 5 and descriptions. Will an NPDES or an SWPPP permit be required for the stock pile site locations? Please refer to bid item no. 5 and descriptions. Will there be any work hour and/or day per week limitations? Work hours are described in Section Will there be a required MOT plan for Duane Palmer Blvd? There is a bid item for MOT on the project and it is to cover the contractor s means and methods for an MOT. SLID Stormwater Treatment Area Addendum No / 2015

4 Can the existing canals be backfilled in the wet? If not, will compaction testing be required on a per-lift basis? The perimeter canals can be backfilled and compacted in the wet but testing for compaction will be performed once the filling reaches above elevation 37.0 NGVD. For dewatering purposes, what is the invert of the channel bottom west of the pump station? No answer can be provided for this question. The SLID pump station will not be used as a dewatering system. Technical Specification E States: Where muck is encountered at the boundary of the site, sheeting shall be installed and left in place to preclude future damage to the installed fill by adjacent muck. Is there an anticipated area where this may be required? No specific area is made available. Procedures are to be reasonably followed where muck is encountered. It appears that Cross Sections 4 and 5 do not show the existing elevations of the perimeter ditch. Can you provide clarification? The question appears to pertain to Section Cut nos Please refer to plan sheet C-03 to obtain perimeter ditch bottom elevations. Can you provide clarification on the Automatic Water Quality Samplers? Can a sample make and model be provided to be supplied by the contractor on an as equal basis? Bidders are to disregard this bid item. SLID will procure the samplers. It has been noted that dewatering activities can be conducted from the existing pump station. Are there any limitations on the use of this station? (example: time of day, duration, water elevations, etc.) CAS is not aware of any dewatering activity that will be conducted by the SLID pump station. Bidders are not to assume that the SLID pump station will be part of their dewatering system. SLID Stormwater Treatment Area Addendum No / 2015

5 No specific SWPPP exists in the plans. It is the responsibility of the contractor to create a SWPPP for the application of the NPDES NOI? Yes; hence the purpose for bid item nos. 5 & 6 (See table below from bid schedule). The contractor is in charge of its means and methods and must therefore produce these items. IN ADDITION TO THE ABOVE DESCRIPTION, THE LUMP SUM (LS) PRICE FOR THE STORMWATER POLLUTION PREVENTION PLAN (SWPPP), NATIONAL POLLUTION DISCHARE ELIMINATION SYSTEM (NPDES), & CONSTRUCTION DEWATERING PERMITTING SHALL INCLUDE BUT NOT BE LIMITED TO THE INSTALLATION OF TURBIDITY CURTAINS, SILT FENCES, SOD FOR EROSION CONTROL (NOT FOR FINAL) OR OTHER APPROPRIATE BEST MANAGEMENT PRACTICES TO CONTROL SEDIMENT & EROSION, NPDES PERMITTING, SWPPP PERIODIC MONITORING AND MAINTENANCE, & COMPLIANCE WITH SFWMD & FDEP PERMITS ISSUED FOR THE PROJECT. THE SWPPP PLAN & NPDES PERMIT WILL BE SUBMITTED AT THE PRE-CONSTRUCTION MEETING. CONTRACTOR IS TO ASSUME FULL RESPONSIBILITY WITH COMPLIANCE OF ALL SFWMD & FDEP PERMITS ISSUED FOR THE PROJECT PERTAINING TO SEDIMENT & EROSION CONTROL. 5 NPDES & SWPPP 1 LS 6 CONSTRUCTION DEWATERING (INCLUDING PERMITTING) 1 LS We are requesting a one week extension on the due date be issued, in order to be able to incorporate factors relating to the answers of the questions.
