Failure to do so may subject the Bidder to disqualification. This addendum consists of 2 pages with nine (8) attachments, totaling 10 pages. PRE-BID MEETING MINUTES The attached Pre-Bid Meeting Minutes and a copy of the meeting Sign-In Sheets (total of 6 pages) shall become a part of the Contract Documents. Items discussed at the meeting can be found within the meeting minutes. PROJECT DATES The following is a list of important dates for this project. Documents in the Project Manual are replaced with updated versions attached to this Addendum. Review of Bid Date, Time, and Format Bids are due on March 26, 2019 no later than 2:00 P.M.. in the office of Western Carolina Facilities Management Department, 3476 Old Cullowhee Road, Cullowhee, NC to the attention of Daniel Fiskeaux-Project Manager. Please be aware that overnight packages from FedEx and UPS are not always received at WCU prior to 2:00 p.m. and bidders should plan accordingly. Carefully follow the Instructions to Bidders included within the Project Manual. Use Bid/Acceptance Form in Project Manual. Submitted proposal must have original signatures. Photocopied signature sheets or faxed or ed proposals will not be considered. Provide two original hardcopies of bid in an opaque, sealed envelope, marked with the project name, bid date and time, SCO ID#, name of the bidder, bidder s contractor license number clearly printed on the outside of the envelope. Please clearly mark BID ENCLOSED and ATTN: DANIEL FISKEAUX on the outside of the bid envelope and on the front and back of any additional mailing envelopes such as UPS or FedEx. 4 Identification of Minority Business Participation and either Affidavit A or Affidavit B must be attached to the bid. Failure to properly complete and include the required MBE documentation may result in disqualification of the bid. Additional information can be found in the MBE Guidelines included within the Project Manual. Bid, Performance, and Payment bonds are NOT required for this project. It is the responsibility of the bidder to deliver their bid to the correct place/person, prior to the announced time for the bid opening. Bidders may contact Mr. Fiskeaux to confirm receipt of their bid. Bidders were asked to review directions regarding withdrawal of proposals. Bids will be received and opened in accordance with the requirements of the General Statutes of North Carolina. c. Obtaining Bid Documents: Bidders must fully examine the Project Documents and existing site conditions prior to submitting their bid. Bid Documents consist of the Project Manual and Project Drawings. One hardcopy of the design documents have been provided to attendees of the pre-bid meeting at no cost. Additional hardcopy sets may be requested from Atlas Engineering for a refundable plan deposit of $ Electronic copies of the documents may be requested from Atlas Engineering and are free of charge. Bidders are responsible for checking all drawing scales on electronic versions when using them with estimating software and are responsible for making sure that electronic sets are complete. Notify the Designer of any questions, concerns, errors as soon as possible, but no later than 7 days prior to the bid (by March 19th). The last addendum will be issued by 2:00 p.m. on March 20, Issue of Addendum No. 01 will include the pre-bid meeting minutes. Receipt of addenda must be marked on the Form of Proposal (after signatures). Bidders were reminded that they must be familiar with the State's General Conditions and Supplementary Conditions, included within the Project Manual. 3. Review of Form of Proposal: a. Fill out completely base bid, all bid alternates and unit costs. b. Base Bid cost must include estimated quantities of unit rate work per Section c. Bidders must write in "Add" or "Deduct" for Bid Alternates. d. Provide unit rates for listed items. Unit rates will be used in the event that the actual quantity of work performed is more, or less, than the estimate in the base bid. Meeting Minutes Pre-Bid Meeting Meeting Date: 3/12/ Page 2 of 5

5 e. Attach the required MBE forms and Bid Bond form. Forms have been included in the Project Manual with the Form of Proposal. If a contractor will self-perform the work and intends to submit Affidavit B, they must make sure to fill out the Identification of Minority Business Form with none and $0 (not blank) in the places provided. Bidders should take care to carefully follow the MBE requirements and contact the Owner, Designer, or State HUB personnel with any questions or concerns during the bidding process. f. The Owner reserves the unqualified right to reject bids if the bidder fails to insert a price for all of the bid items including the base bid, bid alternates, or unit rates requested, fails to mark addenda, or provides incomplete or incorrectly completed forms. Don t leave any spaces blank. g. The award of the contract will be made to the lowest responsible and responsive bidder on the basis of the base bid and bid alternate(s) accepted by the Owner. The bid alternates are not necessarily listed in a prioritized format and may be selected out of order based mainly on the available budget. 4. Review of Supplemental General Conditions: a. Submittals must be in PDF form unless physical samples are being submitted. Specify on shop drawings all materials to be used. b. Maintain on-site log of work that varies from contract documents. c. Daily logs d. Provide list of addresses and emergency telephone numbers for GC, key personnel, and all subcontractors. e. Background checks and sex-offender registry for contractor and subcontractors. f. Notify WCU of any accidents immediately g. Utility interruption, hot-work, egress, inspection certificates 5. Scope of Work: a. Examine Project Documents: Project Documents include the Drawings and Project Manual. Available online at WCU s website. b. Visiting the Site: Contractors to contact Daniel Fiskeaux to schedule additional site visits. c. Base Bid: Includes unit costs at the quantities listed in the Project Manual d. Bid Alternates: There are two bid alternates, described in the project manual. Do not forget to write in Add/Deduct in the table on the Bid Acceptance Form. e. Specified Coating System Requests for Substitution. Requests for substitution must be received by no later than 5:00 p.m. on March 19, 2019 to allow for time to review and issue applicable addenda. 6 6. Work Hours/Coordination Items: a. Contract Time: Work hours are from 7:00am to Sunset unless otherwise approved by WCU. Contractor may not mobilize to site prior to May 13. Contractor may visit site for the purposes of planning, measurements for creation of shop drawings, etc. if coordinated with WCU. b. Coordination with Building Schedule: Not necessary for the Tower, however coordination with events taking place on the field north of the tower may be necessary. c. Staging and Storage: Contractor to receive 8 parking spots from WCU for staging and parking. All staging areas to be fenced in if storing materials. See attached drawing for project area and staging allowed by WCU. d. Access to the Building Interior: Coordinate with WCU for obtaining a key to the lower access hatch to the Alumni Tower. 7. General/Owner Items: a. Deliveries: Ensure that drivers know exactly where to deliver materials to. Be mindful of difficulties turning around in the parking lot. b. Hot Work Permits: See WCU s policies in Project Manual c. Insurance Requirements: Ensure that your insurance certificate contains the Exact language shown in the Project Manual in the Supplemental General Conditions Section 1.07-B-h-1 and repeated below: Not-withstanding the preprinted cancellation provisions on this form, coverages afforded under the policies will not be cancelled, reduced in amount nor will any coverages be eliminated until at least thirty (30) days after mailing written notice, by certified mail, return receipt requested, to the insured and the owner, of such alteration or cancellation. d. Expectations: Contractor is expected to comply with all WCU policies. WCU has a zero-tolerance for harassment. e. Preservation of Existing Legacy Pavers: In two areas around the base of the tower contain Legacy engraved pavers. These pavers are to be protected to prevent damage during construction. 8. Questions from Pre-Bid Meeting: Q1: Is orange snow-fence suitable for fencing of the work area? A1: No, fencing will need to be movable, full-height, chain-link with weighted base plates. Q2: Can the contractor use just water for pressure washing of the tower? A2: If the contractor can achieve acceptable results and not damage the tower surface with just water, then pressure washing with just plain water is acceptable. 7 Q3: Do the welded wall brackets for the M3 repair need to be stainless steel? A3: No, Atlas will accept brackets that have been both galvanized and painted in leu of stainless steel brackets. Q4: Is water available on site? A4: Yes, water is available on the outside of the building directly to the west of the alumni tower. Q5: Is there electrical power available on site? A5: Yes, there is electrical outlets inside the Alumni Tower at Level 2 and Level 3. Q6: What is the expected restoration of landscaping required? A6: The grass can be repaired by leveling the grade and seed & straw. Seed is a special WCU blend available from a local supply store. Q7: Will WCU remediate the bird droppings on the top two levels of the tower? A7: Yes, WCU will remediate the bird dropping on the top of the tower prior to the start of the project. Q8: What is the extent of pressure washing required on the ground adjacent to the Alumni Tower? A8: Contractor is expected to pressure wash the area around the base of the tower that is inside the steps surrounding the base of the tower.
