ELECTRICAL SAFETY INSPECTION REPORT FASHION FORUM LTD , Diakhali, Zamgara, savar, Dhaka, Bangladesh. Factory List: 1. Fashion Forum Ltd. 2. IDS Washing Ltd. Inspected by: Karma Report Generated by: Khan Inspected on April 1, 2014

SUMMARY Fashion Forum Ltd., owns two seven storeyed building. The 1st seven storeyed building constructed from ground floor to 5th floor in 2006 and the top two floors completed in 2011 and the construction of 2nd seven storeyed building(g+6) is scheduled to complere in july,2014. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Fashion Forum Ltd. Page 2 of 6

FINDINGS AND RECOMMENDATIONS Finding #: E- 1 Cables entering or leaving the changeover switch panel not supported and protected. Install a vertical cable tray with protective cover from cable trench to changeover switch panel to support and protect the entering and leaving cables. Cable entering/leaving panel without support. Within 1 month Finding #: E- 2 Cables encased in flexible PVC conduit not supported and protected. Install a cable tray with protective cover at least two meter above the ground and route and protect the cables safely. Cables not supported and protected Fashion Forum Ltd. Page 3 of 6

Finding #: E- 3 Cables encased in PVC pipes not supported and routed properly. The rigid PVC (industrial graded) pipe used for surface wiring must be continuous through-out its length and properly supported (clamped with saddle clamp, at regular interval of 600 mm).the conduit shall run vertically or horizontally, shall never at angle. Wiring through PVC pipes not supported properly Finding #: E- 4 Main service cables from pole mounted distribution transformers not supported and protected properly. Install cable tray or ladder to support and protect the main service cables from pole mounted distribution transformer to main switch (MCCB) inside the factory building and protected against weather and physical damages. Main incoming service cable from REB not protected and supported properly Finding #: E- 5 Main service cables from LT panel to production not supported and protected. Cables must be carried in cable trays with protective cover from LT panel to production floors to protect them against possible physical stress/damages throughout its length. Main service cables for the production floors not protected Within 1 month Fashion Forum Ltd. Page 4 of 6

Finding #: E- 6 Section of cables between cable tray and BBT junction box not supported and acute bending. Rearrange the cables; cables should be terminated from one point to another in such a way that there will be no strain on the termination point. Cables should be supported or protected cable tray or conduit throughout it s whole length to protect it from atmosphere and physical damage. Section of the cables between cable tray and BBT junction box must be supported and protected by extending the cable tray or may be additionally supported. Cables not supported Finding #: E- 7 Steam pipe line, wiring encased rigid PVC pipes and unprotected distribution cables passed very close to each others. Keep necessary clearance between the steam pipe and electrical wiring and raceways to prevent the cable insulation from damage due to heat emitting from steam pipe. Cables should be carried in cable tray with protective cover to protect it from physical damage. Steam pipe and unprotected cables passed very close to each others Fashion Forum Ltd. Page 5 of 6

Finding #: E- 8 Distribution & LT Panels Remaining gaps/opening around the cables or BBT passing through wall are not sealed with fire rated materials. Cables should be encased in covered cable tray/rigid conduit passed through wall. Remaining gaps/opening around the cables or BBT passing through walls at different floors through the electrical shaft must be sealed with fire rated materials. Within 1 month BBT and cables passing through one floor to other floor; the remaining openings not sealed with fire rated materials Fashion Forum Ltd. Page 6 of 6
