1 ADDENDUM NO. 3 DATE: December 13, 2018 INVITATION FOR BIDS: M R- Bank Street Pedestrian & Bike Improvements (UPC ) DATED: November 2, 2018 BID DUE DATE: December 20, 2018 at 2:30 p.m. EST -CHANGE OPENING DATE: December 21, 2:30 p.m. EST -CHANGE Pre-Bid Date: December 6, 2:00 p.m. EST- CHANGE SUBJECT: Meeting Summary dated 12/06/2018, Agenda, Attendance Sheet, Questions, Responses &Clarifications, and Updated Bid Form Ladies/Gentlemen: Please take note of the following: Questions, Responses and Clarifications 1. Question #1: The project description for the additive bid in addendum number 1 references pavement markings, but there are none of these items are included on the bid form. Is this work to be included, and if so, can the bid form be revised to include these items? Response: In the base Project sheet, item are pavement markings. 2. Question #2: The bid forms have line items for 12 inch reinforced concrete pipe, however the plan sheet call for 15 inch RCP. Which size pipe is correct? Response Pipe size should be 15 inches. We will update the bid sheet In the base Project sheet, item are pavement markings 3. Question:#3: Item 6 on the additive bid form is for spot restoration of the brick crosswalk. Is the intent of this item only to replace damaged brick and the setting bed, or Is it to include the concrete base as well? Also, can the type, dimensions and finish of the granite bands be provided, if additional are needed? Finally, can the type of brick in the crosswalk be provided if additional brick is required? Response: I am fairly certain that we will not have any of the granite bands stashed away anywhere. We MIGHT have some extra bricks, but they m ay not match. 4. Question #4: Additive Portion bid item# 12- Remove existing fire hydrant and install new fire hydrant Response: Department of Public Utilities will do the work. This item will be taking out from the bid sheet. 5. Question #5: Item ## 27 Maintenance of traffic. Addendum #1 states that MOT is not required. Will the bid sheet be updated? Response: Yes. MOT is not required since the corridor is closed. WE will update the bid sheet. 6. Question #6: Is there a bid sheet for the additive bid? Or is the contractor required to perform takeoff for a 1 lump sum price? Response: There is a bid sheet for additive bid. There are two sheets in the unit price sheet: one is base Project; the other is additive part.

2 7. Question #7: Has there been any measures taken to get Veterans involved on this project? Response: OMBD has expressed a 12% commitment Question #8: Will the street be shut off completely (closed)? Response: Street will be closed on Bank Street from 9th to 11th Street. No open dates yet. 9. Question #9: What are the hours of work? Response: The hours of work are from 7:00 a.m. - 5:00 p.m. 10. Question #10: What is the due date? Response: The bid due date is December 20, 2:30 p.m. 11. Question #11: The Agenda has the 10th and the website has the 11th. Clarification needed. Response: Per Addendum #2, Dated November 28, 2016 vendor questions shall be received by2:30 p.m. on December 11, Vendor must take due notice and be governed accordingly.
