1 ADDENDUM 2 KODIAK ISLAND BOROUGH LIBRARY Kodiak, AK March 7, 2018 TO ALL PLAN HOLDERS OF RECORD: This addendum forms part of and modifies the contract documents as noted below. Bidders must acknowledge receipt of this addendum in the space provided on the Acknowledgement of Addenda form. Failure to do so may submit Bidder to disqualification. This addendum consists of 2 pages plus attachments. SPECIFICATIONS: Section Bid Form: Remove the Bid Form included in the specifications and replace it with the attached Bid Form. Section Supplementary Conditions: Delete 2.4(B) in its entirety and replace with: B. Add the following subparagraphs to Section : e. The Contractor and Subcontractor shall pay prevailing laborers and mechanics rates of pay and other requirements as required and defined in State of Alaska Statutes Title 36 Public Contracts. In accordance with provisions of AS these minimum rates of pay shall be prominently posted at job site by Contractor. f. All Contractors and Subcontractors will abide by all applicable labor-standards provisions of the Alaska Department of Labor with respect to requirements concerning maximum hours of work, minimum rates of pay, overtime compensation, categories and classifications of employees, methods and timeliness of payment of wages, the preparation and preservation of weekly payroll records, reporting requirements, and all other pertinent requirements. Section Unit Prices: Add the attached Unit Prices to the specifications. DRAWINGS: Sheet S200, Detail A, B, and C: Delete 4 Base Course and replace with 4 Base Course, extend 4 past wood foundation on all sides RESPONSE TO BIDDER QUESTIONS: QUESTION 1: At the Chiniak Library there was an above ground fuel tank that was damaged during the fire. Has screening and sampling already been done to determine that there is no soil or groundwater contamination? ANSWER 1: The fuel that was in this tank was pumped by KIB staff shortly after the fire. 2 QUESTION 2: Are there connections to water supply or septic system required? ANSWER 2: Septic is to be connected to existing septic tank as shown on sheet M101, detail 1. For bid purposes assume that the existing septic tank is in usable condition. Source of water supply has not yet been determined. New water line shall be stubbed out of building as shown on sheet M101, detail 2 and capped for future connection to water supply. QUESTION 3: The specifications say the Owner will pay for electric power service during construction. Is that correct? ANSWER 3: Yes. Owner will be coordinating installation of temporary electric power service for Contractor s use, and Owner will be paying for all charges during construction. QUESTION 4: Sheet S001 says to excavate undesirable materials, including organics and ash. Is there any geotechnical information available so we know how much excavation will be required? ANSWER 4: No geotechnical information is available. To provide a baseline for bidding the Bid Form has been modified to include a unit price for excavation of undesirable materials. See attachment included in this Addendum. QUESTION 5: Sheet S200 shows 4 base course at the foundation. What is the extent of base course required? ANSWER 5: See revisions to S200 contained in this addendum. QUESTION 6: Can you clarify which labor rates shall be used on this project? ANSWER 6: Per Section Supplementary Conditions 2.4(B) labor rates are as defined in the State of Alaska Statutes Title 36. Revisions to this section for clarity are contained in this addendum. 3 SECTION BID FORM Chiniak Tsunami Center and Library agrees to perform the work required at the amount specified in strict conformance with the terms of this solicitation and the Contract Documents. This offer is valid for ninety (90) calendar days after the date offers are due. If differences between the written and printed form, the written form shall prevail. BID ITEM DESCRIPTION 1 Base Bid Labor, materials, and equipment for the complete construction of an approximately 1,440 square foot single-story wood-framed building including all associated site work, excavation, foundations, structural frame, enclosure, roofing, interior and exterior finishes, and mechanical, plumbing, and electrical systems. 2 Unit Price 1 Unit Price allowance for removal of undesirable soils below building foundation and backfill with satisfactory subbase material. (Note: Cost of Unit Price Allowance for the purpose of bid award is contractors proposed unit cost multiplied by quantity of 30 CY. Actual payment shall be based upon actual quantities measured.) Addendum Number Date BIDDER QUALIFICATIONS Kodiak Island Borough will only accept bids from qualified bidders. To be qualified to bid on this project bidder must certify that: 1. Bidder is a construction contractor duly licensed in Alaska, who owns, operates, or maintains a place of business, regularly engaged in commercial or residential construction projects. 2. Bidder has the necessary experience, organization, technical qualifications, financial resources and facilities available to perform the contemplated work. 3. Bidder has a current Alaska Business license and in all respects meets Federal, State, and local requirements to qualify as a responsible bidder. By signing this form (and providing seal if a corporation) Bidder warrants that it meets these requirements. Failure to provide the required signatures and other information may disqualify the bid. 5 SECTION UNIT PRICES PART 1 - GENERAL 1.1 RELATED DOCUMENTS A. Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section. 1.2 SUMMARY A. Section includes administrative and procedural requirements for unit prices. 1.3 DEFINITIONS A. Unit price is an amount incorporated in the Agreement, applicable during the duration of the Work as a price per unit of measurement for materials, equipment, or services, or a portion of the Work, added to or deducted from the Contract Sum by appropriate modification, based on the actual quantity measured. 1.4 PROCEDURES A. Unit prices include all necessary material, plus cost for excavation and relocation of contaminated material, provide and install backfill, insurance, applicable taxes, overhead, and profit. B. Measurement and Payment: Unit price is measured in per cubic yard of contaminated soil excavated, relocated and backfilled with approved backfill. Measurement of contaminated soil shall be by method agreed to in writing by Owner and Contractor. C. Owner reserves the right to reject Contractor's measurement of work-in-place that involves use of established unit prices and to have this work measured, at Owner's expense, by an independent surveyor acceptable to Contractor. 6 PART 3 - EXECUTION 3.1 SCHEDULE OF UNIT PRICES A. Unit Price 1: Removal of undesirable soil and backfill with sub-base. 1. Description: Excavation of material below building foundation identified as being undesirable (organics, volcanic ash, etc.) and disposed of off-site, and replacement with satisfactory subbase material according to sheet S Unit of Measurement: Cubic yard of undesirable soil excavated. 3. Quantity Allowance: The bid form describes the quantity of material that is the basis of contract award. Actual payment shall be based upon quantity of material measured. A change order adding to or subtracting from the unit price allowance of 30 cubic yards shall be made to determine the actual pay amount. END OF UNIT PRICES UNIT PRICES LIBRARY
