Date of Addendum: September 22, NOTICE TO BIDDERS This Addendum is issued to all registered plan holders pursuant to the Instructions to Bidders and Conditions of the Contract. This Addendum serves to clarify, revise, and supersede information in the Project Manual, Drawings, and previously issued Addenda. Portions of the Addendum affecting the Contract Documents will be incorporated into the Contract by enumeration of the Addendum in the Owner/Contractor Agreement. The Bidder shall acknowledge receipt of this Addendum in the appropriate space on the Bid Form. 2 4 REVISIONS TO DIVISION 00 PROCUREMENT REQUIREMENTS AND CONTRACTING REQUIREMENTS Document Instructions to Bidders, (not reissued). AIA Document A , Page 4, Article 4 Bidding Procedures, 4.1 Preparation of Bids, ADD Paragraph 4.10 as follows: a. Document Forms, (not reissued). AIA Document A , Page 16, Article 3 Contractor, REVISE Paragraph 6 NOT USED as follows: a Bids shall not include sales and use tax. 6 TAXES Bidders are directed to submit bid prices that do not include sales tax. The Project is exempt from sales tax pursuant to Section RSMo. The Contractor will receive a Project Exempt Certificate and a Missouri Tax Exemption Letter from Truman State University to use when purchasing materials on a tax free basis. It is the Contractor's responsibility to provide the necessary documentation to any subcontractors and suppliers. These documents shall be used solely for the purchase of materials being directly incorporated into or consumed in the construction of work under this Contract. BIDDER QUESTIONS - REQUESTS FOR INFORMATION / CLARIFICATION (responses follow questions in bold, underlined print) The details on sheet A4 illustrate a one piece pvc jamb extension. Pella will provide two separate pieces (extension jamb & casing) that will require additional blocking from what is shown. In order to make sure all bidders are including the required blocking please review the attached detail and clarify the extent of blocking to be provided). Details on sheet A4 have been revised to show additional blocking; as well as correcting blocking that was shaded as existing, but is actually new (at sills). Is the pvc trim to be painted? No, the PVC white finish is acceptable. Exterior elevation general notes. Please clarify extent of plant growth to be removed from building. At the elevation work is performed, none is anticipated. D. Please verify that the entire exterior of the building is to be cleaned and sealed. Incorrect, the extent of cleaning and sealing is limited to shaded portions of the building indicated on sheet A3, not the entire building. E. Note 3 states to re-point all mortar joints as shown and as required. Please clarify quantity of re-pointing that will be required. All masonry in the shaded portions of the building indicated on sheet A3 is to be tuckpointed (mortar at brick joints, and raked back mortar joints with sealant at stone-to-stone, and brick-to-stone joints). F. Please verify that note 4 applies to stone window sills as well as copings. Incorrect, note 4 applies only to coping and cap stones in the shaded portions of the building indicated on sheet A3, not window sills located lower on the building and not part of this project scope. 3 G. Please clarify quantity of (040100): Brick removal and replacement, Stone removal and replacement, Stone fragment removal and patching, Masonry crack injection. A brick replacement allowance is added to the Bid Form. Note that any replacement brick needed for work on the parapets is to be salvaged from the demolished chimneys to the extent possible. Stone replacement, fragment patching, and crack injection portions of the specification indicate work procedures for these building elements and areas in the event work is required. Work of this nature will be addressed if and when it occurs, through the allowances. H. Per section the contractor is to repair all areas disturbed by construction with new sod. Will seeding be accepted? If so, please provide specification. Sod is required. I. Will the contractor be able to stage/cut materials at an area on each floor level during the construction process or will they be required to set up outside of the building? Yes, the Contractor may set-up multiple locations for cutting materials in the building but must protect the areas and clean-up promptly after completion of work in the area. J. Will contractor be able to drop the masonry debris generated from the chimney demolition down the existing chimney or will the debris need to be hoisted from the roof? The chimney materials will need to be carefully removed from the roof and not dropped down the chimney. Note that replacement brick needed for work on the parapets is to be salvaged from the demolished chimneys to the extent possible. K. Section 6 of the general conditions is noted as not used. Please verify that the project is tax exempt. The project is tax exempt. See the modified Instructions to Bidders, and General Conditions of the Contract for Construction in paragraph 4 of Addendum No. L. Please verify that the EXTERIOR ELEVATIONS GENERAL NOTES on Sheet A3 apply only to shaded areas P01, as indicated by the note WORK APPLIES TO MASONRY AT SHADED INDICATED AREAS, and not to the entire building. Yes, as indicated, the scope of masonry maintenance work applies only to the shaded areas on A3, not the entire building. M. Please verify the locations that have Head H3 and Jamb J3, shown on Sheet A5 and have gyp board work. It is noted as BATHROOMS, however, on Sheet A4, Head H2 and Jamb J2 are noted as BATHROOMS and do not have the gyp board work. It can be assumed that bathrooms are located in locations that have E03 type windows utilizing obscure insulated glass, however, the locations that have H3/J3 vs H2/J2 are not specified. The H3, J3, and S3 details on A5 should be considered typical at bathrooms, and therefore gypsum board cutting and patching work should be included in the base bid at all bathroom windows. Bathrooms are located at windows denoted as E03 on the building elevations. N. Per ROOF DEMOLITION KEYNOTES on sheet AD1, Note D01 says REMOVE AND REPLACE ANY EXISTING DAMAGED ROOF DECKING, WITH NEW DECKING TO MATCH EXISTING ADJACENT ROOF DECKING. (TYPICAL). However, per spec section A, Allowance No. 2 includes 500 board feet of removal of deteriorated / damaged 1x wood roof decking and replacement with material matching existing, as required. Please verify that we are not to carry any more than the allowed 500 board feet per the allowance, and disregard the excerpt from note D01 that indicates all damaged decking to be replaced. The directions are complementary and neither is to be disregarded. All damaged / deteriorated wood decking is to be replaced, and to accomplish that work, 500 board feet of decking material removal and replacement is to be included in the base bid by allowance. If the field verified amount of replacement turns out to be more or less than 500 board feet, the contract amount will be adjusted up or down accordingly based on the unit price. ADDENDUM NO

4 O. Am I correct in my understanding that the only Maintenance of Masonry scope is on the back of the parapets at the ends of the gable roofs? Will there be an allowance or unit price for replacing damaged brick or copings on those parapets not visible during the per-bid that are discovered during the construction portion of the project? Yes, as indicated, the scope of masonry maintenance work applies only to the shaded areas on A3, not the entire building. A brick replacement allowance is added to the Bid Form. Note that replacement brick needed for work on the parapets is to be salvaged from the demolished chimneys to the extent possible. P. We have been asked to provide a proposal for the joint sealants on the exterior side only for the new vinyl fiberglass windows. The details do not indicate sealant at the brick jambs or heads of the windows on the exterior side (re: details H1, J1, H2, & J2 on sheet A4) with only details S1, J1A, J1B & S2 having sealants on the exterior side of the panning. Is this the intent? I see in the window spec there are some sealant tape accessories? Are these going to provide the sealant closure at those window jambs & heads not indicating sealants? Sealant is to be installed at the full perimeter of windows, and where trim pieces join. Consult Pella on the specific requirements for concealed and exposed sealant details of their windows and trim. Q. Also, will there be a pre-bid attendance list issued? 9 SECTION ALLOWANCES ADDENDUM NO PART 1 - GENERAL 1 RELATED DOCUMENTS 2 Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section. SUMMARY Section includes administrative and procedural requirements governing allowances. Types of allowances include the following: Related Requirements: 3 Quantity allowances. Contingency allowances. Section "Unit Prices" for procedures for using unit prices, including adjustment of quantity allowances when applicable. DEFINITIONS 4 Allowance is a quantity of work or dollar amount established in lieu of additional requirements, used to defer selection of actual materials and equipment to a later date when direction will be provided to Contractor. If necessary, additional requirements will be issued by Change Order. SELECTION AND PURCHASE At the earliest practical date after award of the Contract, advise Architect of the date when final selection, or purchase and delivery, of each product or system described by an allowance must be completed by the Owner to avoid delaying the Work. At Architect's request, obtain proposals for each allowance for use in making final selections. Include recommendations that are relevant to performing the Work. Purchase products and systems selected by Architect from the designated supplier. 5 ACTION SUBMITTALS Submit proposals for purchase of products or systems included in allowances in the form specified for Change Orders. 10 6 INFORMATIONAL SUBMITTALS Submit invoices or delivery slips to show actual quantities of materials delivered to the site for use in fulfillment of each allowance. Submit time sheets and other documentation to show labor time and cost for installation of allowance items that include installation as part of the allowance. Coordinate and process submittals for allowance items in same manner as for other portions of the Work. 7 QUANTITY ALLOWANCES Allowance shall include cost to Contractor of specific products and materials ordered by Owner or selected by Architect under allowance and shall include freight and delivery to Project site. Unless otherwise indicated, Contractor's costs for receiving and handling at Project site, labor, installation, overhead and profit, and similar costs related to products and materials ordered by Owner or selected by Architect under allowance shall be included as part of the Contract Sum and not part of the allowance. Unused Materials: Return unused materials purchased under an allowance to manufacturer or supplier for credit to Owner, after installation has been completed and accepted. 8 If requested by Architect, retain and prepare unused material for storage by Owner. Deliver unused material to Owner's storage space as directed. CONTINGENCY ALLOWANCES Use the contingency allowance only as directed by Architect for Owner's purposes and only by Change Orders that indicate amounts to be charged to the allowance. Contractor's overhead, profit, and related costs for products and equipment ordered by Owner under the contingency allowance are included in the allowance and are not part of the Contract Sum. These costs include delivery, installation, insurance, equipment rental, and similar costs. Change Orders authorizing use of funds from the contingency allowance will include Contractor's related costs and reasonable overhead and profit. D. At Project closeout, credit unused amounts remaining in the contingency allowance to Owner by Change Order. 9 ADJUSTMENT OF ALLOWANCES Allowance Adjustment: To adjust allowance amounts, prepare a Change Order proposal based on the difference between purchase amount and the allowance, multiplied by final measurement of work-in-place where applicable. If applicable, include reasonable allowances for cutting losses, tolerances, mixing wastes, normal product imperfections, and similar margins. Include installation costs in purchase amount only where indicated as part of the allowance. ALLOWANCES - ADDENDUM NO

11 4. If requested, prepare explanation and documentation to substantiate distribution of overhead costs and other markups. Submit substantiation of a change in scope of Work, if any, claimed in Change Orders related to unit-cost allowances. Owner reserves the right to establish the quantity of work-in-place by independent quantity survey, measure, or count. Submit claims for increased costs because of a change in scope or nature of the allowance described in the Contract Documents, whether for the purchase order amount or Contractor's handling, labor, installation, overhead, and profit. Do not include Contractor's or subcontractor's indirect expense in the Change Order cost amount unless it is clearly shown that the nature or extent of Work has changed from what could have been foreseen from information in the Contract Documents. No change to Contractor's indirect expense is permitted for selection of higher- or lowerpriced materials or systems of the same scope and nature as originally indicated. Return damaged or defective products to manufacturer for replacement. 13 SECTION UNIT PRICES ADDENDUM NO PART 1 - GENERAL 1 RELATED DOCUMENTS 2 Drawings and general provisions of the Contract, including General and Supplementary Conditions and other Division 01 Specification Sections, apply to this Section. SUMMARY Section includes administrative and procedural requirements for unit prices. Related Requirements: 3 Section "Allowances" for procedures for using unit prices to adjust quantity allowances. Section "Contract Modification Procedures" for procedures for submitting and handling Change Orders. DEFINITIONS 4 Unit price is an amount incorporated into the Agreement, applicable during the duration of the Work as a price per unit of measurement for materials, equipment, or services, or a portion of the Work, added to or deducted from the Contract Sum by appropriate modification, if the scope of Work or estimated quantities of Work required by the Contract Documents are increased or decreased. PROCEDURES Unit prices include all necessary material, plus cost for delivery, installation, insurance, overhead, and profit. Measurement and Payment: See individual Specification Sections for work that requires establishment of unit prices. Methods of measurement and payment for unit prices are specified in those Sections. Owner reserves the right to reject Contractor's measurement of work-in-place that involves use of established unit prices and to have this work measured, at Owner's expense, by an independent surveyor acceptable to Contractor. D. List of Unit Prices: A schedule of unit prices is included in Part Specification Sections referenced in the schedule contain requirements for materials described under each unit price. 15 SECTION SELECTIVE DEMOLITION - ADDENDUM NO PART 1 - GENERAL 1 RELATED DOCUMENTS 2 Drawings and general provisions of the Contract, including General and Supplementary Conditions and Division 01 Specification Sections, apply to this Section. SUMMARY Section Includes: Related Requirements: 3 Demolition and removal of selected portions of building or structure. Salvage of existing items to be reused or recycled. Section "Summary" for restrictions on use of the premises, Owner-occupancy requirements, and phasing requirements. Section "Execution" for cutting and patching procedures. DEFINITIONS Remove: Detach items from existing construction and dispose of them off-site unless indicated to be salvaged or reinstalled. Remove, Salvage, and Reinstall: Detach items from existing construction, in a manner to prevent damage, prepare for reuse, and reinstall where indicated. Existing to Remain: Leave existing items that are not to be removed and that are not otherwise indicated to be salvaged or reinstalled. 4 MATERIALS OWNERSHIP 5 Unless otherwise indicated, demolition waste becomes property of Contractor. FIELD CONDITIONS Conditions existing at time of inspection for bidding purpose will be maintained by Owner as far as practical. Notify Architect of discrepancies between existing conditions and Drawings before proceeding with selective demolition. Hazardous Materials: It is not expected that hazardous materials will be encountered in the Work. SELECTIVE DEMOLITION ADDENDUM NO

16 D. If suspected hazardous materials are encountered, do not disturb; immediately notify Architect and Owner. Hazardous materials will be removed by Owner under a separate contract. Storage or sale of removed items or materials on-site is not permitted. PART 2 - PRODUCTS (Not Used) PART 3 - EXECUTION 1 EXAMINATION 2 Review Project Record Documents of existing construction or other existing condition and hazardous material information provided by Owner. Owner does not guarantee that existing conditions are same as those indicated in Project Record Documents. PROTECTION Temporary Protection: Provide temporary barricades and other protection required to prevent injury to people and damage to adjacent buildings and facilities to remain Provide protection to ensure safe passage of people around selective demolition area and to and from occupied portions of building. Provide temporary weather protection, during interval between selective demolition of existing construction on exterior surfaces and new construction, to prevent water leakage and damage to structure and interior areas. Protect walls, ceilings, floors, and other existing finish work that are to remain or that are exposed during selective demolition operations. Protect furniture, furnishings, and equipment that have not been removed. Remove temporary barricades and protections where hazards no longer exist. SELECTIVE DEMOLITION, GENERAL General: Demolish and remove existing construction only to the extent required by new construction and as indicated. Use methods required to complete the Work within limitations of governing regulations and as follows: Remove decayed, vermin-infested, or otherwise dangerous or unsuitable materials and promptly dispose of off-site. Dispose of demolished items and materials promptly. Site Access and Temporary Controls: Conduct selective demolition and debris-removal operations to ensure minimum interference with roads, streets, walks, walkways, and other adjacent occupied and used facilities. Removed, Salvaged, and Reinstalled Items: Clean and repair items to functional condition adequate for intended reuse. SELECTIVE DEMOLITION ADDENDUM NO

17 D. 4 Reinstall items in locations indicated. Comply with installation requirements for new materials and equipment. Provide connections, supports, and miscellaneous materials necessary to make item functional for use indicated. Existing Items to Remain: Protect construction indicated to remain against damage and soiling during selective demolition. When permitted by Architect, items may be removed to a suitable, protected storage location during selective demolition and cleaned and reinstalled in their original locations after selective demolition operations are complete. SELECTIVE DEMOLITION PROCEDURES FOR SPECIFIC MATERIALS Roofing: Remove no more existing roofing than what can be covered in one day by new roofing and so that building interior remains watertight and weathertight. See Section Asphalt Shingles for new roofing requirements. 5 Remove existing roofing system down to substrate. DISPOSAL OF DEMOLISHED MATERIALS Remove demolition waste materials from Project site and dispose of them in an EPA-approved construction and demolition waste landfill acceptable to authorities having jurisdiction. 6 Do not allow demolished materials to accumulate on-site. Remove and transport debris in a manner that will prevent spillage on adjacent surfaces and areas. Remove debris from elevated portions of building by chute, hoist, or other device that will convey debris to grade level in a controlled descent. Burning: Do not burn demolished materials. CLEANING 7 Clean adjacent structures and improvements of dust, dirt, and debris caused by selective demolition operations.
