2 TO ALL BIDDERS: THIS ADDENDUM SUPPLEMENTS THE ORIGINAL DRAWINGS AND SPECIFICATIONS AND SHALL BE TAKEN INTO ACCOUNT IN PREPARING BIDS, AND SHALL BECOME PART OF THE CONTRACT DOCUMENTS. Revise the Contract Documents as follows: QUESTIONS: Question: How do the 3-5 conduit for electric service terminate? Answer: They connect to 3 of the 6 existing spare conduit. See notes on revised plan 10 detail 1/10. Question: Is the pump controller, meter and disconnect switch mounted on pipe or tubing & unistrut? Answer: Provide the mounting rack detailed in details 1/11, B/11 & A/11. See revision to details 3/11 on revised plan 11. Question: What are the specs on the surge arrestor on detail 5/11? Answer: Provide Delta LA603 for 4 wire volt. Question: Sheet #11 Detail #5, the drawing calls out "utility meter", no model or specification listed in drawing or specs. Answer: Provide meter trim per City of Westerville Division of Power requirements. The meter will be provided by the City. Question: Sheet #10 Detail #3 shows service feeders from meter to transformer as (4) #3 in 2" conduit. Sheet #11 Detail #5 shows service feeders from meter to transformer as (4) #1 in #2" conduit. Which one is correct? Answer: Provide per detail #3 sheet 10. Question: Sheet #11 Detail #5; the drawing calls out 100 amp 480 VAC 3 pole, S/N, UL Service Entrance Label. What does 'S/N' call for? Addendum No. 01 Please acknowledge receipt of this Addendum No. 01 on the Bid Schedule submitted with your Sealed Bid INFORMATION & CLARIFICATIONS Queries from Pre-Bid meeting, Westerville PW-14 [ (WTP)], November 3, SECTION Type of motor shaft. Is vertical hollow shaft correct? Response: the pipe described here is possibly misleading. The motor is connected to the suction adapter (below bowl assembly) and not directly to the lineshaft. Action: Item 2.2.A. delete, vertical hollow shaft,. SECTION Is the requirement for a foot valve correct? Why not check valve instead? Response: Foot valve is correct. The most recent well constructed at Westerville (PW-10) has a foot valve. Action: none Addendum No. 01 November 9, 2017

7 SECTION Questions regarding the number of, the inside diameter of, and the material for probe tube(s) inside the Pitless Adapter. Responses: The tube(s) house water level probes and/or other wires/cable. The 1.5 inch inside diameter for the tube(s) can be reduced to provide improved fit. Although initially only one tube is required, the specified number will remain at two to provide future flexibility should another probe be warranted. There appeared to be some confusion as to the type of material specified for the tubes. It was suggested the tubes be specified as PVC; however, paragraph clearly specifies the tube material to be PVC. Actions: Paragraph Replace 1.5 with Paragraph Replace 1.5 with SECTION A question was raised concerning discharge of water to Alum Creek during testing. Can the discharges be overland to the creek or are the discharges required to be piped all the way to the creek? Response: Paragraph requires that discharges be piped all the way to Alum Creek using the negotiated easements. Action: none General. A question was raised as to who is responsible for collection, preservation and analytical testing of well water to meet Ohio EPA requirements. Response. Nothing in the plans or specifications addresses water sampling. Action: CONTRACTOR is not required to perform any of the described actions. 8 REQUEST FOR INFORMATION: The following is in response to RFIs received by the City of Westerville, Ohio. Q1. The prints call for 20 of screen and the specs call out 15 of screen. Which is correct? A1. The correct information is as follows: The screen extends from 55' to 70'; therefore it is 15 feet. The correct screen size is Q2. Is it possible to obtain a copy of the sign-in sheet from the November 3 pre-bid conference? A2. The sign-in sheets have been attached. This ADDENDUM modifies the CONTRACT DOCUMENTS.
