Where ELIMINATION cannot be reasonably applied then, SUBSTITUTION, ENGINEERING, ADMINISTRATION & PPE can be considered as preferred controls in descending order. Persons Working Outside Of Approved Conditions Or SWMS Persons Not Understanding Job Procedures / Steps All persons must complete the site specific induction and hold an industry induction card and have signed onto SWMS. PPE to be worn-, high visibility vest/clothing, steel capped boots, hard hat and sun block as required Ensure all persons complete a tool box talk with the nominated Damien and understand all risks and safety control measures Manual Handling Use correct manual handling techniques as per Work Safe guidance Project safety advisor Damien Step 4 Set up excavator Unsafe, poorly maintained machine causing injury to site crew or the public The operator to present up to date maintenance records and a machine specific Risk Assessment Damien Step 5 Excavate pier footings Struck By Machine Use spotter to assist with positioning of machine, keep area barricaded off from others, licensed operator only to operate machine Crush Hazard Keep clear of swing area whilst in operation, licensed operator only to operate machine Damage to underground services An underground asset check to be completed prior to excavation. 5 We, the undersigned, confirm that we were consulted in the development of this Safe Work Method Statement nominated above and have been explained its contents. We also confirm that we are qualified to carry out the works identified above, a copy of to evidence our required qualifications have been to undertake this activity are current. We also clearly understand that the controls in this SWMS must be applied as documented; otherwise work is to cease immediately.
