1 GENERAL CONTRACTOR PRE-QUALIFICATION FORM This form must be completed and submitted by all firms wanting to be pre-approved to bid on the Missouri State Capitol Building Exterior Stone Renovations and Repair, Project No. O , as a prime general contractor. A PRE-QUALIFIED GENERAL CONTRACTOR MAY BID THE PROJECT ONLY BY INCLUDING A PRE-QUALIFIED MASONRY SUBCONTRACTOR. In general, the project consists of the restoration of the State of Missouri Capitol Building and includes, but is not limited to, stone repairs (dutchman, redressing existing stone, replacement of stone, restoration cleaning, and repointing mortar joints) and general replacement of stone pavers around the Capitol. The estimated budget for the project is $30-$40 million, including the main building façade, dome/drum, penthouses, and surrounding terrace/plaza. Definitions: For the purposes of this document, Owner will refer to the State of Missouri, Office of Administration, Division of Facilities Management, Design and Construction. For the purposes of this document, historic masonry repair will refer to repairs, cleaning, repointing, and related masonry work performed on natural stone facades greater than seventy (75) years old. Note that not all of the stone/masonry work included in this project is historic masonry repair. DESCRIPTION OF THE QUALIFICATION PROCESS: All respondents to this RFQ will be evaluated based upon financial responsibility, safety record, ability to adhere to schedules, past experience of the firm and the firm s proposed project team with projects of similar size, construction type, schedule and scope, and other information requested below. The Owner will assign an evaluation team comprised of representatives of the design team, the construction manager, and the Owner s staff to evaluate and score the respondent s qualifications. The evaluation team will subjectively evaluate the respondent s qualifications and assign points for each category identified below. A total of 1,000 points is possible. A minimum of 800 points must be achieved to be pre-qualified to bid on the project. After the evaluation team has evaluated and scored the qualifications of all respondents, the Owner will notify respondents whether they are pre-qualified to bid on the project. The anticipated schedule for the pre-qualification process is as follows: Pre-qualification submissions due to the Owner 10/23/2017 Review of qualification submissions Week of 10/23/2017. 2 The weighting of points for the pre-qualification evaluation will be as follows: Organizational Information: 50 Points Project Team Experience: 200 Points Financial Responsibility and Capacity: 150 Points Safety Program and Record: 200 Points Relevant Experience and Past Performance: 400 Points TOTAL POSSIBLE POINTS: 1000 Points Irrespective of any point totals, mandatory requirements for pre-qualification include: 1. Successful completion of project(s) similar in value and scope by the firm in last ten (10) years. 2. No work related fatalities in the last five (5) years. Firms not meeting these mandatory requirements will not be pre-qualified. The Owner reserves the right to seek clarification of any information provided by a respondent. ON OR BEFORE 5:00 P.M. 3 A. ORGANIZATIONAL INFORMATION (50 POINTS) (Provide in separate document) 1. Provide a brief history of how the firm started and developed, when the company started, a list of the primary officers and their respective titles who are involved with the company, and how the company is organized. 2. Provide a brief description of the type of services the firm typically provides. 3. Describe the services that will be provided by the firm on this project and the anticipated percentage of the work that will performed by the firm s own work force. 4. Include an organizational chart showing all persons who will be involved with this project, their roles and responsibilities, and their working location during the entire life of the project. The firm will be required to have, at a minimum, the following project staff: a qualified key Project Manager, a Job Superintendent and a full-time, on-site Supervisor who have served in similar project team roles and have previous experience with projects of a similar scope and complexity. The on-site Supervisor and the Job Superintendent may be the same person. B. PROJECT TEAM EXPERIENCE (200 POINTS) 1. List the qualified Project Manager, Job Superintendent, and on-site Supervisor below. 2. Attach resumes for the Project Manager, Job Superintendent, and on-site Supervisor, listing their name, experience, time with the company and experience with projects of similar scope and complexity. 3. For the on-site Supervisor, list three (3) projects of similar scope and complexity that he/she has worked on and provide the contact information of the owner and the architect for each project. Projects including masonry work and/or historic masonry repair on limestone or marble structures older than seventy-five (75) years are preferred and may be given additional points in scoring. One (1) of the on-site Supervisor s projects shall be with the firm submitting this document and shall be included in Part E.1 of this form Relevant Experience and Past Performance: Project Experience. 4. Please note that substitution of the on-site Supervisor prior to or during the project will only be allowed upon the Owner s written approval, and any such substitution must meet the same requirements and conditions. The firm s most recent assigned Experience Modification Rate (EMR); include a signed letter from the firm s workers compensation carrier indicating this rate. B. The firm s Recordable Incident Rate for the past five (5) years. C. The firm s Days Away, Restricted or Transferred (DART) rate for the past five (5) years. 4. 6 E. RELEVANT EXPERIENCE AND PAST PERFORMANCE (400 POINTS) 1. Project Experience: List five (5) past and/or present projects of a similar scope and complexity as this project that your organization has completed in the past ten (10) years. Projects that include masonry work and/or historic masonry repair on limestone or marble structures older than seventy-five (75) years are preferred and may be given additional points in scoring. Project contacts must be able to be contacted for a reference as part of the pre-qualification evaluation, and current, valid contact information must be provided. Submission of photographs of each project is preferred and may be given additional points in scoring. Each picture should be labeled with the Project Name and Number. Provide a narrative of the scheduling programs and strategies that your firm has used in the past to maintain each phase of a project schedule, including how weather was managed. Also, include details regarding your firm s efforts for management of critical submittals and closeout efforts on past projects. 3. Has your firm ever defaulted on a construction contract or failed to complete awarded work? If so, please provide a description of the project and an explanation of why the work was not completed. 4. Has your firm, or any of its owners, officers or members, ever been sued by client or owner of a project that the firm performed work on?
