1 CRESTON FAMILY DENTISTRY: ADDENDUM #3 JUNE 26, 2014 CRESTON FAMILY DENTISTRY Addendum #3 June 26, 2014 Creston, Iowa The following items have been assembled as changes, corrections, clarification to the drawings and specifications for this project dated February 27, These changes shall be a part of the contract documents and bids for this project and shall include all costs or credits involved with these items. Contractors shall acknowledge this addendum on their bid proposal form. Changes, corrections or clarifications to the DRAWINGS and SPECIFICATIONS include the following: GENERAL NOTES: 1. General Contractor s Bidders List: No other bids will be accepted. Grace Ice Water Shield or ICF manufacturers recommendation, below grade to footing and up wall 12 above grading, to be compatible with ICF insulation and approved by Architect. 5. REFERENCE Stone Sill and Horizontal Stone look on drawings: A. Stone to set on ICF Brick Ledge blocks. See attached drawings Addendum #3 Sheets 3-1, 3-2 (typical). B. See Stone Cladding Specification Section (See attached) 6. Clarification: Aluminum Doors to be narrow style. 7. Design loads for the entire office area on the main level is 50 psf. Submit product literature, unit shop drawings, certification, test reports, full size sample(s) of each color selected Quality Assurance A. All high density concrete masonry units shall be Waterford Stone masonry units manufactured by an Oldcastle Company. All units shall conform to ASTM C90-11b and shall have a hand chiseled finish with a range of colors as specified. B. Field Constructed Mock-up: Construct a sample panel, no less than 4 x 4, of units of the color to be used in the project. The cleaning agent and method should also be determined and applied at the same time the sample panel is constructed Delivery, Storage and Handling A. Concrete Stone Masonry Veneer: Waterford Stone Masonry Units as manufactured and distributed by Oldcastle Companies. B. Solid units with physical dimensions and characteristics complying with ASTM C55-11 and the physical properties of ASTM C568/C568M-10, Class II. C. Color: Minimum of three variegated color blending as selected from manufacturer s standard selections. D. Size: 4 inches in width nominal with face dimensions selected from a minimum of 15 face sizes to match ashlar pattern indicated on drawings or established in the approved mock-up. E. Anchors: Provide anchors to suit design requirements for material and installation requirements. F. Coursings: Install masonry construction anchored solidly to backing, properly aligned, plumb and true in required layout, making straight, level courses, unless otherwise indicated. 01. 4 page Maintain mortar joint thickness of 3/8 (10mm) to 3/4 (19mm) either horizontally or vertically. 03. Tool joints as follows: a. Flush b. Concave c. Recessed B. Placing and Bonding: Lay masonry in full bed of mortar with full head joints, properly jointed with other work. Buttering corners of joints, deep or excessive furrowing of mortar joints is not permitted. 01. Back-bevel bed joints to remove excess mortar droppings in cavity as work progresses. 02. Fully bond intersections and external corners. 03. Isolate masonry partitions from vertical structural framing members with a control joint as indicated. 04. Provide for expansion and control joints as indicated on the drawings. 05. Do not adjust masonry units after placing. Where resetting of masonry is required, remove, clean units, and place in new mortar. 06. Leave joints under shelf angles and elsewhere as indicated or required open to receive sealant. C. Anchors: Provide anchors to suit design requirements indicated or required. 01. Masonry backing: Wire anchors or adjustable anchors. Minimum spacing 18 inches (450 mm) horizontally and 32 inches (800 mm) vertically. 02. Steel stud backing: Adjustable anchors attached with corrosion-resistant screws having a minimum nominal shank diameter of 0.19 inch (4.8 mm). Minimum spacing 18 (450 mm) horizontally and 32 (800 mm) vertically. 03. Wood stud backing: Corrugated sheet metal anchors, wire anchors or adjustable anchors. Attach to backing with a corrosion-resistant 8d common nail, or a fastener with equivalent or greater pullout strength. Minimum spacing 18 inches (450 mm) horizontally and 32 inches (800 mm) vertically. 04. Increase quantity of wall ties around perimeter of openings, at wall terminations and corners, and along parapet walls, placed within 8 inches (203 mm) of openings and edges of masonry. D. Built-in items furnished by other trades, and leave accurate openings necessary for subsequent installation of other work to maintain required strength and appearance of masonry construction. 01. Fill solidly with mortar around conduit and sleeves passing through masonry work. 02. Build-in loose steel angle lintels, providing bearing indicated in full mortar bed. E. Prevent grout, mortar, and soil from staining face of masonry. Remove grout and mortar from these surfaces immediately. F. Protect base of masonry walls from rain splashed mud and mortar splatters. G. Install weep holes in veneer masonry at maximum 24 (610 mm) on center horizontally above through-wall flashing, above shelf angles, lintels, bottom of walls, and elsewhere as indicated on drawings. H. Do not permit mortar to drop or accumulate into cavity air space or to plug weeps Installation A. Lighting: Provide adequate lighting for masonry work by placing all lighting a reasonable distance from the wall for even illumination. Do not use trough lighting. B. Cutting: Make all unit cuts, including those for bonding, holes, boxes, etc., with motor driven masonry saws, using either an abrasive or diamond blade. The facing shall be free from chips, cracks, crazes or any other imperfection that would detract from the overall appearance of the finished wall when viewed from a distance of fifteen (15) feet at right angles to the wall with normal lighting Flashing of Masonry Work A. Install flashing at locations shown in the plans and in strict accordance with the details and the best masonry flashing practices Weep Holes and Vents A. Install weep holes and vents at proper intervals (32 O.C.) at courses above grade, above flashing, and at any water stops (32 O.C.) over windows, doors, and beams Cleaning A. Keep walls clean daily during installation using brushes, rags, and burlap squares. Do not allow excess mortar lumps or smears to harden on the finished surfaces. Remove green mortar with burlap or a dry cloth. Harsh cleaning methods after walls have been erected may mar the surface of the units Final Clean down A. No acid or acid base solutions shall be used in the cleaning of Waterford Stone units. A detergent masonry cleaner shall be used following the manufacturer s instructions and the surface shall be thoroughly rinsed with clean water Maintenance A. Waterford Stone units, when properly installed and cleaned, need virtually no maintenance other than routine cleaning with standard commercial grade cleaning agents. Graffiti, paint or dye stains may need special cleaning methods and products. Contact manufacturer for specific cleaning recommendations Installation Recommendations A. Cavity wall construction is recommended for exterior walls, with proper flashing, venting and weep holes. B. A complimentary or matching mortar color is recommended when using Waterford Stone units. C. A water repellent mortar additive is highly recommended for all exterior walls. D. Control joint spacing should be approximately max. and located at inside corners, 2-4 from outside corners, wall height changes and at one or both sides of windows or large openings Limitations A. The facing is not intended for use as an impervious surface. B. Muriatic acid should not be used as a cleaning agent or in direct contact with the facing. C. The easily applied liquid quickly dries into a rubberized, highly durable, water-resistant, vapor-permeable membrane. R-GUARD Spray Wrap provides superior protection against water intrusion while minimizing potential for condensation within walls and allowing accumulated moisture to dry while reducing energy costs and lowering the risk of mold and mildew. R-GUARD Spray Wrap is compatible with most paints, sealants and selfadhered waterproofing or air barrier components. Use PROSOCO R-GUARD Spray Wrap as a high-performing water-resistive barrier or as part of a continuous, building-wide air barrier system. Appropriate for vertical, above-grave applications to exterior sheathing, CMU, cast concrete and most other common building materials. 7 Installation Before applying, read Preparation and Safety Information sections in the Manufacturer s Product Data Sheet for PROSOCO R-GUARD Spray Wrap. Refer to the Product Data Sheet for additional information about application. Do not dilute or alter R-GUARD Spray Wrap, except for use in sprayers. The product may need thinning with up to 5 percent fresh water, based on surface, drying conditions and equipment. Mix well before use with a low-speed drill and clean mixing paddle. Avoid mixing air into the membrane. Application: Exterior Sheathing (Gypsum, Plywood, OSB) 1. Spray or roller apply sufficient Spray Wrap to achieve a continuous, pinhole free coating. Some gypsum sheathing will require additional material due to varying substrate porosity. 2. When spray applying, back roll as necessary to ensure there are no pinholes, voids or gaps in the membrane. 3. Inspect membrane before covering. Repair any punctures or damaged areas by applying additional Spray Wrap. Overlap repairs, penetration treatments, transitions, rigid flashing and other air barrier components to ensure positive drainage and continuity of the air and water-resistive barrier. Application: CMU Wall Construction 1. Spray or roller apply sufficient Spray Wrap to fill and cover the entire face of the exterior wall assembly. Let dry. 2. Spray or roller-apply a second coat of Spray Wrap to achieve hide. The finished application must be continuous and free of voids and pinholes. Back roll spray application to achieve a void- and pinhole-free surface. Take special care to achieve full coverage around wall ties or surface irregularities. 3. Inspect membrane before covering. Repair any punctures or damaged areas by applying additional Spray Wrap. Overlap repairs, penetration treatments, transitions, rigid flashing and other air barrier components to ensure positive drainage and continuity of the air and water-resistive barrier. Curing and Drying Complete drying times vary with temperature, humidity and surface conditions. Protect from rain or freezing until completely dry. At 70 F (21 C) and 50% relative humidity, R-GUARD Spray Wrap dries to touch and can be over coated in 2-4 hours. Coverage Coverage rates may vary depending on surface porosity, moisture uptake and other factors. Unless otherwise required by the referenced test method, performance test results were achieved when the product was applied at 10 wet mils to DensGlass gold fiberglass mat gypsum sheathing. Many gypsum sheathing products require additional material to achieve hide and the desired mil thickness for a pinhole free coating. Actual rates must be determined through mockup applications. Exterior Gypsum Board, OSB & Plywood: 50 to 100 square feet per gallon CMU: 30 to 60 square feet per gallon per coat Cleanup Clean tools and equipment with water immediately after use. Dried material must be removed mechanically.

PREPARATION To ensure best results, apply to clean surfaces free of contaminants. Chemical residues, surface coatings or films may adversely affect adhesion. Protect people, vehicles, property, plants and all other surfaces not intended to receive Spray Wrap. Remove and replace damaged sheathing. Surfaces to be coated must be continuous. On exterior sheathing, repair cracks larger than ½ inch. On CMU or concrete, fill cracks larger than ½ inch with mortar. Consult a structural engineer for all moving cracks, and repair as needed. Fill, Bridge & Flash Uses: R-GUARD GypPrime + R-GUARD Joint & Seam Filler + R-GUARD FastFlash 1. Fill surface defects and over driven fasteners with R-GUARD Joint & Seam Filler. Fill open joints, seams and cracks measuring up to ½ inch (13 mm) with R-GUARD Joint & Seam Filler. 2. Consolidate and seal cut edges of gypsum board sheathing in rough openings with fast-drying R-GUARD GypPrime. Gun and spread R-GUARD Joint & Seam Filler into all inside corners, cracks, open joints and seams in rough openings. Allow Joint & Seam Filler to skin over. 3. Gun and spread R-GUARD FastFlash to create an opaque, monolithic flashing membrane over the inside of all rough openings and extending 4 6 inches ( mm) over the face of the exterior wall assembly. Allow FastFlash to skin over. Use FastFlash to create a continuous waterproof membrane which is free of voids or pinholes. See individual product data sheets and R-GUARD Installation Guidelines for more information. Let all joint and seam fillers and rough opening treatments skin over before applying R-GUARD Spray Wrap. Surface and Air Temperatures Surface and ambient temperatures should be 40 F (4 C) and rising and below 110 F (43 C) during application and drying. Wind and high temperatures will accelerate drying. ALWAYS TEST ALWAYS TEST a small area of each surface to confirm suitability and desired results before starting overall application. Test with the same equipment, recommended surface preparation and application procedures planned for general application. Hot Weather Precautions: If air or surface temperatures exceed 95 F (35 C), apply to shaded surfaces and before daytime air and surface temperatures reach their peak. Hot surfaces may be cooled with a mist of fresh water. Surfaces may be damp but must be free of standing water before application. Keep containers closed and out of direct sunlight when not in use. Cover open pails with a wet towel as needed to prevent skinning. Equipment Mix R-GUARD Spray Wrap with a low-speed drill and clean mixing paddle. When roller applying, use a 3/4 inch (19 mm) nap roller. R-GUARD Spray Wrap is compatible with GRACO and Titan airless spray equipment with the following specifications: Minimum 1 gallon per minute output. Minimum hose width of 3/8 inch. NOTE: A ¼ x 3 whip hose may be used for ergonomic purposes. Run 3/8 ID hose all the way to the 3 whip hose. Minimum tip size of Minimum pressure requirement to spray of 2,000 psi at the gun with an airless sprayer rated no lower than 3,300 psi. Remove all filters in sprayer and gun before application. 10 Spray Wrap PRODUCT DATA SHEET Storage and Handling Store in a cool dry place. Keep container tightly closed when not dispensing. Do not open container until preparation work is complete. Do not mix or alter with other chemicals. When stored at or below 80 F (27 C) R-GUARD Spray Wrap has a shelf life of 12 months after the date of manufacture. This shelf life assumes upright storage of factory sealed containers. Do not double stack pallets. Dispose of unused product and containers in accordance with local, state and federal regulations. Apply Spray Wrap to Gypsum, Plywood and OSB sheathing to achieve a continuous, pinhole free coating. Some gypsum sheathing will require additional material due to varying substrate porosity. 2. When spray applying, back roll as necessary to ensure there are no pinholes, voids or gaps in the membrane. 3. Inspect membrane before covering. Repair any punctures or damaged areas by applying additional Spray Wrap. Overlap repairs, penetration treatments, transitions, rigid flashing and other air barrier components to ensure positive drainage and continuity of the air and water-resistive barrier. Application: CMU Wall Construction 1. Apply sufficient Spray Wrap to fill and cover the entire face of the exterior wall assembly. Let dry. 2. Apply a second coat of Spray Wrap to achieve hide. The finished application must be continuous and free of voids and pinholes. If necessary, back roll spray to achieve a void- and pinhole-free surface. Take special care to achieve full coverage around wall ties or surface irregularities. 3. Inspect membrane before covering. Repair any punctures or damaged areas by applying additional Spray Wrap. Overlap repairs, penetration treatments, transitions, rigid flashing and other air barrier components to ensure positive drainage and continuity of the air and water-resistive barrier. Curing and Drying Complete drying times vary with temperature, humidity and surface conditions. Protect from rain or freezing until completely dry. At 70 F (21 C) and 50% relative humidity, R-GUARD Spray Wrap dries to touch and can be over coated in 2-4 hours. Cleanup Clean tools and equipment with water immediately after use. Mechanically remove dried material. BEST PRACTICES PROSOCO R-GUARD Joint & Seam Filler, FastFlash and AirDam are recommended for improved performance of all R-GUARD air- and water-resistive barrier coatings. Though R-GUARD Spray Wrap can be located anywhere in the wall assembly, the ideal location is on the cavity-facing side of the exterior sheathing or CMU backup. This positions the air barrier to accommodate transitions with other building components. It also lets the air barrier perform as a water-resistive barrier. It is not necessary to apply R-GUARD Spray Wrap to the inside of rough openings prepared with FastFlash. Always use two coats of R-GUARD Spray Wrap on CMU walls. Hot Weather Precautions: If air or surface temperatures exceed 95 F (35 C), apply to shaded surfaces and before daytime air and surface temperatures reach their peak. Hot surfaces may be cooled with a mist of fresh water. Surfaces may be damp but must be free of standing water before application. Keep containers closed and out of direct sunlight when not in use. Cover open pails with a wet towel as needed to prevent skinning. Coverage rates will vary depending on surface porosity, moisture uptake, and other factors. Many gypsum sheathing products require additional material to achieve hide and the desired mil thickness for a pinhole free coating. In some cases two coats may be required. Actual rates must be determined through mock-up applications. 23 technical services Amtico Installation Guidelines General The installation of Amtico is straightforward and similar to the installation procedures that apply to all quality resilient tile floors. Good preparation is essential for a trouble-free installation. Do not install Amtico until jobsite testing and subfloor preparations are finished and the work of all other trades is complete. Site conditions must comply with the relevant building codes and local, state and national regulations. Documentation of Moisture and ph test results is required when processing claims. For guidance about subfloor testing and Amtico installation recommendations. Contact Mannington Technical Services at Amtico is recommended for use over properly prepared concrete, suspended wood, metal and other suitable substrates. Amtico can be used with radiant heating systems (maximum subfloor surface temperature of 81 F). Amtico is not suitable for external installation or unheated locations. Amtico tiles, adhesive, jobsite and subfloor must be acclimated to a stable condition before installation. Following installation, Amtico should be protected from traffic for 24 hours, point loads and rolling traffic for 48 hours and should not be washed for 5 days. The flooring should remain at a temperature between F (13-27 C) during its service life. Always follow other manufacturers written recommendations when using their proprietary materials for subfloor preparation. A. Material Receiving, Handling & Storage 1. Immediately remove any shrink wrapping and verify materials delivered are correct style, color and quantity. 2. Report discrepancies immediately to Technical Services at Labor claims for installation of products installed with visual defects or incorrect style will not be honored. 3. Store all materials off the floor (keep cartons flat) in a weather-tight enclosure between F (13-27 C). 4. Acclimate tile, adhesive and jobsite to a stable condition between F (18-27 C) and 50% +/- 10% RH for 48 hours before, during and after installation. Acclimate all materials onsite and off stack tile cartons to a single layer keeping cartons flat. Temporary heating and cooling should be used if permanent HVAC is not operational. 5. Tiles should not be stored close to exterior walls, in direct sunlight or near HVAC vents. B. Jobsite Testing 1. Before jobsite testing, the building envelope must be sealed (walls, roofing, windows, doorways etc., installed). 2. Anticipated environmental conditions must be maintained or 75 F +/- 10 F (24 C +/- 6 C) and 50% +/- 10% RH a minimum of 48 hours before and during testing (ASTM F 710). 3. Test sites must be properly prepared and protected for the duration of testing to achieve valid results. 4. Surface Flatness for all Subfloors: The surface shall be flat to 1/8 in 10 ft (3 mm in 3 m). To check flatness, place a 10 ft straight edge on the surface and measure the gap or use another suitable method. 5. Concrete Subfloors: a. Concrete subfloors must be finished and cured without additives, curing compounds, hardeners or surface treatments that may prevent proper bonding of the flooring materials (ACI and ASTM F710). b. Randomly check concrete subfloor for porosity using the drop water test. Place a 1 inch diameter drop of water directly onto the concrete subfloor. If the water droplet does not dissipate within 60 to 90 seconds the subfloor is considered non-porous. c. Concrete subfloors must have a minimum compressive strength of 3000 psi. d. Concrete subfloors shall not consist of lightweight concrete or gypsum with less than 100 lbs. /cubic foot density. e. Moisture Testing: Perform either the preferred In-situ Relative Humidity (RH) Test (ASTM F2170) or the acceptable Moisture Vapor Emission Rate (MVER) Test (ASTM F1869). Internal RH readings above 80% or MVER readings above 5 lb/1000 SF/24 hours require a moisture suppressant system. f. Alkalinity: Must test surface alkalinity (ASTM F710). A 7.0 to 10.0 ph is acceptable. Reduce higher ph by mopping on a vinegar/water solution: 1 pint vinegar to 2 gallons water. 6. Wood Subfloors and underlayment panels shall have the moisture content tested using a suitable wood pin meter. Readings between the wood subfloor and underlayment panels should be within 3% and have a maximum moisture content of 14% or less. Effective moisture control in basements and crawlspaces is essential for achieving a successful long term installation. C. Moisture Suppressant System Concrete subfloors that exceed 80% RH or 5 lbs MVER require a Moisture Suppressant System. Due to complexities associated with moisture vapor transmission, emissions and movement of soluble salts (alkalinity) in concrete subfloors, we do not offer, recommend or warranty a specific solution for excess moisture in concrete slabs.
