This Addendum forms a part of the Contract Documents. The following conditions take precedence over any conflicting conditions in the Drawings and Specifications. The Drawings and Specifications are hereby amended in the following particulars: 1.1 CLARIFICATIONS A. Consistent with the legal advertisement for bids, a pre-bid conference was held at 9:00 am local time on April 18, Attendance at this pre-bid conference was mandatory for General Contractors to be eligible to bid the project. B. The pre-bid conference minutes and sign-in sheet are attached. 1.2 REVISIONS (DRAWINGS) A. None 1.3 REVISIONS (SPECIFICATIONS) A. Remove Form C-15 Detail of Project Sign from the Table of Contents. B. Replace Specification Section Wood Flooring in it s entirety. 1.4 ATTACHMENTS A. 3 WALLACE STATE COMMUNITY COLLEGE Haynes Theater Renovations BC# ; Aho Architects Project Number: PRE-BID CONFERENCE MINUTES April 18, 2017 at 9:00 AM 1) Welcoming remarks. Reminder to sign-in on sheet at front of room or being circulated. 2) Introductions and names and relationship to job of personnel (Owner and Architect). 3) Project Directory: Names of Architect and personnel involved. Project Principal: Tim Aho. Project Managers: Edmond Miller. Administrative Assistant: Stephanie Vaughn. 4) Client representatives involved: Dr. Vicki Karolewics, Mr. Jason Morgan, and Mr. Billy Rose. Do not contact the Owner directly. 5) Bid Documents available for Pre-Qualified General Contractors and subs/suppliers through Alabama Graphics Digital Plan Room (this is how addenda will be issued also). Also posted/listed with plan room services such as isqft, Reed, etc. See legal advertisement for more info. 6) Every General Contractor and every Subcontractor should read and be familiar with all of the front-end documents and all of Division 1 of the Project Manual, in addition to the work they are bidding and other work they have to coordinate with. 7) Pre-Bid Approval: All comparable products from the manufacturers listed in the project manual, requests for substitutions, additional manufacturers, pre-approved subcontractors and suppliers may be submitted no later than specified time prior to the bid date. 8) Bid date per legal advertisements: Thursday, 4/27/17. Bid opening is at 2:00pm (local time), in the same location as this pre-bid conference, in the large conference room. Any bid date changes will be changed only by an official addendum issued by the Architect. 9) Note that Instructions to Bidders should be read by each bidder. 10) Special Instructions to Bidders shall be reviewed. Among other requirements, note sales tax avoidance, bids are to be held open 60 days. 11) Proposal Form included in Project Manual. The amount that the GC proposes for the alternates will be an add to the base bid, not the net total bid including the base bid + alternates. Loose/electronic (PDF) copies available from Architect. Note new sales tax requirement Form C-3A Accounting of Sales Tax attachment to Proposal Form must be submitted with bid.

4 12) Note requirements for submittal of subcontractor list, major suppliers, etc. within 24 hours (work days, e.g. 2 pm on 4/28/17) after receipt of bids. 13) No Addenda have been issued to date. Addendum will be issued with minutes/sign-in from this pre-bid conference. All Addenda will be issued via Alabama Graphics. 14) Insurance requirements should be read by each bidder, and should be provided to each General Contractor s and Subcontractor s insurance carrier for review. Insurance requirements are indicated in General Conditions. 15) Note requirements: Section Supplementary Conditions, pertaining to Compliance with H.B Alabama Immigration Law (E-Verify MOU and Contract language). 16) Construction schedule requirements: Contract Time for this project is 90 consecutive calendar days from the Notice to Proceed, must be substantially complete. See Proposal Form. 17) See Specs for coordination and further related information. 18) Contingency Allowance (Section ). Make sure you include this in your bids. 19) Verify existing conditions at the site. 20) The Owner is a tax-exempt entity, sales and use tax avoidance procedures will be utilized on this project, per the Owner. See Section Sales and Use Tax Savings for more info. There is a new way of handling sales tax avoidance, so please read this section carefully. Bids shall no longer include sales tax. Form C-3A attachment to the Proposal Form (C-3) is required for estimated tax accounting. A final report of sales taxes avoided is required by ADOR and is a condition prior to final payment. 21) Contractor shall have access to available existing utilities (water, electric power, etc.) onsite, for reasonable use (paid by the Owner). Restroom facilities by Contractor. 22) Liquidated damages amounts - as indicated in Article 49 of the General Conditions of the Contract. 23) Site restrictions - as indicated in Section and other locations on the Drawings and in the Project Manual. 24) Pre-Construction Conference required for this project. Other mandatory inspections involving the Dept of Construction Management will include a final. The Permit Fee must be paid to the Dept of Construction Management by the GC, along with signed contracts prior to scheduling the Pre-Construction Conference. 25) Contractor s schedule must coordinate the overall construction schedule of the project. Watch for long lead-time items; established Contract Time allows for this. 26) Contractor s job meetings - coordination of the Work with all subcontractors and suppliers is required.

5 27) Safety and Protection - Contractor s responsibility. 28) Work limits protection - the public, Owner s staff, workers, etc. 29) Project Sign: Not required on this project. 30) No Subcontractor signs. 31) Requirements for quality control, testing, and inspections are located throughout the project manual, and should be reviewed so that they are understood prior to bidding the project. 32) Location of on-site Contractor s office shall be coordinated with the Owner and Architect. 33) Superintendents and construction personnel experience requirements - as indicated. 34) Submittal Procedures: Submittals requirements indicated. Contractor is required to check, mark, stamp, and approve and/or reject submittals, prior to submittal to Architect. Refer to General Conditions for additional information and related requirements. Electronic submittals preferred except where physical samples, color charts, etc. are required. 35) Site maintenance requirements indicated. Daily attention required. 36) Contractor Programs and Conduct of Personnel (behavior, tobacco, firearms, etc.). 37) Section Closeout Procedures and General Conditions should be reviewed by each bidder. 38) Final payment will not be made until the requirements of these sections and General Conditions are completed Sales tax final accounting report also required. 39) Project Record Documents have to be kept up-to-date on the project and will be checked periodically during the Work of the project. 40) Section Selective Demolition : note requirements for scheduling and coordination with other Work of the project, as well as schedule for completing the Work of the Project, which are generally indicated in this Section and in detail on the Drawings and other parts of the Project Manual. 41) GC is responsible for accepting all existing field dimensions prior to starting work. 42) Site tour and brief overview of project and scope by Aho Architects. 43) Questions?

6 a) Are the contractors required to obtain a business license with the City of Hanceville? Yes. This is required of all contractors working onsite. b) Will there be any extended work hours? Extended work hours will be taken into consideration. c) Will the contractor be required to address any required steel reinforcement and/or correction? Not at this present time. 44) Additional site visits availability, contact persons, etc. Please be respectful and minimize disruption of the Owner s ongoing operations. 45) Closing remarks.

8 WALLACE STATE COMMUNITY COLLEGE HAYNES THEATER RENOVATION WOOD FLOORING SECTION FLOATING STAGE FLOORING - PINE PART 1 - GENERAL RELATED DOCUMENTS: Specification Interior Painting Quality Assurance: Supplier shall be a firm established in the field. Submit references. Installer shall be a company exhibiting a minimum of ten (10) years continuous experience in the athletic flooring field and approved by the manufacturer. Submit a list of at least three completed projects of similar magnitude and complexity where this pattern of flooring was installed using maple strip flooring. Include owner references with this submittal. Manufacturers wishing to gain prior approval shall request, in writing, for owner s qualification criteria. Submittals: Submit three copies of manufacturer s product data. Samples: Prior to commencing work, a mock-up of 24 x24 shall be reviewed by the Owner and Architect onsite. Maintenance Literature: Three copies of MFMA Care and Maintenance of wood floors. Certification: Manufacturer shall provide certification that all materials meet grade, quality and treatment if applicable. Delivery, Storage and Handling: Materials shall not be delivered to the jobsite until all overhead work is complete. Room temperature shall be degrees and a consistent relative humidity maintained. Job Conditions/Sequence: Do not install floor system until requirements in paragraph above are met. Permanent heat, light, and ventilation shall be operating and maintained during and following installation. JOB NO PAGE 1 OF 4

9 WALLACE STATE COMMUNITY COLLEGE HAYNES THEATER RENOVATION WOOD FLOORING Contractor shall request verification if existing floor-mounted microphone jacks and power outlets are to be abandoned or shall remain. Coverplates for floor-mounted receptacles to be furnished by others and installed by flooring contractor. After floors are finished, area is to be locked to allow time for curing of the finish. Contractor shall protect finished floor until inspection and acceptance. PART 2 - PRODUCTS Manufacturers: The following manufacturers' products have been used to establish minimum standards for materials, workmanship and function: "BIO-CUSHION SUBFLOOR Robbins Sports Surfaces ( ) PINE FINISH FLOOR - See Performance specification for details Other approved manufacturers: AACER FLEX Aacer Flooring (877) Equal products of other manufacturers may be used in the work provide such products have been approved, by the Architect. Materials: All dimensions are nominal per industry standards. Membrane - 6 mil polyethylene sheeting Robbins "BIO-CUSHION resilient component: Resilient mount shall be 7/16 thick Bio Pad. Subfloor system: Lower layer - 1/2" x 4' x 8' CDX plywood. APA-Rated Exterior grade SYP or manufacturer-approved underlayment panel. Upper layer - 1/2" x 4' x 8' CDX plywood. APA-Rated Exterior grade SYP or manufacturer-approved underlayment panel. Subfloor fasteners - One inch coated staples or equivalent Subfloor adhesive - PL-400 Construction adhesive. Flooring fasteners - Two inch barbed floor cleats or staples PINE FLOORING : DIMENSION: 1 x 4 (nominal) Random Length JOB NO PAGE 2 OF 4

10 WALLACE STATE COMMUNITY COLLEGE HAYNES THEATER RENOVATION WOOD FLOORING CUT: SPECIES: GRADE: Mixed Grain to be used Southern Yellow Pine C&Btr. All flooring to be side-matched with tongue & groove and kiln-dried. Perimeter base: Robbins Inc. Vent-Cove, or approved equal. Color shall be Black. Finishing material: NOFMA-approved, oil-modified sealer. Floor to be painted Expansion joint cover at edge of all wood and concrete: Architectural Art MFG., Inc Model #K , Aluminum. Equal approved by the flooring manufacturer may also be used. Transition Ramps: Heavy duty black rubber ramps with metal expansion covers, as required. PART 3 - EXECUTION Inspection: Following demolition of existing wood floor system, Inspect existing concrete floor for proper dryness and tolerance and report any discrepancies to the Architect in writing. All work required to put the existing concrete floor in acceptable order shall be the responsibility of this contractor. Broom or vacuum clean substrates to be covered immediately before product installation. After cleaning, examine substrates for moisture, alkaline salts, carbonation, or dust. Proceed with installation only after unsatisfactory conditions have been corrected Installation: "Bio-Cushion" subfloor system: Install polyethylene with joints lapped a minimum four inches (4"). Install "Robbins Bio-Pad" pad on lower layer of underlayment using manufacturerrecommended fasteners. Position underlayment perpendicular to the intended course of the maple flooring. Stagger all joints and space panels 1/4" apart. Install second layer of underlayment diagonal to direction of finish flooring. Stagger all joints and space panel 1/4" apart. Secure panel with construction adhesive and staples placed 12" O.C. with perimeter staples placed 6" O.C. Machine nail pine flooring to plywood subfloor in straight lay pattern. 11 WALLACE STATE COMMUNITY COLLEGE HAYNES THEATER RENOVATION WOOD FLOORING Sanding: end joints shall be driven up tightly and proper spacing allowed for humidity changes affecting the floor. Flooring contractor shall be allowed to incorporate spacer joints in the floor to insure adequate expansion and to adjust for pattern creep. Provide 2" expansion voids at all walls and vertical obstructions. Sand floor using coarse, medium, and fine paper. After sanding, run 100 grit screen over entire floor with rotary sander. Clean floor to remove all dust and debris prior to sealing wood. Finishing: Apply one coat of oil-modified stain/sealer. Burnish and apply second coating. Burnish and allow to dry before allowing traffic on the floor. Perimeter molding: Install vented rubber base at walls/floor intersection where required. Use screws or adhesive. Use pre-formed outside corners and neatly miter all inside corners. Install rubber ramps and metal expansion coverplates per manufacturer s guidelines. END OF SECTION JOB NO PAGE 4 OF 4
