2 ADDENDUM #2 WINDOW AND DOOR REPLACEMENT WEST ROCKS MIDDLE SCHOOL 81 WEST ROCKS ROAD NORWALK, CT STATE PROJECT NO EC S/P+A PROJECT NO DATE: March 13, 2017 The following changes to the Drawings and Project Specifications shall become a part of the Drawings and Project Specifications; superseding previously issued Drawings and Project Specifications to the extent modified by Addendum No. 2. General Information/Clarifications: All references to Phase I of II throughout the package shall be revised to read Phase I of I. Changes to the RFP: INVITATION TO BID: o Page 1: Last paragraph, add the following: In accordance with Connecticut General Statute Sections 4a-100 and 4b-91, a responsible bid must contain two (2) documents: The Contractor Prequalification Certificate and the Update (Bid) Statement. The classification GENERAL BUILDING CONSTRUCTION B or DOORS AND WINDOWS is required as a minimum. Preliminary construction schedule. Second paragraph, delete MANDATORY in its entirety. All interested parties may bid on the project. o Page 2, add to the end following: In accordance with Connecticut General Statute Secs. 4a-100 and 4b-91, a responsible bid that exceeds $500,000 for this vertical building project must contain two (2) documents: The Contractor Prequalification Certificate and the Update (Bid) Statement. These two (2) documents must be submitted with the bid form. Contact the DAS Contractor Prequalification Unit at for more information. The classification GENERAL BUILDING CONSTRUCTION B or DOORS AND WINDOWS is required as a minimum. RESPONSE FORM has been deleted in its entirety. 4 1.1 RESPONSE FORM Phase I of I Windows and Doors Replacement Project West Rocks Middle School (State # EC) Vendor Name - Address - Phone - Fax - - Manager - Fed ID# The undersigned hereby declares that he/she has or they have carefully examined the plans, specifications and project site and has satisfied himself as to all the quantities and conditions, and understands that in signing this proposal he waives all right to plead any misunderstanding regarding the same. The undersigned further understands and agrees that he will furnish and provide all of the necessary material, machinery, implements, tools, labor, services, and other items of whatever nature, and to do and perform all the work necessary under the aforesaid conditions, to carry out the contract and to accept in full compensation therefore the amount of the contract as agreed to by the Contractor and the City. The undersigned further agrees, in case of variations of quantities from those shown or specified, the following unit prices will be used in adjusting the contract price. If quantities are authorized by the City, the following amount will be added to the contract as required. Unless otherwise noted, each UNIT PRICE shall include all equipment, tools, labor, permits, fees, etc., incidental to the installation and completion of the work involved. It is further understood and agreed that all the information included in, and attached to, or required by the Request for Bid shall become public record upon delivery to the City. In the bid items that follow, Bidder shall include all applicable taxes, fees and other incidental costs. Bidder must bid on every item. Prices are to be written in words and figures.
