Calvada Blvd., #200 Pahrump, NV Tonopah Office: PO Box Radar Road Tonopah, NV Nye County Comptroller Accounting Contracts Finance Grants Purchasing November 21, 2017 Request for Quotes Pursuant to NRS and Nye County Purchasing Policies and Procedures Manual, requests for quotes to perform the scope of work for the drilling of Methane Gas Monitoring Wells at the Round Mountain Landfill are hereby requested. Nye County is proceeding with soliciting quotes at/on the property as set forth above. Solicitation for quotes is as follows: 1. Quotes to perform the scope of work will be accepted by the County and must be submitted to Honey Strozzi via mail addressed to Nye County Purchasing Department, 101 Radar Road, Tonopah, NV 89049, via at or via fax on or before December 21, 2017 at 9 am. Quotes received after the deadline will not be accepted. 2. The selected vendor/contractor will be required to enter into a contract upon notification of selection. Terms to be included in the contract include but are not limited to evidence of $1,000, of liability insurance naming Nye County as insured and Workmen s Comp for employees, compliance with all applicable federal, state, local laws, permits and licenses, and compliance with applicable federal/state/local dust control laws and regulations, compliance with applicable federal/state/local laws and regulations regarding well drilling. A Notice to Proceed will be provided to the selected vendor/contractor. Work must begin within 10 calendar days from issuance of Notice to Proceed. 3. All work completed under this proposal shall be completed under the direct supervision of a Nevada-licensed well driller. The driller shall be responsible for filing the required Notices of Intent and well completion logs with the Nevada Division of Water Resources. Additionally, driller will provide Nye County with drilling logs and As-Built documentation for each well within 30 calendar days after completion. County will be responsible for Waiver Applications and Affidavits of Abandonment with the Nevada Division of Water Resources. 4. The scope of work for the drilling of Methane Gas Monitoring Wells as stated above includes but is not limited to: mobilization of personnel, equipment and supplies to the Round Mountain Landfill and drilling and installation of three landfill gas monitoring wells as described below. The location of the landfill is shown in Appendix A Figure 1. The locations for landfill gas monitoring well construction are shown in Appendix A Figure 2. Monitoring well construction details are shown in Appendix A Figure 3. Typical well head and probe valve completion configuration are illustrated in Appendix A Figure 4. The Contractor shall be responsible for providing all well construction materials. Nye County will provide access to the drilling site. Drilling water is not available at the site. The contractor shall provide for the disposal of all waste materials and shall leave the site in a neat and tidy condition. The Contractor shall furnish the necessary equipment for completion of the proposed scope of work. The equipment shall be the type and capability to complete the work in an efficient manner and shall be maintained in operable condition at all times. 2 Construction of three (3) Landfill Methane Gas Monitoring Wells Drill hole in native alluvial soils; 3 at 20 depth Below Ground Surface (bgs). See Figure 3 in Appendix A for details. Install and label deep probe with end cap set in native soil at the base of the hole and screened interval (0.020-inch machine slotted Sch 40 ¾ PVC) set from 15 to 20 (20 ) feet bgs (provide for removable top cap to prevent material from falling inside probe) Backfill with pea gravel to 14 feet bgs Backfill with bentonite chips (or hole plug) to 12 feet bgs and hydrate Backfill with native soil to 11 feet bgs Install and label shallow probe with end cap set in native soil at 11 feet bgs and screened interval (0.020-inch machine slotted Sch 40 ¾ PVC) set from 6 to 11 feet bgs Backfill with pea gravel to 5 feet bgs Backfill with bentonite chips (or hole plug) to 3 feet bgs and hydrate Backfill with concrete to surface, set locking well monument (6 diameter) a minimum of 2 feet above ground surface, and finish 3 x3 x6 concrete pad at surface - top of concrete finished to drain away from monument. Backfill inside of monument to 12 inches below top with clean washed sand Install concrete-filled, red painted bollards (minimum 4-inch diameter) 1.5 beyond the concrete pads, on all four (4) sides at each well location. The bollards shall extend from a minimum of 4 feet Below Grade Surface (bgs) to 4 feet above ground surface, or as directed by the Project Engineer/Nye County. All project activities shall be performed during normal operating hours 8am to 4pm daily, excluding County holidays. Any deviations from this schedule must be approved by the Public Works Director prior to being implemented. A pre-construction meeting shall be held with representatives from Nye County Public Works and the Contractor prior to the start of the work. TBD. All work must be completed from January 2, 2018 to January 31, Quotes to include Attachment B - Schedule of Items and Prices. 6. County reserves the right to reject any and all quotes received by reason of this request for solicitation of quotes. Monitoring wells will be constructed to provide for monitoring and control of explosive gas in accordance with Nevada Administrative Code (NAC) Operating criteria: Control of explosive gas.
