1 Addendum No. 2 Anderson Court Window Replacement TO: FROM: All Plan Holders Sherman Carter Barnhart Architects 2405 Harrodsburg Road Lexington, Kentucky DATE: July 16, 2015 The purpose of this Addendum is to clarify further the requirements of the plans and specifications. The bidders are governed by the information in this Addendum as if included in the plans and specifications. This Addendum does hereby become a part of the Contract Documents. Each bidder shall acknowledge receipt of this Addendum in the space provided in the Bid Form. This Addendum consists of Five (5) 8-1/2 x 11 pages. This Addendum is being FAXED or ED to all plan holders. A. Pre-Bid Meeting 1. On July 9, 2015 at 2:00 p.m., a Pre-Bid Meeting was held at the Human Resources Office of the Housing Authority of Paducah. The minutes of that meeting are attached and shall hereby be incorporated into the Contract Documents. B. General 1. At 710 Boyd Street, a window has been recently replaced, and shall serve as the basis-of-design, for remaining window replacement, for this Contract. 2 July 9, 2015 PRE-BID MEETING MINUTES RE: ANDERSON COURT WINDOW REPLACEMENT S-C-B Project No MEETING DATE: July 9, :00 p.m. LOCATION: PRESENT: HAP, HRC Building Refer to attached sign-in sheet. On this date, a Pre-Bid Meeting was held to discuss the project. Below is a summarization of items discussed. 1. Introduction a. Kevin opened the meeting welcoming everyone and introduced the Project Team as follows: 1 Melissa D. Yates, Executive Director, HAP 2. Demonte Orr, HAP 3. Todd Shoulta, HAP 4. Holly King, Sherman Carter Barnhart Architects, PLLC 5. Kevin Matthews, Sherman Carter Barnhart Architects, PLLC 6. Brandon Ward, Sherman Carter Barnhart Architects, PLLC 2. Clarifications and Addendum a. Kevin stated all items discussed today will become part of the Pre-Bid Meeting Minutes, which will be incorporated into the addenda. b. It is the intent to send out the final addenda, to the Plan Holders, no later than July 16, The Printer will issue all addenda. c. Any item requiring clarification must be brought to the attention of the Kevin Matthews, by or fax, by noon, July 16, 2015, for inclusion in the last addendum. Do not call.

All bidders are accountable to read the contract documents, which include the photos and project manual; including but not limited to, technical specs and bidding requirements, and content therein, regardless of whether items were discussed during the pre-bid meeting or not. a. Bids are due on July 23, 2015 at 10:00 a.m., at HAP Central Office front desk. Sealed bids will be time and date stamped upon receipt, at the front desk. All bids will be publicly opened and read aloud at the HRC Building. b. Faxed bids will not be permitted. 4. Contractor of Award a. Note time requirements for submittals, etc. 5. Project Overview a. Kevin provided a brief overview of the project which includes, but is not limited to the following: 1. Contractor is responsible to verify ALL sizes, ALL quantities, requirements for installation, and provide all materials, flashings, sealants, subsills, etc., for a complete installation. 2. Inclusion of products other than those specified, does not require prior approval. Manufacturers listed in the specifications are used to establish a level of quality. Other manufacturers may be acceptable provided the product complies with the Construction Documents. Burden of proof shall be the bidder's responsibility. 3. Be advised this is a Family, occupied site. 4. There are Allowances and Unit Prices requested, on the Bid Form. 5. Read the specs, if there is a discrepancy, tell me. 6. Pay attention to the warranty period and warranty requirements. b. Demonte stated a formal site visit will be conducted on July 13 th, at 10:00am. Meet at the Site Management Office. c. Demonte stated that the Bathroom Windows are NOT to be replaced. 7. Questions and Answers a. Question: Will electronic submittals be acceptable? Yes. And, response will be electronic. Note Architect review period is (14) working days. b. Question: What is the time for completion? 60 days, which includes time for submittal review and ordering materials.

4 Page 3 c. Question: Will a phasing schedule be required? Yes, the Contractor of Award will need to provide a schedule so the Owner can advise the Tenants, when work will occur in their Unit. d. Question: How will the Units be accessed? The Contractor of Award will be provided a key. The Contractor will sign-out the key in the morning and return/sign-in the key at the end of that work day.
