1 The following items represent clarifications, additions, deletions, and/or modifications to the Contract Bid Documents for the above-referenced project. This Addendum shall hereafter be regarded as part of the Contract Documents. Items not referenced herein remain unchanged. Receipt of this Addendum shall be acknowledged below and on the Bid Form. This addendum consists of 5 pages. These items replace original items previously issued or are to be added to the Bidding and Construction Documents as indicated. PART I PERTAINING TO THE SPECIFICATIONS: FRONT ENDS Please revise INSTRUCTIONS TO BIDDERS, Page 10, Section 20, TIME FOR PERFORMANCE, Subsection 20.1 to state: 20.1 Time is of the essence in the performance of this Contract. The Contractor specifically agrees that contract time will begin on the date that the RCDD issues the Notice to Proceed. All work to be performed under the provisions of this Contract shall be completed to Substantial Completion in not more than TWO HUNDRED SIXTY (260) calendar days, and an additional 30 days to Final Acceptance, subject only to delays caused through no fault of the Contractor or acts of God. Work hours shall be between 7:00 AM and 5:30 PM. Revise to include the following paragraph to the INSURANCE REQUIRED section that starts on page 15: Builders Risk INSTALLATION COVERAGE: CONTRACTOR must provide Builders Risk installation coverage to include coverage for materials stored at the project site, property while in transit, and property stored at a temporary location for the amount of materials involved in this contract. Bid Form: The bid form has been revised. The last two columns that break down the cost per each SWFWMD grant have been removed. Grant number N346 has been added to bid items 7&8. The quantity for bid item 8 has been increased from 114 to 195. The quantity for bid item 11 has been decreased from 8 to 7. The quantity for bid item 13 has been increased from 2 to 3. Bid item 14 should read Asphalt Drive Restoration (N327). The unit for item number 17, Chain Link Fence/Gate Replacement/Relocation (N327) has been changed from LS to LF. A new bid item (number 18) for Open Cut / Directional Drill Rock Allowance (N327 and N346) has been added to the bid form. See attached revised BID FORM - PRICE SCHEDULE. TECHNICAL SPECIFICATIONS If rock is encountered during pipe installation by the open cut or directional drilling method at the depths indicated on the plans, a line item has been added to the bid form to pay for this. Revise Technical Specification Section 01025, Item #s 2, 3, & 4, to state additional compensation will be Page 1 of 5

2 provided for rock removal on an established cubic yard basis per Item # 18 on the bid form. The bid form has been revised to reflect this. Revise Technical Specification Section Item #s 5 to include fence gates, reject piping, rip-rap, and asphalt restoration Bulleted items for schedule of values shall also include 16 x 12 Tapping Sleeve and Valve (connection to existing RCDD reclaimed water line). Section Measurement and Payment, revise Project Allowance bid item number from 18 to 19. Section Measurement and Payment, add the following: 18. Rock Excavation Measurement of the removal of rock to be paid for under this item shall be the actual inplace (not loose) cubic yards of rock material suitably removed and disposed of off-site in accordance with the Contract Documents. Extend of rock excavation shall be determined by the RCDD Project Manager and/or Engineer and unauthorized excavation shall not be considered for payment. Payment for removal of rock shall be made at the unit price per cubic yard, which price and payment shall be full compensation for performing all necessary labor, materials, equipment, transportation and other work necessary to complete the Work in accordance with the Contract Documents, including excavation of work, dewatering, and proper disposal of the rock. PART II PERTAINING TO THE DRAWINGS: Please add the following note to Sheet 16 (Fairway Lakes Subdivsion) of the plans: INSTALL 81 ¾ IRRIGATION METERS (BADGER) AND BOXES AT EACH HOME PER DETAIL SHEET 26 Please change the existing note on Sheet 16 (Fairway Lakes Subdivsion) of the plans: PROPOSED 6 ISOLATION VALVE ASSEMBLY AFTER EXISTING VALVE PER DETAIL SHEET 19 All notes to install 3/4" water meters shall be revised to reference CCU detail 5/8 1 WATER METER, PAGE # 82, #W-06-1WM-5/8 on plans Sheet 26. All 3/4 water meters shall be Badger Recordall Meters for Reclaimed Water type or approved equal and be remote read adaptable. Page 2 of 5

3 Revise notes on sheet 13 to state REMOVE 12 LF OF EXISTING 6 HIGH CHAIN LINK FENCE, REPLACE WITH 2-6 FT WIDE DOUBLE GATES. Revise note on sheet 14 (plan view) to state PROPOSED DIRECTIONAL DRILL 420 LF 6 DR11 HDPE REUSE PIPE UNDER EXISTING UTILITIES. Revise notes on sheet 21 to state REMOVE 12 LF OF EXISTING 6 HIGH CHAIN LINK FENCE, REPLACE WITH 2-6 FT WIDE DOUBLE GATES. See added note on attached Sheet 21 REMOVE AND REPLACE 389 SY OF EXISTING ASPHALT between asphalt road and 3 elbow leading to discharge riprap. PART III - CLARIFICATIONS / QUESTIONS CLARIFICATIONS The cross connection assemblies shall be demolished last after all other improvements have been reasonably made. The intention is to minimize the time customers are without irrigation water. Contractor shall be responsible for the construction and payment of 2 electric meters and any electrical upgrades at the existing wastewater treatment plant control building required to connect the delivery facility and pump station. Sheet E-1, note #10, states "Control panel, SCADA, & wireless development & electrical installation by Severn Trent Services. No exceptions. Contact Robert Migdal rmigdal@stes.com (239) " The contractor shall supply a hydrant meter and pay for water usage for testing. Stored materials will be paid only if there is a PAID receipt. A retainage of 10% of the total contract amount shall be withheld from payments until final completion. QUESTIONS Question: Is there a SWFWMD or project requirement to buy American products? Answer: No. However it is encouraged. Page 3 of 5

4 Question: How will we find the service lines for the homes that do not have visible meter boxes on Sheet 16, Subdivisions Riverside, Fairway Lakes, Bay ridge, and Sheet 17 Eagle Trace? Answer: It will be the contractors responsibility to soft dig to locate all service lines. Record drawings may be made available. Question: I have reviewed the plans for the subject project and am requesting that Fusible PVC pipe be allowed in lieu of HDPE for the horizontal directional drill elements. Use of Fusible PVC will provide the following upfront cost advantages as well as long term performance advantages: 1. Since 6 HDPE is connecting to 4 PVC and 10 HDPE is connecting to 8 PVC, Fusible PVC can be 4 connecting to 4 PVC and 8 connecting to 8 PVC since it has the same ID for a given nominal size pipe as B&S PVC. With no need to increase the HDD pipe a size, then the bore hole can be smaller, the fittings don t require fused adapters (FPVC accepts mechanical fittings directly just like B&S PVC), and the pipe weight drops. All this allows for a less expensive HDD installed cost. 2. Utilizing Fusible PVC for the HDD elements results in the entire pipeline being the same thermoplastic pipe. PVC has been demonstrated by decades of use in potable and reclaim water systems to be resistant to oxidative attack by disinfectant compounds (example chlorine, hypochlorite, chlorine dioxide) commonly used to combat biological growth and pathogens. HDPE has been proven by numerous third party research studies (reference following link - to be susceptible to oxidative attack by the disinfectants compounds under pressure applications of water/reclaim water distribution. Answer: Per Charlotte County Detail G-08-DB, Page 23, Typical Directional Bore on Sheet 30 of the plans, Fusible PVC is an acceptable alternative to HDPE. Below is a write-up from Charlotte County Utilities Standard Specifications (January 1, 2008). Page 4 of 5
