1 12 E 4 th St, Duluth MN July 17, 2018 Request for Proposal BID #: Grand Marais Phase PROJECT NAME: Nordic Star Housing Development Phase 2 ANNOUNCEMENT DATE: July 17, 2018 DUE DATE: July 24, 2018 at Noon PUBLIC BID OPENING: July 24, 2018 at 1:00 pm PROJECT OVERVIEW: One Roof Community Housing ( One Roof ) and Common Ground Construction as general contractor, in partnership with Grand Marais/Cook County EDA is requesting proposals for the installation (sub-contract) of four modular units in Grand Marais, MN. This project is slated to begin in August 2018 and completed by November 1st, Bids will be accepted for the following work: foundation, plumbing, electrical, HVAC, transportation of modular units, crane, modular setting, and carpentry. Potential bidders may bid on a single or multiple components of the project. Information associated to this request for proposal includes: 1. 2 Questions regarding this Request for Proposal should be directed to: Brad Gilbertson, Project Manager at or PROPOSALS: Subcontractors wishing to be considered for this project must submit proposals which conform to the following requirements: A. Cover sheet proposals must include a cover sheet that states: a. Bid # and Project Name (Lutsen #1-2018; Lutsen Apartments, Lutsen, MN) b. Bidder s company name, address, and point of contact c. If the Bidder is a partially owned or fully-owned subsidiary of another organization, identify the parent organization and describe the nature and extent of the parent organization s approval rights, if any, over the activities of the Bidder d. The bid is a firm fixed-price e. Bidder has the capacity, availability, and experience to meet the timelines f. Bidder agrees that payment for work shall be on a percentage complete, plus on-site stored materials minus retainage. Final payment shall be made after: a Certificate of Occupancy or Completion has been executed; all lien releases have been received for completed work; and final inspection has been conducted B. Pricing provide a highly detailed bid C. Additional Documentation: a. Copy of Subcontractor s license b. Proof of general liability, risk, and workers compensation insurance IMPORTANT: Late or incomplete proposals will not be accepted. All accepted proposals will be opened at our office in a public setting on Tuesday, July at 1:00pm where Subcontractors are welcome to be present. Proposing bidders may be contacted for additional information. All bidders may request the results. Common Ground Construction reserves the right to engage in discussions or negotiations with none, any, or all bidders as part of the selection process. Common Ground Construction reserves the right to select or reject any or all proposals, and if deemed necessary, to engage in subsequent proposal rounds. After selection, Common Ground Construction and awarded bidder will finalize all outstanding matters and execute a contract. SPECIFIC REQUIREMENTS: 1) All requirements and standards must be included in a FIRM FIXED PRICE submitted by Subcontractor and shall become part of a NO-LIEN CONSTRUCTION AGREEMENT between Common Ground Construction and Sub-contractor. Sub-Contractor may not assign construction agreement to another party. 2) All Subcontractors who procure Subcontractors must ensure that State procurement regulations are complied with. 3) Final payment will only occur after the Certificate of Occupancy or Completion has been issued. 4) Sub-contractors are responsible for exact measurements, materials, and labor needed for bidding purposes. Bids must include all labor and materials, unless noted otherwise, to complete the job in a workmanship-like manner. 5) All costs to complete the Work described in these specifications and included on any drawings or plans should be included in the bid. Any scope of work Subcontractor does not believe is

3 covered, but is needed for a workman-like completion of the Work, should be discussed with Common Ground Construction before submitting a proposal. 6) All materials used are to be new, of first quality and without defects, except in the case of harvest, salvage, or recycled materials which have been approved by Common Ground Construction. 7) Change orders are for unforeseen circumstances or substantial changes to the Scope of Work only. Common Ground Construction must approve all change orders before additional Work is started. 8) Subcontractor is Common Ground Construction s partner in the field and Subcontractor will work closely with Common Ground Construction to resolve any defects in materials or workmanship. Subcontractor agrees to remedy any defect due to faulty materials or workmanship and pay for all damage to other work resulting there from, in accordance with the timeframe indicated in the current State statute. 9) Subcontractor must be licensed and bonded, if applicable. 10) Subcontractor must obtain and pay for all applicable permits and fees. Common Ground Construction shall ensure proper zoning of building lot to allow permitting to proceed. Subcontractor must comply with all rules of the permitting process as established by Cook County. 11) All work will comply with any and all applicable Federal, State and local Codes and Ordinances. 12) Subcontractor understands that grant funds used to underwrite this project require oversight and approvals. Scope of Work for Completion of Modular Homes NORDIC STAR HOUSING DEVELOPMENT GRAND MARAIS PHASE 2 When providing bid, please itemize all items. NOTE: This request is for the construction of 4 modular homes. Foundation/Mason All work must be performed according to federal, state and local building codes. The sub-contractor is responsible for permit application, permit approval and fees. Permit ready drawings of the foundation will be ready at a later date. 1) Supply and install a Poured or ICF perimeter crawl space foundation to the specifications provided by the modular home manufacturer (HOMARK HOMES). Foundation walls to meet Minnesota State Code requirements. Crawl space will be heated, so foundation and footings must be well insulated. Soil on interior of foundation to be covered with pea rock, polyurethane and sealed to meet code requirements for perimeter foundations. 2) NOTE: Cliff 2, has a 22 x 24 attached garage, with floor drain (drain installed by plumber). Drop garage slab down inside poured wall so there is a curb. 3) Note: Brad 2, has a 14 x 24 attached garage, with floor drain (drain installed by plumber). Drop garage slab down inside poured wall so there is a curb. 4) Drain tile installed on interior and exterior with sump pit.

4 5) Provide and install marriage wall supports per specifications provided by modular home manufacturer (HOMARK HOMES); or continuous footing may be used. 6) Sidewalks per plan 7) 4 x 4 x 4 Concrete pad at front entrance of home where steps will be built. Electrical All work must be performed according to federal, state & local building codes and completed by a licensed and insured electrician. The electrician is responsible for permit application, permit approval and fees. 1) Supply and install electrical service from the utility company to service panel in home per specifications provided by modular home manufacturer (HOMARK HOMES). Excavator to provide all trenching. 2) Supply and install all electrical in crawlspace needed for furnace, sump pump and radon system. 3) Verify that all electrical connections are working in the home. 4) Supply and install electricity to the garage, outlets per code, 4 interior lights and 2 exterior lights and garage door opener outlet. Plumbing All work must be performed according to federal, state and local building codes and completed by a licensed plumber. The plumber is responsible for permit application, permit approval and fees. Water and sewer will be stubbed into the house by the excavator. 1) Supply and install water line from water supply line provided by Excavator to the home and its plumbing system per specifications provided by modular home manufacturer (HOMARK HOMES). 2) Supply and install sewer line from the sewer line provided by Excavator to the home and its waste system per specifications provided by modular home manufacturer (HOMARK HOMES). 3) NOTE: LP GAS TANK, INSTALLATION AND CONNECTION TO HOME WILL BE PROVIDED BY THE LP GAS PROVIDER. 4) Any onsite gas connections to be done by plumber. 5) Supply and install floor drain in garage. 6) Complete all onsite connection to hot water heater supplied and installed by Homark homes. 7) Supply and install a passive radon mitigation system in the crawlspace of the foundation. Connect the passive radon mitigation system to the rest of the system provided by and installed in the modular home by HOMARK Homes. 8) Supply and install sump pump. 9) Test all plumbing systems (drains, waste, vent, supply, and gas) to verify that they are in working condition. HVAC All work must be performed according to state and local and state building codes and completed by a licensed HVAC contractor. The HVAC contractor is responsible for permit application, permit approval and fees. 1) Supply and install horizontal furnace and all duct work needed to connect the boots and registers that are supplied by Homark in crawlspace for heating and cooling systems. All Duct work to be sealed to comply with energy star codes. 2) Provide and supply enough heating duct work necessary to create a heated crawl space. Crawl space will be insulated and the goal of this additional venting is to prevent the crawl space, and the water and sewer lines within it from freezing.

5 3) Test all HVAC systems supplied and installed by Homark to verify that it is working properly. 4) HRV system supplied and installed by Homark. Carpentry 1) Construct attached garages, 2 with covered entry and 2 without; verify plan and site drawings to determine exact plans needed. 2) Complete all siding, fascia and roofing as needed. 3) Supply all fasteners to complete job as needed. 4) Building material to be supplied by Common Ground Construction. Finishing Work After all units are set we will have a pre-bid meeting on site to bid the interior and exterior finishing work. An announcement of the date and time will be sent. House Transportation 1) Delivery of 4 Buildings (8 sections total) from Red Lake Falls, MN to Grand Marais, MN including all associated permits Crane Work 1) Transfer of 4-2 piece modular homes from delivery truck onto crawl space and all permits associated Setting 1) Guiding the transfer of the modular units onto their foundations, including proper bolting and anchoring 2) Installation of hinge roof system 3) Supply and install support posts in foundation per manufacturer specifications 4) Complete temporary weatherproofing to prohibit any water damage to buildings before button up is completed NOTE: There are 4 homes being bid with 2 different layouts, please review all plans and site drawings to determine correct plans, locations and site configurations.
