1 The Neighborhood Health Clinic Additions & Alterations City of Naples/ Design Review Board PROJECT DESCRIPTION The Neighborhood Health Clinic is located at 121 Goodlette-Frank Road North. The current building was completed in 2001, and is two-stories of approx. 11,132.0 SF. (First Floor is 7,254. SF; Second Floor is 3,878. SF). The Building houses a Non-Profit Medical Clinic for the working uninsured, low income adults in Collier County. It consists of a Waiting Room, Patient Toilet, Check-In/ Out, Nurse s Station, Exam Rooms, Medication Dispensing Room, Lab, Offices, Break Room, Laundry and Stairs and an elevator. The success in the mission of the Clinic is evident as the need now exceeds the current resources and physical facilities. In 2014, land was purchased directly to the south of the Clinic for the purpose of expansion. The proposed single-story Addition is for enhancing medical, dental, social services and educational opportunities. It is proposed to be 8,470. SF. It is extremely important to share the Check-In/ Reception, Waiting Area and Staffing. Connectivity has been a driving factor in the proposed plans. The proposed Addition of 8,470. SF will contain a Dental Department with 5-Dental chairs, a Dental Lab, Sterilization, X-Ray and support facilities including Meeting Rooms. Also included are 5-offices, an Isolation Room and a Medical Lab. The proposed Addition is in the same character and style to match the existing Clinic Building. It is however, one-story and is set-back as per the Goodlette-Frank Corridor Guidelines. Those requirements are for 50-0 front-yard set-back for one-story; and 75-0 front-yard set-back for two-stories. Considering the front-yard set-backs, we may request a reprieve/ deviation for a small portion to be two-stories in height- but only used on the ground level as an Entry Porch. At issue is the existing tower element is the main door to the Clinic. It is less visible with the proposed new Addition. Therefore, we would consider aligning a tower extension with the face of the Additions for the entry to be seen from the driveway access and easily identified. The proposed Addition is also designed to advance a larger Master Plan of which it is a major part. The Master Plan is a road map for the future and would envision

Eventually, the Clinic and facilities could grow to be approx. 46,950 SF The site is a collection of parcels which had bordered the Seacoast Rail Road and Train Depot for Naples. This accounts for the unusual shape of the parcels that make up the large site. The site is now the corner of Goodlette-Frank Road North and Central Avenue; which is a major approach to Downtown Naples.

3 The Neighborhood Health Clinic Additions & Alterations Design Review Board THE MISSION The immediate mission of the Clinic is to provide an expanded Dental Component, a Medical Lab (for blood draw), meeting Rooms, Offices and Classrooms. To do so and connect to the existing building it is necessary to re-configure the existing Check-In/ Reception, Waiting Room and Patient Toilet Room. The proposed Addition will utilize the common areas described above (Check-In, Waiting Room, etc.) for efficiency, Staffing and control. Along with the new building Addition the project will include parking, landscaping/ irrigation, water management and site work. Stantec Engineering is providing those services.

4 The Neighborhood Health Clinic Additions & Alterations Design Review Board EXISTING CONDITIONS The adjacent site to the south currently has an Automotive Service and Repair Shop. It will be removed as well as a former slab of another previous building on the site. As much as possible the existing curb-cuts, site access and driveways will be retained. Further to the south is Auto Europa and a small Car Repair (free-standing) building, which will remain.

SF (one-story) Addition to the existing Clinic Building with parking, landscaping, water management, etc. An Addition on the existing Second Floor of the original Clinic Building (south end) for 8 to 9 new Offices and a Board Room. The Addition would be approx. 1,270. SF. Other adjacent areas would be renovated to match the new décor.

6 The Neighborhood Health Clinic Additions & Alterations City of Naples Design Review Board FEMA & THE 50% RULE FOR IMPROVMENTS BELOW FLOOD ELEVATION The existing Clinic Building was completed in 2001 and its finished floor elevation is approx. 9-0 NGVD. That would be approx. 7-9 NAVD which was confirmed by a recent survey. The site is in the AE-7 Flood Zone (2015 DFIRM Map). Therefore, the existing Clinic Building is above FEMA for the minimum floor elevation. It is approx And exceeds 7-0 minimum. However, the latest 2015 Florida Building Code Edition 5 requires an additional 12 for free-board. The Florida Building Code references the ASCE-24 Guidelines for flooding. In summary, the new Additions will be at Elevation 8-0 NAVD, as it is 7-0 for FEMA plus 1-0 for free-board. Therefore, the existing Clinic Building would be 3 to 4 below the required finished floor elevation for a permit. Considering these varying heights, the areas of the existing building to tie-into the new construction should remain at the lower level. Thus, any transition between rooms, doors, etc. will occur elsewhere. So, the new Waiting Room, Patient Toilet Room and a portion of the Hall will be at the lower elevation (therefore, 3 to 4 below ASCE-24). This is allowed, as these areas are less cost than 50% of the value of the entire existing building- called the 50% Rule. A calculation compared to the Collier County Property Appraiser s Office values is attached. Generally, the budget allowed for this area would be approx. $694,000. which is much more than that needed, Lastly, the areas to be tied-in must be structurally independent and separated by a minimum of a 2 air space from ground to sky. Only flashing and expansion joints will connect the structures for water-proofing. This condition will be visually un-detectable. This condition helps serve another requirement to be a 4-hour fire separation so that the floor areas of the two sides will not exceed that allowed by the Florida Building

7 page two, continue Code for Table 503 for Allowable Building Areas and Type 3 Construction. Note the existing building is Type 3 while the proposed Addition will be Type 2, non-combustible. We consider Type 3 for this calculation as it is the most restrictive. Therefore, the Addition will have a 4-Hour, fire-rated wall as part of the structurally independent separation to allow the floor area to exceed the limits of (FBC) Table 503. An internal ramp will provide the 3 to 4 rise needed. In this case, the run will greatly exceed the minimum for a ramp. The Hallway will easily accommodate the ramp. This is a minor elevation change, thankfully. In conclusion, there is easy transition between the two floor elevations that will not be very noticeable; and meet the applicable codes and Guidelines.

Site deviation will be presented by Mr. John Passidomo for a rear-yard set-back to be a straight line along an awkward property line condition shared by a neighboring parking lot. 2. An optional request is to relocate the Entry Tower from its current location to the face of the proposed new (Waiting Room) Addition so that it would be visible from the driveway/ curb-cut access from Goodlette-Frank Road. The intention is to easily identify the entry from parking and pedestrians
