1 NYSDOT TRANSPORTATION MAINTENANCE DIVISION PAVING PROJECT QUALITY ASSURANCE ASSESSMENT Rating Guidelines (Revision #1) INTRODUCTION The condition assessment is intended to be performed from a customer or user s orientation. It is not intended to be a technical evaluation of the projects. For this reason densities and depths are not included. The scorer is to assess the consequences or results of the paving project. Projects to be assessed may include OGS let projects such as State Forces delivered and VPP s and contracted maintenance paving such as D contract single course overlays (simplified or standard). The assessment criteria are to be applied the same without regard to who did the work. These guidelines aim at establishing a consistent set of criteria for evaluating each project feature. Criteria are established for the A, C, and F condition. Since conditions may not exactly reflect one of these letter grades the scorer can use scores of B or D for intermediate conditions. Use N/A if the criteria is Not Applicable. In a rare case where a feature is not applicable and it is not an option on the checklist, write in N/A. PROJECT INFORMATION Most of the project information section may be completed prior to arrival at the project site. ROUTE & PROJECT LIMITS: Indicate the Route number (State or Federal) and the project limits (reference markers and/or intersecting roads). PROJECT TYPE: State Forces, VPP, MbC, or D# WORK PERFORMED BY: Indicate who performed the work. For example, residency forces, contractor (cite specific contractor), or residency forces with vendor paver and operator (cite vendor). DATE PAVED: Note the date(s) the paving was done (if known). PROJECT COST: Indicate the cost of the project (only if easily ascertained). Include cost of residency labor and equipment in addition to materials and contract costs. REGION, COUNTY, PROJECT NUMBER, SCORER, DATE SCORED: Self-explanatory. 1

2 SCORING INSTRUCTIONS Overall Impression A section has been provided for Overall Impression of the paving project. This is intended to be similar to the General Recommendation used in Bridge Inspections. This is to allow the scorer to account for a subjective reaction to the results of the entire paving project. For example, many small deficiencies that singularly may receive scores of C could lead to an overall impression of a reasonably well constructed project. Similarly, one significant deficiency, such as a poorly constructed longitudinal joint for a major portion of the project, would lead to a poor overall impression. This score should reflect on whether the project leaves the overall impression of a good, mediocre, or poor project. Comments For each rating component a space is provided for comments. The intent is for the scorer to express the rationale for the grade. For example, under Centerline and Lane Joints a score of B may include a comment like Centerline joint poorly overlapped for 1000 feet.. Scoring The scorer is required only to provide grades and comments during the evaluation. Calculation of scores will be done in the office. Scores will be calculated after the checklist is completed by multiplying the numeric equivalent of the letter grade by the percentage on the scorer sheet. Then the totals for each feature will be added and that total will be divided by the total percent (which may be less than 100 due to N/A ratings). Letter grade equivalents are as follows: A = 4, B = 3, C = 2, D = 1, and F = 0. The best possible score is Complete Project Overall Impression A- Paving project has well constructed joints and uniform surface texture. Pavement markings are applied as per MUTCD. Shoulders are backed up (if necessary) and intersecting roads and driveways are transitioned correctly. C- Some areas of joints are not well constructed, some areas of surface texture are not uniform, or some intersecting roads and/or driveways are not transitioned properly. F- Large areas of joints are not well constructed, large areas of surface texture are not uniform, several intersecting roads and/or driveways are not transitioned properly, or any portions of shoulders are not backed up (if necessary). 2

3 Travel Lanes, Ramps Turning Lanes, Shoulders & Bike Lanes Surface Texture A- Surface texture is uniform with no roller marks, drag marks, or poorly compacted areas. C- Some areas (less than 2 %) of surface texture are not uniform or have roller marks, drag marks, or poorly compacted areas. F- Large areas (more than 2%) of surface texture are not uniform or have roller marks, drag marks, or poorly compacted areas.. Longitudinal Joints A- Well constructed throughout the project. C- Some areas (approximately 5%) show improper overlap, or show poor alignment (with deviations of more than 150 mm). F- Any sections are open, or large areas (more than 10%) show improper overlap, or show poor alignment (with deviations of more than 150 mm). Ride A- The pavement s ride is good with little or no deformations. C- The pavement s ride is fair with some deformations. F- The pavement s ride is poor with frequent deformations. Cross Slopes A- The project s crown is properly placed and the entire project exhibits appropriate cross slopes across the entire section. C- Very small portions (approximately 5%) of the project s crown is improperly placed or very small portions (approximately 5%) of the project do not exhibit appropriate cross slopes across the entire section. F- Small portions (10% or more) of the project s crown is improperly placed or small portions (10% or more) of the project do not exhibit appropriate cross slopes across the entire section. Start, End, Bridge and Turning Lane Transitions A- All transitions are smooth. C- All transitions except 2 are smooth. F- Four or more transitions are not smooth. Shoulder Back-Up & Rounding A- The shoulders on the entire project are properly backed up (if necessary) and/or transitioned to the original surface with no backup material on the pavement. C- The shoulders on the entire project may have been properly backed up and/or transitioned to the original surface, but some minor settlement has occurred or the transition to the original surface is not proper or shoulder backup material is on the pavement. F- Any portion of the shoulders on the project are not backed up properly. 3

4 Intersections & Driveways Surface Texture A- Surface texture is uniform. C- Some areas (approximately 10%) of surface texture are not uniform. F- Large areas (20% or more) of surface texture are not uniform. Radii - Within the scope of the project: A- All radii are properly constructed with consistent radii and smooth transitions to tangent sections. C- A very small number (approximately 5%) of radii are not properly constructed. F- A small number (10% or more) of radii are not properly constructed. Transitions - Within the scope of the project: A- Minor route and driveway profiles blend well with existing surfaces C- A very small amount (approximately 5%) of minor route and/or driveway profiles do not blend well with existing surfaces F- A small amount (10% or more) of minor route and/or driveway profiles do not blend well with existing surfaces Drainage Adjustments to Drainage and/or Utility Structures A- All drainage (including gutters) and/or utility structures have been adjusted to the proper height. C- A small number (approximately 10%) of drainage (including gutters) and/or utility structures have not been adjusted to the proper height or a larger number drainage and/or utility structures have been adjusted, but not to the proper height. F- Some (20% or more) drainage (including gutters) and/or utility structures have not been adjusted to the proper height or most of the drainage and/or utility structures have been adjusted, but not to the proper height. Curb Reveal A- The curb reveal is within the standards for the type of curb throughout the project. C- A small area (approximately 10%) of curb does not have the proper reveal. F- Some areas of curb (20% or more) do not have the proper reveal. Shoulder Transition to Gutters A- All shoulder transitions to gutters are properly constructed C- A small amount (approximately 10%) of shoulder transitions to gutters are not properly constructed F- Some (20%) of the shoulder transitions to gutters are not properly constructed 4

5 Striping Centerline, Lane & Shoulder Stripes A- All centerline, lane, and shoulder stripes are placed as per the MUTCD and temporary pavement markings have been properly removed. C- A very small amount (approximately 5%) of the centerline, lane, and shoulder stripes are not placed as per the MUTCD and temporary pavement markings have been properly removed. F- A small amount (10% or more) of the centerline, lane, and shoulder stripes are not placed as per the MUTCD or temporary pavement markings have not been properly removed. Special Markings A- All special markings are placed as per the MUTCD. C- A very small amount (approximately 5%) of the special markings are not placed as per the MUTCD. F- A small amount (10% or more) of the special markings are not placed as per the MUTCD. Additional Comments Address any areas of concern. These may include underpass clearance, ADA ramps, lane and/or shoulder widths, paving under guide rail sections, pavement profile, reflective cracking, etc. 5
