2 Telecommunications Consultants, LLC Addendum ) Change 1.0 GENERAL SPECIFICATIONS AND OVERVIEW Bidding Requirements: The Contractor completing installation shall have been in business for at least five years, not necessarily under the same name. The Contractor completing the installation shall be experienced in voice and data installation. The Contractor shall supply names and telephone numbers of contacts for their past four projects to be used as references. The Contractor shall have on staff a Building Industry Consulting Service International (BICSI) Registered Communications Distribution Designers (RCDD) or BICSI Technician. The RCDD or Technician will be the Project Manager and shall approve construction design, drawing, and upon completion of installation, certify compliance with the standards and installation practices as specified by Technology Consultants, LLC. For the bidding process, the Project Manager must submit a signed and/or stamped Bill of Materials (BOM) consisting of supplies and materials (excluding material and labor cost) with the bid. If the Project Manager is a BICSI Technician signatures will be sufficient on the BOM. The BOM should be adequate to establish if all necessary materials and supplies have been included in the bid. The Project Manager who signs the BOM shall be the same Project Manager throughout the project with the exception of written consent from the owner. A manpower schedule and a project time line will also be required within the bid package to verify that the contractor will be able to complete the project on time. A per-unit drop cost for the addition or deduction of drops must be submitted with the original quote. This rate is good for all additional drops until testing has been completed. Change 4.02 EQUIPMENT BAYS 1. The TR must have one A-C grade 5' x 4' x 3/4" plywood backboard mounted by screw to the north wall of the TR and one A-C grade 3' x 4' x 3/4" plywood backboard mounted to the east wall. The plywood backboard must be of good quality and have no knots. The plywood backboard must be painted on all sides with two (2) coats of fire-retardant paint (International Fire Resistant Systems, Inc. part no.: FF88). The plywood backboard should be mounted 24" above the finished floor with the 4' dimension vertical. All screw heads and scuffs due to construction shall be touched up with the fire-retardant paint. 3

Change 6.0 FACILITIES DESCRIPTION 6.10 Data/voice station wiring from the TR to a communications outlet will consist of CommScope part no.: 6ECMP or Berktek equivalent (ivory or white). The four (4) pairs of unshielded wires will be terminated at each end on an ivory GIGA channel mini-jack RJ45 receptacle (Hubbell part no.: HXJ6OW or Panduit equivalent) using TIA/EIA 568B-A standards and labeled Data/voice station wiring from the TR to a communications outlet will consist of CommScope part no.: 6ECMP, Berktek part no.: , or General Cable part no.: (ivory or white). 6 Re: Project LaGrange (LaGrange Library) Date: March 28, 2011 Questions from Pre Bid Meeting, How firm is the completion date of May 4, 2011? Response: Some leeway could be arranged but no more than one week 2. Who can be the project manager for this project? Response: Because of the complexity of this building, an RCDD will be required for the project management. 3. How many walls need to have painted backer board installed? Response: All four (4) walls should have 4x8 plywood installed with FF88 paint covering both sides. 4. How many four post cabinets are required? Response: Two (2) four post cabinets Part # GL720E 2436 Series with stated options. No 2 posts racks are required 5. Where is the phone demarcation point Response: In the MC at the 110 Block 6. Where are the cables terminated? Response: In the 4 post cabinet using 48 port patch panels or a hybrid patch panel 7. Will there need to be a ladder rack installed in the MC? Response: A ladder rack will need to be installed by the TelCom contractor and should be either B Line or Monosystems. 8. How is the spine wire manager to be mounted? The TelCom contractor will have to install and secure to the existing furniture with an 18 cable loop 9. What manufacturer s jacks will be permitted to be installed in the power and data modules? Response: Hubbell or Panduit Category 6 and keystone form factor is recommended 10. What type of receptacles will be allowed for the access points? Response: When possible use the existing junction boxes provided for this function. If a J Box is not present, a biscuit above the ceiling may be permitted.

7 11. What cable vendors are allowed on this project? Response: The three vendors allowed are: Berktek, Commscope, General Cable, all category 6E Plenum only 12. How are cables in the floor boxes mounted? Response: Where terminating in existing floor boxes, there should be an interface plate installed by others. It is the TelCom contractor s responsibility to interface with the mounting bracket in the floor box. It is recommended to use a keystone form factor. 13. What if there is a long lead interface hardware item; what should be done? Response: A temporary solution should be implemented so testing can be completed by the target date. 14. Will there be any need for flooded cable in basement floor boxes? Response: No, normal cable should be sufficient; this is a sub floor installation. 15. Where will all of the station cables be terminated? Response: In the MC; there is no IC in this building 16. Can the 4 conduits between floors be modified so that the cable runs can be shortened? Response: Yes, the 4 conduits are dedicated solely to telecommunications cabling; However, there should be no sharp edges and the use of plastic bushings is required 17. What are the orange dots in the J Boxes? Response: These boxes are dedicated for telecommunications. If these boxes are not used, they should be blanked.
