1 REQUEST FOR TENDER - SERVICES 1. PURPOSE AND SCOPE This specification is intended to govern the supply of all labour, materials and equipment for the dismantling, demolition of a single dwelling on the property known as 7958 Beaverdams Road, Niagara Falls, Ontario. It is further intended that this specification will provide for the removal of all waste debris on site, all debris resulting from the demolition or removal, leaving the site in graded, leveled condition, free of hazards all in accordance with the following specifications and all requirements as stated in the Request for Tender (RFT). Should a bidder submit an alternate method than what is proposed, the City reserves the right to evaluate it on any means it feels is justified and will accept or reject the bid based on the findings of their review. 2. PRE-BID SITE MEETING A pre-bid site meeting is scheduled for August 9th, 2016 at 1:00 p.m. local time at 7958 Beaverdams Road, Niagara Falls, Ontario. Site contact is Mr. Geoff Holman, Project Manager, City of Niagara Falls. The pre-bid site meeting is mandatory if bidders are not able to attend they will not be able to bid on this Tender. 2 3. UTILITIES The City has arranged for all utilities (gas, hydro, telephone) to be disconnected. Before commencing any work on the property, the Contractor shall confirm that all utility services (gas, hydro, telephone) have been shut off. The Contractor shall sever all sewer laterals and water services and properly cap same at the nearest point to the property line. The end of these services shall be marked with a wooden 2x4 painted blue for water and green for sewer. The 2x4 shall be located at the cap and extend 600 mm above the ground surface. The City shall be called for an inspection prior to backfilling any of the capped services. City staff will mark the location of the live sewer lateral prior to the demolition. 4. PRE-CONDITION SURVEY If the property owner gives consent, the Contractor shall complete a pre-condition survey of 5600 Montrose Road prior to starting the demolition. 5. TEMPORARY WORK STOPPAGE The City will notify the contractor of any funeral services that will be in the immediate area of the demolition. The Contractor will be required to stop work for the duration of the funeral service. Permits a) The Contractor shall, at the Contractor s expense obtain all necessary permits, pay all fees and charges and comply with all building, fire and health and safety rules and regulations applicable to the demolition of buildings and the preservation of public health and safety. b) The Contractor shall also comply with all other applicable Provincial Laws and Regulations pertaining to demolition and disposal and transportation of waste material to and from the site. c) The City of Niagara Falls will apply for the Demolition Permit for the site and provide it to the successful Contractor. d) A notice of Project shall be submitted by the Contractor to the Ministry of Labour. Forms for this purpose can be obtained from their local office. e) The Contractor shall obtain, or be in the possession of, a Certificate of Approval to haul waste and shall submit a copy of this certificate to the City prior to commencement of any demolition work. 3 f) The manner in which demolition is carried out will be governed by the requirements of the Occupational Health and Safety Act and the Environmental Protection Act and Regulations there under, current at the time of demolition and in addition to the Building Code Act, and all amendments therein, and any other applicable law together with the additional requirements by the City of Niagara Falls as contained herein. (See City Requirements). It is noted that the Occupational Health and Safety Act, amended, and Regulations thereunder, where applicable, will govern the execution of this project. g) For complete information, reference should be made to the Regulations, a copy of which will be made available to the Contractor upon request to the Health and Safety Administrator at City Hall. h) No demolition or dismantling of any part of the structure shall commence until the Demolition Permit has been issued by the City of Niagara Falls, the City has obtained the completed insurance forms and contractor safety checklist and all remediation works have been completed and authorization has been given from the City to proceed with demolition. 6. PUBLIC WAY PROTECTION AND FENCING a) All boundaries shall be fenced to the satisfaction of the City and as required by the appropriate Provincial requirements, to ensure that the entire site is protected against unauthorized entry. b) No work shall be permitted on the demolition project until the above measures are in place. The contractor will be responsible for the safety and security of the site for the duration of the project. 7. TRAFFIC CONTROL, PUBLIC SAFETY AND SECURITY a) For all work, the Contractor must provide traffic and pedestrian control in accordance with Book 7, latest edition, of the Ontario Traffic Manual. When required by the City of Niagara Falls, daily traffic plans are to be prepared by a qualified person and kept on the work site at all times. The Contractor will be required to submit documentation that all person assigned to traffic control have completed the required training per Book 7 as identified above. b) The Contractor shall ensure that all measure are taken to protect the public at all times including hoarding and other safety barriers per the applicable regulations as stated in this RFQ. c) The work shall be fully supervised and controlled to create as little disruption, dust and noise as possible consistent with the Contractor s stated dust abatement policy and the applicable City of Niagara Falls bylaws related to noise and public disturbances. The supervision will be the responsibility of the Contractor. Page 3 of 7
4 8. DISPOSAL OF MATERIALS a) Debris resulting from the demolition shall be removed from the site expeditiously and shall be dumped at an approved landfill site in accordance with Municipal Policy. The Contractor shall RECYCLE as many materials as possible. b) Any alternate location or other form of disposal shall be in accordance with the Ministry of Environment Guidelines, the Environmental Protection Act and the regulation thereunder. c) Salvage or materials from the demolition either within or outside the area shall not be sold at or near the demolition site. The general public shall be prohibited from accessing any part of the demolition area. 9. REMEDIATION OF HAZARDOUS MATERIALS The City of Niagara Falls has completed A Phase 1 Environmental Site Assessment of the subject property. The results are contained in the report prepared by Oakhill Environmental Inc. for Ontario Environmental & Safety Network Ltd. found in Appendix D. All hazardous materials are to be removed from the house and property as per the Designated Substances and Hazardous Materials Abatement Specification and Project Design prepared by Ontario Environmental & Safety Network Ltd in Appendix E. The demolition of any structures is not to begin without authorization from the City of Niagara Falls. Should the contractor find any additional hazardous materials on site, they shall notify the Project Manager immediately. All work shall stop until the material can be properly tested and a safe remediation plan put in place. 10. SAFE DRINKING WATER ACT The Safe Drinking Water Act, 2002 (SDWA) requires Owners and Operating Authorities of drinking water systems to have an accredited Operating Authority. In order to become accredited, an Operating Authority must establish and maintain a Drinking Water Quality Management System (DWQMS). Bidders shall make themselves aware of the SDWA and DWQMS in place. If requested by the owner of the drinking water system, bidders shall participate in relevant awareness/information sessions either before or after closing of the tenders (see Appendices F & G). The successful bidder and its sub-contractors working on a drinking water system shall comply with all applicable regulations under the SDWA and also meet all the requirements of the DWQMS. 5 11. ACCESSIBILITY REGULATIONS FOR CONTRACTED SERVICES Contracted employees, third party employees, agents and others that provide customer service on behalf of Owner are legally responsible to be in compliance with the provisions outlined in Section 6 of the Ontario Regulation 429/07 with respect to training under the Customer Service Standard of the Accessibility for Ontarians with Disabilities Act (2005). The Contractor shall ensure that such training includes, without limitation, a review of the purposes of the Act and the requirements of the Regulation, as well as instructions regarding all matters set out in Section 6 of the Regulation. By signing Statement C the bidder has agreed to be in full compliance with this regulation. The successful bidder will be expected to complete the confirmation form and submit the same at the Preconstruction Review Meeting. A City of Niagara Falls training pamphlet has been included in Appendix H. Contractors shall be responsible for notifying the general public when planning/implementing a disruption to any transportation walkway, pathway, driveway, roadway, etc. It shall be the Contractor s responsibility to notify the general public of any disruption to regular service; 24 hours prior to the commencement of the disruption. In the case where construction activities will cause a disruption to normal service, appropriate signage shall be installed. At the Owner s request, information relating to the disruption shall be supplied to allow for the posting of disruption notices on the Owner s web page of choice. The posted signage shall as a minimum include the headings listed below and shall be filled out by the Contractor and installed in advance of the service disruption. 12. CITY REQUIREMENTS The Contractor will be required to perform the following works, to be undertaken and completed on or before November 1, 2016, which shall form part of the demolition procedure: The footings, weeping tile and foundation walls shall be completely removed from the house and garage. The basement floor shall be broken and completely removed prior to backfilling along with any granular material and shall be inspected by the City prior to backfilling. All building materials and debris including sidewalks, patios, driveways, curbs are to be completely removed from site. The following items are to be removed and disposed of in accordance with all applicable regulations (See Appendix I for photos). All concrete stairs and landings outside the house are to be removed. The concrete front porch, stairs and walkway are to be removed. All concrete rubble on site is to be removed. All wood debris is to be removed. Various tree stumps and brush is to be removed. 6 The tent trailer is to be removed from the site. The brick barbecue is to be removed from site. The deck footings are to be removed from site. Miscellaneous debris bricks, light poles, metal sheets etc shall be removed from site. All property line fences are to remain in place. All water services and Sewer lines shall be capped at the property line and the location marked with a 2x4 painted blue for water and green for sewer. The existing berm around the house is to be removed and used to fill in the excavation left from the basement. Any fill material used to backfill the excavation shall be compacted in 300mm lifts. The City reserves the right to perform any compaction testing deemed necessary. TCLIP testing has been performed and the results are provided in Appendix J. All asphalt, granular and curbs from the existing driveway are to be excavated and removed from site. The Contractor shall include in their unit price bid the allowance for one full day of excavation and the removal of five (5) loads of material from site to the Walkers hazardous material disposal facility. A representative from Oakhill Environmental Inc. will be on site to direct the Contractor and take soil samples. All required TCLIP testing will be provided by Oakhill Environmental Inc. 13. BACKFILL AND SURFACE MATERIALS a) The Contractor is encouraged to recycle as many materials as possible from the demolition. b) The Contractor is to supply clean fill material (soil) at no Additional cost to the contract. The Contractor must submit all soil testing results to the City before any material can be brought on site. c) All costs associated with the loading, transportation, placement and compaction of backfill material shall be included in the price bid. d) The finished site extending over the area of the demolished buildings shall be topped with 100mm of topsoil supplied by the Contractor and graded so that the surface drainage does not result in ponding or adversely affecting adjacent properties. MISCELLANEOUS REQUIREMENTS a) The Contractor shall not burn any trash or debris on the demolition site. b) The Contractor shall not subcontract the work or any part thereof without the consent in writing by the Project Manager. c) The Contractor shall ensure the roads and sidewalks are kept clean at all times. d) The Contractor shall not obstruct or deposit any material on the Road allowance without first obtaining permission from the City of Niagara Falls and Niagara Region. e) The adjoining properties shall be protected in an adequate manner. No access over these properties will be permitted without first obtaining the City s and affected property owners consent. f) The hours of work shall be 7:00 AM to 6:00 PM Monday to Friday. Weekend work will only be permitted with the permission of the City and/or Niagara Region. Page 7 of 7
