2 SUMMARY factory building is a proposed six storied (proposed) building. The construction works started in July 2013 and they started their production in The six (G+5) storied building has an approval as an industrial structure. Only ground and first floor construction and installation is completed during inspection date. There are two separate sheds for utility and Dining purpose. There were 495 workers in the factory during the inspection. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which would be further dealt with as part of follow-up inspections. The table below summarizes the major electrical safety issues identified during the Recommendations have been provided to each finding. inspection. 3 FINDINGS AND RECOMMENDATIONS FINDING NO: E- 1 CATEGORY: DESIGN, DRAWINGS & RECORDS Electrical Single Line Diagram (SLD) and electrical layouts are unavailable. Draw as-built electrical SLD and electrical layouts, mentioning all required information by qualified engineer and get it reviewed by Accord. REMEDIATION TIMEFRAME: 12 WEEKS FINDING NO: E- 2 CATEGORY: DESIGN, DRAWINGS & RECORDS Thermographic scanning of the entire electrical system has not been performed. Thermographic scanning of the entire electrical system must be performed twice in a year. REMEDIATION TIMEFRAME: 12 WEEKS Page 3 of 10

Insulation resistance test of all power cables (up to Floor distribution board or SDB) must be performed in a periodic manner and recorded. REMEDIATION TIMEFRAME: 12 WEEKS FINDING NO: E- 4 CATEGORY: DESIGN, DRAWINGS & RECORDS Electric safety program is not initiated. Electrical safety training and awareness program for the electrical personnel and workers must be initiated and recorded. REMEDIATION TIMEFRAME: 12 WEEKS FINDING NO: E- 5 CATEGORY: Lightning Protection & Earth Lightning Protection System (LPS) needed but has not been installed. HT cable dropping from 11kV pole must be protected in steel pipe of required size at least 2m from the ground level to protect from physical injury by moving objects. Service line drawn from 11KV REB supply line. REMEDIATION TIMEFRAME: 5 WEEKS FINDING NO: E- 7 CATEGORY: TRANSFORMER ROOM Excess cable length not arranged and supported. Excess length of existing HT cables coiled near transformer must be protected and laid safely. REMEDIATION TIMEFRAME: 6 WEEKS HT cable coiled near transformer. FINDING NO: E- 8 CATEGORY: TRANSFORMER ROOM No fire rated barrier/protection between the transformer and other occupancy (LT panel & PFI plant i.e. control panel) Construct fire rated brick wall up to the ceiling around the transformer to separate it from other occupancy and install louvers and exhaust fan for cooling the transformer room. REMEDIATION TIMEFRAME: 6 WEEKS FINDING NO: E-10 CATEGORY: TRANSFORMER ROOM Transformer installed on floor not firmly supported. Transformer base must be rigidly fixed to the foundation, with appropriate fastening fixtures. Base of power transformer in the utility shed. REMEDIATION TIMEFRAME: 5 WEEKS FINDING NO: E- 11 CATEGORY: CABLE & CABLE SUPPORT Cable trench is not covered. 7 FINDING NO: E- 12 CATEGORY: TRANSFORMER ROOM Panel baseplate not installed to allow cable entry. Install base plate of the panel and make hole into it then fit cable gland (required sized) for cable entry and exit to the panel and seal all the unused openings by suitable means to make the panel dust and vermin proof. REMEDIATION TIMEFRAME: 8 WEEKS LT panel utility Shed. FINDING NO: E- 13 CATEGORY: EQUIPMENT & MACHINE IPS battery directly lay on concrete floor. Battery must be placed on the battery stand made of noncombustible material (steel fabricated). REMEDIATION TIMEFRAME: 4 WEEKS IPS battery in utility shed. FINDING NO: E- 14 CATEGORY: DISTRIBUTION PANELS Phase separators/barriers between different phases not installed. Install separators between different phases of MCCB to prevent flashover. Standard separators provided by the MCCB manufacturer must be used. REMEDIATION TIMEFRAME: 3 WEEKS Inside LT panel in utility shed. Page 7 of 10

8 FINDING NO: E- 15 CATEGORY: DISTRIBUTION PANELS Wires in panel are spliced and insulated with PVC insulating tapes Wire joints in panels must be tightly connected using terminals or sockets crimped and insulated. PIB tape and heat-shrink tubes may be used for insulation. REMEDIATION TIMEFRAME: 3 WEEKS Wires joints found Inside PFI panel. FINDING NO: E- 16 CATEGORY: CABLE & CABLE SUPPORT Cables passing through wall/floor slab are not protected at the entry/exit point(s) Cable passing through wall Cables passing through permanent wall/floor slab must be protected. Seal the opening by fire rated material protecting power cables thus no smoke can pass through this REMEDIATION TIMEFRAME: 6 WEEKS Cables passing through permanent wall of utility shed and production floor. FINDING NO: E- 18 CATEGORY: CABLE & CABLE SUPPORTS Cable duct/channels are filled with lint/dust Clean all the cable channels/ducts; then seal them for permanent (you may use checkered/perforated plate at the bottom). But ensure that you have used proper sized cable for all of your electrical loads. REMEDIATION TIMEFRAME: 1 WEEKS Wiring duct in production floor. FINDING NO: E- 19 CATEGORY: SWITCH BOARD & PANELS Combustible material inside panel. Remove all the combustible materials from electrical panel.establish a periodic cleaning program and maintain records of the activities Distribution panel in production floor. Provide earth connection for body and doors of metallic distribution boards using green cables preferably braid so that the metallic door remains at zero potential all the time REMEDIATION TIMEFRAME: 3 WEEKS Distribution panel board in production floor FINDING NO: E- 21 CATEGORY: SWITCH BOARD & PANELS Multiple cables terminated at busbar. Terminate each power cable at single terminal and use proper sized cable lug. Multiple cables termination is allowed if all are soldered properly and powering to a single circuit.
