Acknowledge receipt of this addendum in the space provided on the BID FORM. Failure to do so may subject Bidder to disqualification. List of Documents: Description Bid Form, Attachment 3 2 pages 4 pages The original Bid Documents are updated by the information as follows: Item 1: Instructions to Bidders, Attachment 1, Page 4 of 5, Item 33: Replace item (b) with the following: X (b) The lowest bid shall be the lowest total of the bid prices on the base contract and those additive or deductive alternates, unit prices, and allowance items that were specifically identified in the bid solicitation [see bid proposal form] as being used for the purpose of determining the lowest bid price. Item 2: Item 3: Item 4: Contractor to include in the base bid price fifty-five (55) of the wireless gateways shown on the drawings and as specified Section Summary of Work, Item 1.2.D, add the following, 16. Contractor to provide in the base bid a complete 900mhz wireless survey of all ILS lock locations shown on the drawings to determine requirements (if any), for additional gateways; including shared spread of gateways. The survey shall be conducted at the beginning of work and shall be timed to prevent delay of the construction. 17. Contractor shall include the price for removal of all existing hardware as required for the installation of the specified hardware. Coordination for replacement of District cores shall be included. Section Summary of Work, Item 1.2.I., delete the word none and add the following: UNIT PRICING Price shall include all general requirements and general conditions. Addendum No. 3 Page 1 of 2

2 (Contractor shall place the price for each unit pricing total amount on the bid form Items 6 and 7) UP-01 Furnish and install specified wireless gateways, price shall also include all investigation needed for the installation, configuration and testing etc. for a complete and working installation Owner has the right to use one unit at a time. Allow for 75 units. UP-02 Furnish and install wireless locks, work shall include removal of existing locks and hardware, coordination with the District for core removal and installation, configuration and testing etc. for a complete and working installation Owner has the right to use one unit at a time Allow for 10 units Item 5: Item 6: Item 7: Section 13721, Item 2.3. A, change the specified wireless lock part numbers to the following: Cylindrical - ILS-C4P6MLxSC9 (Contractor to field verify backset and door thickness and order locks in the appropriate quantities as required.) Mortise - ILS-C4P6MFNSCx9 (Contractor to field verify door handing and order locks in the appropriate quantities as required.) Section 13721, Item 2.3.B. Wireless Gateway, add, 7. Contractor to utilize the following wireless gateway part number - ILS-C4XCMWAPE Delete the Bid Form in its entirety and use the attached Bid Form All other terms and conditions of BID NO.: 10-11/27 to remain the same. If awarded the contract, the undersigned hereby agrees to sign said contract and furnish the necessary Payment Bond, Performance Bond, and Certificates of Insurance within 10 calendar days after the Notice of the Award of this contract and agrees to commence construction within 10 calendar days after the Notice to Proceed is issued by the District. 2. The undersigned has checked carefully all the above figures and understands that the District will not be responsible for any errors or omissions on the part of the undersigned in making up this bid. 3. Peralta Community College District reserves the right to reject any or all bids, to waive any irregularities or informalities not affected by law, to evaluate the bids submitted and to award the contract according to the proposal which best serves the interests of the District. 4. All pages of this Bid Form must be completed and signed in ink. The bid will be awarded to the lowest responsive, responsible bidder. Bid Bond Each bid shall be accompanied by a cashier's check payable to the Peralta Community College District, or a bidder's bond executed by an admitted surety insurer, licensed to do business in the State of California as a surety, made payable to the Peralta Community College District in an amount not less than ten percent (10%) of the maximum amount of the bid. The check or bid bond shall be given as a guarantee that the bidder to whom the contract is awarded shall execute the contract documents and shall provide the required payment and performance bonds as specified therein within ten (10) days after the notification of the award of the contract. As part of this bid, please provide relevant information about your company s knowledge and experience, including a list of three or more projects with brief descriptions that demonstrate your experience as required in Section A Under Public Contract Code, a Responsible bidder is defined as a bidder who has demonstrated the attribute of trustworthiness, as well as quality, fitness, capacity, and experience to satisfactorily perform the public works contract. The District will be the sole determinate of whether a bidder is considered a Responsible Bidder. The District will be checking with the clients references provided to determine if the bidder has the experience necessary to perform this work and to determine the lowest responsive, responsible bidder. Attach documentation that will substantiate your minimum of three continuous years of experience, and evidence of certification of Lenel Value Added Reseller (VAR) certification and Master Level Technician certification.
