4 1.0 EXECUTIVE SUMMARY Carnow, Conibear & Assoc., Ltd. (Carnow Conibear) was retained by The Chicago Public Building Commission of Chicago (PBCC) to perform a pre-demolition asbestos-containing material (ACM) and hazardous material survey at the Haas Park Fieldhouse in Chicago, Illinois. The survey incorporated accessible areas of the interior and exterior of the building. The asbestos survey consisted of visually inspecting the interior and exterior of the subject site to identify suspect asbestos-containing materials, collecting samples from suspect materials, analyzing samples for the presence of asbestos, and quantifying the identified asbestoscontaining materials. The asbestos-containing materials identified include: Pipe Insulation in Attic (~ 300 linear ft.) Pipe Fittings in Attic (~ 50 fittings) Window Glazing (23 windows) Exterior Window Caulk approximately 16 linear feet located on each window (23 windows) Exterior Door Caulk approximately 22 linear feet located on each door (4 doors) Note: The water supply, electrical power, and small boiler remained in operation at the time of the inspection. Because of this, walls near the restroom pipes were not demolished to inspect for pipe insulation inside wall cavities, the electrical panel was not inspected for the presence of asbestos containing cement board or other asbestos containing materials behind the metal panel, and any insulation inside the small boiler was not inspected. Prior to demolition, these areas should be inspected for ACM. The hazardous material survey consisted of visually inspecting the subject site to determine the presence and location of polychlorinated-biphenyl (PCB)-containing equipment and materials (lighting ballasts, switchgears, transformers, and hydraulic fluids), and mercury-containing equipment and materials (mercury lamps, thermostats, switches, thermometers, regulators, and gauges). Hazardous materials identified during the survey included: 68 Fluorescent Bulbs 34 Ballasts One Mercury-containing thermostat Three Fire Extinguishers The hazardous materials survey also included lead-based paint sampling. The lead-based paint sampling consisted of visually inspecting the painted survey areas to determine representative paint histories and collecting random samples utilizing an X-ray fluorescence (XRF) spectrum analyzer. The lead-based paint sampling confirmed lead-based paint in the survey areas as defined by the Department of Housing and Urban Development (HUD). 5 Interior brick walls (Intermittent positive readings on the brick walls may indicate possible presence of past murals on these walls. As of January 15, 2010 the walls were one solid tan color.) Exterior soffit wood Exterior security window grates metal Carnow Conibear recommends incorporating this information into future demolition/renovation documents regarding the presence and location of asbestos-containing materials. All asbestos abatement activities shall be conducted by a licensed contractor in accordance with the Illinois Department of Public Health (IDPH), USEPA National Emissions Standards for Hazardous Air Pollutants (NESHAPS), and Occupational Safety and Health Administration (OSHA) regulations and requirements. Carnow Conibear also recommends incorporating the data from this report into future demolition/renovation documents regarding the hazardous materials and lead concentrations within paint. Prior to demolition, remove, dispose, and/or recycle all regulated materials as identified in this report. All demolition/renovation work shall be performed in accordance with the requirements of OSHA's Safety and Health Regulations for Construction (29 CFR ) and all applicable local, state, and federal rules and regulations. The demolition contractor shall utilize engineering control methods to reduce or eliminate lead exposures for the site workers. 6 2.0 INTRODUCTION Carnow, Conibear & Assoc., Ltd. (Carnow Conibear) was retained by The Public Building Commission of Chicago (PBCC) to perform a pre-demolition asbestos-containing material and hazardous material survey at the Haas Park Fieldhouse located at 2404 North Washtenaw Street in Chicago, Illinois. The survey was conducted on January 15, Timothy Panfil, an Illinois Department of Public Health (IDPH) licensed asbestos building inspector (IDPH # ) and lead inspector (IDPH # L ) conducted the asbestos and lead-based paint survey, as well as the hazardous materials survey. See Appendix D for a copy of the applicable inspector s licenses and certificates. 2.1 History of Asbestos Asbestos is a naturally occurring mineral with separable, long thin fibers with a chain-like crystal structure. There are several different types of asbestos including Amosite, Chrysotile, Tremolite, Actinolite, Anthophyllite, Crocidolite. The three most commonly used asbestos types are chrysotile (white), amosite (brown/off-white) and crocidolite (blue). Asbestos is used for its unique properties, including: fire resistance, high tensile strength, poor heat and electrical conductivity, and chemical resistance. First commercially mined in 1870 in Canada, asbestos is usually found mixed into other minerals. Pipe insulation, automotive brakes, shingles, wallboard, and blown-in insulation are just a few of the products that have been found to contain asbestos. Currently, deposits are still mined in Australia, Canada, South Africa, and the former Soviet Union. Although the federal government suspended production of most asbestos products in the early 1970s, installation of these products continued through the late 1970s and even into the early 1980s. Asbestos products such as floor tile and adhesives are still being imported today. There are two forms of asbestos according to the United States Environmental Protection Agency (USEPA). The two forms include friable and non-friable ACMs. Friable ACMs contain greater than one (1) percent asbestos and can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Friable ACMs are thought to release fibers into the air more readily, however many types of non-friable ACMs can also release fibers if disturbed. Nonfriable ACMs are hard in nature and generally tightly bound in the matrix making asbestos fiber release more difficult. There are three (3) categories of ACMs used in buildings. These include: 1) Surfacing Materials - ACMs which are sprayed or troweled on surfaces (walls, ceilings, structural members) for acoustical, decorative or fireproofing purposes. 2) Thermal System Insulation - Asbestos containing insulation used to inhibit heat transfer or prevent condensation on pipes, boilers, tanks, ducts and various other components of hot and cold water systems, and heating, ventilation and air conditioning (HVAC) systems. 3) Miscellaneous Materials - Other largely non-friable products and materials such as floor tile, ceiling tiles, mastics, roofing felt, concrete pipe, siding, fabrics, etc. The Public Building Commission of Chicago Haas Park Field House PS North Washtenaw Street, Chicago, IL Page 3

7 Materials should generally be presumed to contain asbestos, unless they have been sampled and analyzed by appropriate laboratory analysis techniques such as Polarized Light Microscopy (PLM) or Transmission Electron Microscopy (TEM). There are several health effects associated with asbestos exposure. The two (2) main routes of exposure for asbestos in the body are inhalation or ingestion through the nose and mouth. The asbestos fibers will either be inhaled into the lungs or ingested into the stomach. The health effects generally associated with asbestos exposure include: Asbestosis is a disease caused by the inhalation of asbestos fibers into the lungs, thereby causing the scarring of tissues. It is a restrictive lung disease which reduces the overall volume and function of the lung. Lung Cancer is caused when abnormal cells within the lung tissue grow uncontrollably and form tumors. A cigarette smoker is fifty (50) times more likely to develop lung cancer if they are also exposed to asbestos. Mesothelioma is a cancer of the chest cavity (pleural lining), or abdominal cavity (peritoneal lining). This is the least common of the diseases caused by asbestos, however it is always fatal. Other diseases include: cancer of the esophagus, stomach, colon, pancreas, pleural plaques, pleural thickening, and pleural effusion. 2.2 Objective Asbestos Containing Material Survey The purpose of the asbestos survey was to identify the location of asbestos-containing materials within the interior and exterior of the Haas Park Fieldhouse in Chicago, Illinois. To achieve this purpose, the following procedures were performed: Visual Inspection to determine the location of suspect materials. Bulk Sampling samples are taken in random locations, to provide representative sampling for each suspect material. Sample Analysis to determine the type and % of asbestos in the material. Materials containing greater than 1% asbestos are considered asbestos-containing. Reporting to prepare a summary report documenting the inspection findings and providing recommendations as warranted Hazardous Material Survey The objective of the hazardous material survey was to determine the presence of leadbased paint, PCB s, and/or mercury containing equipment. To achieve this objective, the following procedures were performed: Visual Inspection - to determine location and quantity Sampling lead-based paint sampling to determine the presence of this constituent Reporting to prepare a summary report documenting the inspection findings and provide recommendations as warranted. 8 The quantities provided in this report only represent the materials found on the day of inspection. Between the time of the inspection and hazardous materials removal, additional chemicals and equipment may be brought onto the property. Conversely, chemicals, equipment and waste materials may be consumed and/or disposed prior to a removal date. 2.3 General Facility Description The subject site is a Chicago Park District Fieldhouse located at 2404 North Washtenaw Street in Chicago, Illinois. The subject site is a one story building and is approximately 1,700 square feet in size. The interior space is primarily constructed of brick and masonry walls, wood ceiling panels, and vinyl floor tile covering concrete. The exterior construction is primarily brick and masonry with a pitched roof covered with asphalt shingles. The heating inside the building consists of a hot water radiator system. 9 3.0 SITE INSPECTION 3.1 Asbestos Containing Material Survey Asbestos Survey Methodology The asbestos survey consisted of two phases. The first phase of the survey consisted of a walkthrough inspection of the subject site to identify homogeneous areas (materials which are uniform in composition throughout) and to assess material condition. The second phase consisted of collecting representative bulk samples from the suspected asbestos-containing materials Suspect Asbestos Containing Material Sample Collection Carnow Conibear collected a total of thirty-seven (37) samples from the suspected asbestos-containing materials. All bulk samples were collected according to Carnow Conibear s Standard Operating Procedures for Bulk Sampling manual. These procedures were developed in-house and are based on methods described in USEPA guidelines. The samples were collected and stored in sample bags with a unique sample identification number prior to delivery to STAT Analysis Corporation (STAT) for analysis. A chain of custody (COC) form was signed and dated by the inspector and the laboratory representative who received the samples. Note: The water supply, electrical power, and small boiler remained in operation at the time of the inspection. Because of this, walls near the restroom pipes were not demolished to inspect for pipe insulation inside wall cavities, the electrical panel was not inspected for the presence of asbestos containing cement board or other asbestos containing materials behind the metal panel, and any insulation inside the small boiler was not inspected. Prior to demolition, these areas should be inspected for ACM Asbestos Sample Analysis The samples collected during the field activities were submitted to STAT for laboratory analysis. STAT s laboratory is accredited for bulk asbestos fiber analysis by the National Voluntary Laboratory Accreditation Program (NVLAP) through the National Institute of Standards and Technology (NIST). STAT utilized dispersion staining and polarized light microscopy (PLM) techniques for analyzing the samples consistent with National Institute for Occupational Safety and Health (NIOSH) methods. PLM is the EPA s recognized method for determining bulk asbestos content. The results of the laboratory testing of the asbestos samples revealed the presence of asbestos-containing materials at the subject site. Table I presents the results of the asbestos containing materials sample analysis. See Appendix A for asbestos bulk sample locations and approximate locations of all identified asbestos materials and Appendix B for the bulk sample laboratory report and chain of custody documentation. 11 3.2 Hazardous Material Survey Hazardous Material Survey Methodology The hazardous material survey consisted of two phases. The first phase of the survey consisted of a visual inspection to determine the location and quantity of hazardous and potentially hazardous materials that may require special disposal. The second phase consisted of a lead-based paint sampling. Table II identifies the materials found in this building and approximate quantities of the materials. The hazardous material survey consisted of visually inspecting the subject site to determine the presence and location of polychlorinated-biphenyl (PCB)-containing equipment and materials (lighting ballasts, switchgears, transformers, and hydraulic fluids), and mercury-containing equipment and materials (mercury lamps, thermostats, switches, thermometers, regulators, and gauges). 12 3.2.2 Lead-Based Paint Survey Methodology Sampling of visible and accessible suspect lead-based painted components and/or surfaces was conducted following modified U.S. Department of Housing and Urban Development (HUD) guidelines of June 1995 for single family housing, Chapter 7: Lead- Based Paint Inspection 1997 Revision, and the EPA and HUD s Performance Characteristics Sheet for the RMD LPA-1 XRF lead paint analysis system. These guidelines apply to residential properties. Carnow Conibear modified these guidelines and tested typical major building components. The sampling was conducted with an XRF spectrum analyzer using a Co-57 isotope with a 12 millicurie source. Painted surfaces which indicate a concentration of 1.0 mg/cm 2 or greater are considered to be lead-based paint as defined by HUD. The survey results were recorded directly in the instrument memory during the field activities. Table III identifies the lead painted components and/or surfaces as defined by HUD. Paint sampling was collected from walls, ceilings, and other substantial architectural components. See Appendix C for a complete testing log of all XRF measurements collected for this assessment. 13 4.0 CONCLUSIONS AND RECOMMENDATIONS Carnow, Conibear & Assoc., Ltd. (Carnow Conibear) was retained by PBCC to perform an asbestos-containing material and hazardous material survey at the Haas Park Fieldhouse located at 2404 North Washtenaw Street in Chicago, Illinois. The survey revealed the presence of asbestos-containing materials and hazardous materials. Based on the survey results, Carnow Conibear recommends the following: Incorporate the data from this report into future demolition/renovation documents regarding the presence of asbestos-containing materials and hazardous materials. All future asbestos removal and/or demolition/renovation work involving the asbestoscontaining materials shall be conducted by a licensed contractor in accordance with IDPH, NESHAPS, and OSHA regulations and requirements. Dispose of all asbestos-containing materials generated during removal and/or demolition/renovation work in accordance with all applicable local, state, and federal regulations. Prior to demolition, remove, dispose, and/or recycle all regulated universal waste materials as identified in this report. All work involving the disturbance of lead based paint shall be in accordance with the IDPH and OSHA regulations and requirements. The demolition/renovation contractor shall utilize engineering control methods to reduce or eliminate potential lead exposures for the site workers. Note: The water supply, electrical power, and small boiler remained in operation at the time of the inspection. Because of this, walls near the restroom pipes were not demolished to inspect for pipe insulation inside wall cavities, the electrical panel was not inspected for the presence of asbestos containing cement board or other asbestos containing materials behind the metal panel, and any insulation inside the small boiler was not inspected. Prior to demolition, these areas should be inspected for ACM. Carnow Conibear has applied prevailing industry standards and reasonable judgment and effort within the scope of work outlined in Carnow Conibear s proposal, while conducting the asbestos-containing material and hazardous material survey. The standards, judgment, and effort used by Carnow Conibear personnel to investigate, assess, and determine the presence of potential environmental hazards and liabilities associated with the subject building are consistent with requirements outlined in federal and state guidelines. Carnow Conibear makes no warranty, express or implied, that the findings and interpretations in this report are a complete representation hazards and liabilities, associated with the building. Findings presented in this report are only indicative of conditions present during the time of the investigation and cannot be used to predict potential future or previous health effects on building occupants. The services performed by Carnow Conibear on this project have been conducted in a professional manner consistent with industry standards at the time of testing. There may be materials that were not identified, because they were located in inaccessible areas and not available at the time of inspection. Carnow Conibear made every reasonable effort to locate mechanical systems and other inconspicuous materials.
