3 INSTRUCTIONS TO SUBMITTERS I. INTRODUCTION & BACKGROUND The Community and Economic Development Department, Land Development Engineering Division invites Proposers to submit a written proposal to provide construction management services for the Community and Economic Development Department, Land Development Engineering Division as set forth in the Scope of Work. The Community & Economic Development Department (CITY) is soliciting proposals for Construction Management Oversight, Public Outreach, Traffic Management Coordination, Soils/Materials Testing Services, and Construction Surveying Services. The requested services will be for the State Route 99 and Pelandale Interchange Reconstruction Project. The CITY is the administering agency for the construction of the interchange project., in cooperation with Caltrans District 10, proposes to reconstruct the State Route 99 and Pelandale Avenue interchange. This project will help facilitate future growth, help alleviate traffic congestion, and will improve the operation of this interchange. Increasing the efficiency of the interchange will also encourage the free flow of goods and services for the region and the State. The proposed interchange improvements include reconstruction of the existing interchange to provide improved operations for turning movements to and from State Route (SR) 99, as well as associated local road improvements at adjacent intersections. The overall project is being funded by the Capital Facilities Fees (CFF), Federal Funds; and the State Route 99 Proposition 1B Bond Savings funds. The Bond Savings funds will provide up to $43.8 million toward the construction phase of the project. The advertisement and award of the construction contract for this project are dependent on the bond funding being allocated to the project. This funding allocation will be determined in October 2013 by the California Transportation Commission (CTC). Therefore, the award of this consultant contract is also dependent on the allocation of the bond funds to this project. II. PURPOSE OF THE REQUEST FOR PROPOSAL (RFP) Therefore, the purpose of this Request for Proposal is to select a qualified consultant to assist the City with Construction Management Oversight, Public Outreach, Traffic Management Coordination, Soils/Materials Testing Services, and Construction Surveying Services. The result of this RFP process will be to select a consultant to contract with the City to perform requested services. Prior to the agreement being executed, the consultant will be subject to a pre-award audit by Caltrans. The pre-award audit examines the consultant s accounting, estimating, administrative systems, proposed costs, and financial condition. The audit is as broad in scope as necessary to meet the objectives found in Exhibit 10-M Standard Audit Program Procedures of the Local Assistance Procedures Manual. 5 IV. SCOPES OF SERVICES CONSULTANT shall furnish Construction Management personnel to coordinate CONSULTANT operations with the CITY. For this RFP Construction Management personnel (including sub-consultants) shall include but not limited to a Project Manager, Resident Engineer, Assistant Resident Engineer, Lead Field Inspector, Field Inspector, Structural Representative, Structural Inspector, Office Engineer, Material Testing/Source Inspector Project Manager, Material Technicians, Construction Survey Project Manager, Survey Party Chief, Survey Crews and Landscape Architect and shall be responsible for all matters related to CONSULTANT personnel and coordinated construction activities under this contract. The City will have a project manager (City Project Manager) assigned specifically to the project to be available for the project duration to aid in the overall contract administration and construction management, including but not limited to, construction inspections, storm water compliance, quantity calculations, progress payments, claim avoidance, labor compliance, and public relations. The CONSULTANT shall be responsible for construction oversight, which includes but is not limited to the overall contract administration and contract compliance, contract change order review, schedule changes, traffic management handling analysis, to assist with public outreach and coordinate with other construction projects to help avoid significant traffic delays. The number of CONSULTANT personnel assigned to the project will vary throughout the duration of the contract. CONSULTANT personnel will be assigned, in varying levels of responsibility, as needed by the CONSULTANT to meet the project schedule, project requirements, and construction activities. Resumes of personnel must be submitted to the CITY for review and approval prior to assignment to the Project. The CITY and CONSULTANT will jointly determine the quality and quantity of services that are required by CONSULTANT personnel. Personnel selected for assignment by CONSULTANT shall be made available for personal interviews prior to acceptance by the CITY. If, in the opinion of the CITY, an individual lacks adequate experience, the individual may be rejected or may be accepted on a trial basis until such time the individual's ability to perform the required services has been demonstrated. If, at any time, the performance of CONSULTANT personnel is unsatisfactory to the CITY, the CITY may release him/her by written notice and may request another qualified person be assigned. If CONSULTANT personnel are on leave of absence, the CONSULTANT firm shall provide approved, equally qualified replacement personnel until the assigned personnel returns to the Project per acceptance of the City Project Manager. The typical workday includes all hours worked by the construction Contractor. The construction Contractor's operations may be restricted to specific hours during the week, which shall become the normal workday for CONSULTANT personnel. The Project Page 5 of 39

6 Manager, with concurrence from the CITY, shall have the authority to increase, decrease, or eliminate CONSULTANT personnel work hours dependent on the schedule and requirements of the construction Contractor. CONSULTANT personnel shall be knowledgeable of and comply with all applicable local, state, and federal regulations. CONSULTANT personnel shall cooperate and consult with the CITY, Caltrans, and permitting agencies during the course of the Project. CONSULTANT personnel shall perform duties as may be required to assure that construction is being performed in accordance with the project plans, specifications & estimates (contract plans and special provisions). All services required hereunder shall be performed in accordance with California Department of Transportation guidelines, regulations, policies, procedures, manuals, and standards, and Cooperative Agreement (Appendix B see attached) except as noted in the special provisions. V. DUTIES AND RESPONSIBILITIES Task 1 Construction Management Oversight - Preconstruction and Bid Services Review plans and specifications and provide a list of items to the City Project Manager on potential conflicts, constructability issues, value-engineering opportunities and consistency between the plans, specifications and estimates. 1.1 Assist in evaluation of bids received. Review of bid for completeness and responsiveness. 1.2 Review contract plans, specifications, estimates, quantities, permits and agreements. 1.3 Review Resident Engineer files. Resident Engineer s files consist of Design Engineer memos to Resident Engineers, and technical reports and studies. 1.4 Review the project schedule which includes all preconstruction and utility relocations by others, and notification timelines noted on all permits, agreements, and contract documents. Upon receipt of contractor s schedule, evaluate and confer with the City Project Manager regarding workability of the schedule or suggest changes that may improve the schedule. 1.5 Review any material related to dispute resolution with contractor s methodologies, policies, and procedures. Work with the City Project Manager to assist as necessary. 1.6 Schedule, coordinate, host a pre-construction meeting with the Contractor, City, and Caltrans. Page 6 of 39

7 1.7 Development of contractor payment schedules, and other procedural items. 1.8 Checking Contractor references, licenses, insurance and sureties. Task 2 Construction Management Oversight - Construction Management Services during Construction This task includes construction management oversight, project observation/inspection during construction. Consultant Management Services shall be in accordance with the Caltrans Construction Manuals. Services include the following: 2.1 Mobilization Coordinate with contractor for proper set up and organization of a field office. Office space for the Construction Management team will be provided by the contractor. 2.2 Project Coordination and Correspondence Coordinate and assist as necessary with contractors, surveyors, material testers, the Design Engineer, other agencies, utility companies, and other parties. Maintain close contact with the City Project Manager, Caltrans Construction Manager, and Design Engineering firm on all correspondence. Review all City Project Manager, Resident Engineer and Contractor correspondence as necessary. Coordinate with applicable parties, as required, to assist in developing responses as necessary. Schedule, Coordinate, and Attend weekly, or as necessary, construction contract coordination meetings with the Contractor and City Project Manager. Schedule, Coordinate, and Attend as necessary, construction contract coordination meetings with the other project and construction managers in the vicinity of project. Perform labor compliance reviews and correspond with contractor with any outstanding issues. 2.3 Schedule Management, Progress Meetings, and Reports Review planned schedule for conformance with the specifications and for reasonableness of the sequence and duration of the activities. Review work progress as compared to the planned schedule and inform the City Project Manager and advise the City upon request of schedule slippage. Analyze schedule to determine impact of weather and change orders. Obtain from Resident Engineer updates of construction schedule incorporating actual Page 7 of 39

8 progress, weather delays, and change order impacts. Resident Engineer to analyze negotiated time extensions due to change orders and other delays, report to the City Project Manager. Prepare and submit a monthly progress report to the City Project Manager describing key issues, cost status, and schedule status. 2.4 Payment Recommendations Review Contractor s initial schedule of values for reasonableness and ease of monitoring. Review quantities submitted with monthly progress payment requests, analyze differences over amount. Prepare and submit monthly progress payment spreadsheet to the City Project Manager for payment processing. 2.5 Requests for Information (RFI) and Material Submittals Review and monitor all Requests for Information (RFI) from Contractor. Review, respond, and/or track responses to RFI related to construction issues. Relay design-related RFI to Design Engineer. Process response to contractor in a timely manner. Attend meetings with the City Project Manager, Contractor and other parties, as needed, to discuss and resolve any outstanding RFIs. Collect, log, review, distribute, track, and respond to all material submittals submitted by the contractor. 2.6 Contract Change Orders (CCOs) Review ALL change orders related to construction issues based on drawings, specifications, and other design information from Design Engineering firm or other project consultants. Perform change order analysis, including reviewing: logs of proposed change orders, change order quotations from Contractor, negotiated change order costs, time extensions, processing final negotiated change orders, and effect of approved change orders in progress payment breakdowns. CCO s (less than $50,000) - Prepare CCOs and recommendations to accompany change order documents and forward to the City Project Manager for review and approval. CCO s ($50,000 and greater) Prepare CCO s and recommendations to Page 8 of 39

9 accompany change order documents and forward to City Project Manager and Caltrans Construction Manager 2.7 Construction Observation/Inspection Services Provide day-to-day on-the-job observation/inspection of all construction work on the project. CONSULTANT inspectors shall make reasonable efforts to guard the City and Caltrans against defects and deficiencies in the work of the Contractor and to ensure provisions of the contract documents are being fulfilled; prepare daily inspection reports documenting observed construction activities; take progress photographs and bind and label them; review contractor record drawing markups; punch lists; coordinate with the City Project Manager for final inspection; and assist with all other matters relating to construction of the project. CONSULTANT inspectors shall verify layout of work prior to the beginning of the permanent work, including prior to foundation being installed, concrete being poured, and wall being erected etc. CONSULTANT to provide a California Licensed Landscape Architect for purposes of aiding on inspections and approval of materials for all landscape features on the project. CONSULTANT to provide a California Licensed Structure Representative for the purpose of inspections and approval of materials for all structural features on the project. CONSULTANT to provide qualified staff to perform paleontological monitoring and salvage during construction excavation activities per the Paleontological Mitigation Plan and the Environmental Commitments Record. Prepare monthly progress payment recommendations by making measurements of bid items on the project cost breakdown, checking the percent complete in the field, and assisting with Contractor meetings to resolve any differences in percent complete. CONSULTANT shall ensure that the Project meets all provisions of the Storm Water Pollution Prevention Plan (SWPPP). CONSULTANT shall assure that the Project meets all applicable regulations of the Air Quality Management District (AQMD) and State Water Resources Control Board (SWRCB). CONSULTANT shall ensure that the Project meets all provisions of the Caltrans Quality Assurance Program Manual. CONSULTANT shall review and ensure compliance with environmental requirements. 10 2.8 Claims Avoidance/Management CONSULTANT shall implement all claims avoidance techniques, including review Contractor s initial schedule submittal and providing feedback for improvement. The Resident Engineer will continue to work with the Contractor until the schedule clearly identifies the critical path of item of the project and the Resident Engineer approves the schedule. Then during the duration of the project, the Resident Engineer will monitor the actual performance of the work and compare it to the approved schedule. The Resident Engineer must continually require the contractor to address slippage in the approved schedule and submit an updated schedule showing approved additional days, if any. Review additional compensation claims that are submitted during the construction period. Provide claim administration, including coordinating and monitoring claims response preparation, logging claims, and tracking claims status. Prepare any necessary documentation for use in dispute resolution hearings. 2.9 Record Drawings Confirm on a monthly basis that the Contractor is maintaining up-to-date marked up prints of construction drawings and documents showing all field changes and as-built conditions. Maintain the same drawings and documents in the construction field office. Task 3 Public Outreach This task includes all public outreach efforts, including but not limited to a ground breaking ceremony, regular updates to the local newspaper, a website that will have photos and regular updates, and a ribbon cutting ceremony at project completion. 3.1 Construction Information Coordinate with Project Manager/Resident Engineer to obtain accurate and up to date construction information in preparation of Public Outreach publications and reports. Maintain close contact with Caltrans Public Information Officer, the City Project Manager and City Public Information Officer on all correspondence. Provide periodic status updates to City Council in regularly scheduled City Council Meetings. Develop Public Outreach strategies based on Project Manager/ Resident Page 10 of 39

11 Engineer, Caltrans and City Public Information Officer, and City Project Manager as necessary. Coordinate with applicable parties, as required, to assist in developing responses as necessary. Attend weekly, or as necessary, construction contract coordination meetings with the Project Manager/Resident Engineer. Develop and maintain contents for website. Emergency notices when needed. Develop a Project fact sheet (including project description, Project budget, Project schedule, LOCAL AGENCY contact information, photos, etc). Prepare Public Outreach information for web updates and newspaper updates in preparation for temporary construction impacts to the traveling public. Plan, coordinate, and run the project ground breaking and ribbon cutting ceremonies. Task 4 Traffic Management Coordination 4.1 Project Preconstruction Analyze Project Traffic Management Plan(s) during pre-construction reviews. Review project staging plans and propose revisions as necessary for construction and design. 4.2 Project Coordination and Communication during Construction CONSULTANT shall coordinate with City Project Manager and Contractor to review project construction baseline schedule. Review Contractor s baseline schedule to identify conflicts with staging and traffic management plans. Review traffic management plan and staging plans for potential conflict with adjacent or nearby projects that are already under construction. Analyze Traffic Management Plan conflicts between projects and recommend revisions to project construction baseline schedules as necessary to reduce the length of temporary impacts during construction to traffic. Delay analysis. Consultant to provide weekly input of lane closure requests to City Project Manager. Task 5 Soils/Materials Testing Services Page 11 of 39

12 5.1 Certified Laboratory for Soils/Material Testing Services CONSULANT shall provide a sub-consultant as a certified materials testing agency (MTA) to perform soils and materials sampling and testing services. Material tester shall be certified to perform soils and material sampling and testing services. The soils/materials testing sub-consultant shall refer to the Caltrans Quality Assurance Program for testing sampling and testing guidelines. For all HMA and RHMA-G construction, the QA/QC construction process will be followed per Section 39 of the 2010 State Standard Specifications. The laboratory, whether temporary or permanent, is to be in the general vicinity of the project area and no more than 50 miles from the field office for the project. The materials testing agency (MTA) shall have an Independent Assurance Program to verify that acceptance testing is being performed correctly with properly calibrated equipment in good working order. All test results shall be reported to the Project Manager/Resident Engineer in the shortest time that the specific test will realistically allow. Failure to comply with specified time limits for test results may result in payment penalties of up to 20% for the tests due to late notification of the results. The CONSULTANT guarantees that the offered equipment, material or services meet all safety requirements applicable in accordance with Cal-OSHA regulations and any other rule or regulation required by the City. Prevailing Wages: All services performed under the agreement will be subject to payment of the applicable prevailing wage to the testing personnel performing the tests. The submittal of certified payroll records will be required as work is performed or when they are needed to verify that the Consultant is in compliance with the State Labor Code. Certified payroll submittals will be to either the City or directly to the Department of Industrial Relations (DIR). If the project fails any tests or inspections, retests or re-inspections will be required. Retests and re-inspections will be billed at the same rates as the original tests. Retests shall be marked on reports and invoices to help facilitate back charging to the contractor, if allowed by the project contract. Soils and materials testing agency/consultant shall submit to the City Project Manager all applicable certifications for the laboratory and testing personnel that will be working on the project. All certifications must be kept current Page 12 of 39

13 throughout the project duration. If certifications are updated while the project is underway, the updated certificate shall be submitted to the City immediately. It is the intent of the City to maintain a consistency of material testing quality throughout each phase of the project. CONSULTANT is therefore encouraged to provide, where ever and whenever possible, the same field personnel for the duration of construction of the project. On days when work is not performed by the construction contractor, such as rainy or unsuitable weather days, the materials testing personnel will not provide services unless authorized by the Construction Manager. Resumes of materials testing personnel must be submitted to the City for review and approval prior to assignment to the Project. If, at any time, the level of performance of any testing personnel is below expectations, the City may release that field person and request that another be assigned as needed. Task 6 Construction Surveying Services CONSULTANT shall provide Surveying Services to perform construction surveys for the project. Survey personnel to include: a Construction Survey Project Manager, Field Party Chief and Survey Crew. The number of Survey Crew(s) and Field Party Chief(s) assigned to the project will vary throughout the duration of the construction contract. Personnel provided by the CONSUTANT will be assigned as needed by the Project Manager/Resident Engineer to meet the schedule of the construction contractor. Surveys will include but may not be limited to temporary construction staking, boundary or right of way staking, topographic surveys, and monument preservation. 6.1 Construction Surveying Services It is the intent of the City to maintain a consistency of construction survey quality throughout each phase of the project. Therefore, CONSULTANT is encouraged to provide the same field personnel for the duration of construction. It is important that the Party Chief(s) assigned to the project be completely familiar with the survey requirements and the assignments for the project. On days when work is not performed by the construction contractor, such as rainy or unsuitable weather days, CONSULTANT will not provide services unless authorized by the City Project Manager. Construction surveying will not be performed when conditions such as weather, traffic, and other factors prevent safe and efficient operation. Resumes of the survey personnel must be submitted to the City for review. CONSULTANT or sub-consultant personnel must be approved by the City Page 13 of 39

14 Project Manager prior to assignment to the project. The City and CONSULTANT will have the responsibility of determining the quality and quantity of work performed by the survey personnel. If, at any time, the level of performance by the survey personnel is below expectations, the City may release the survey crew member and request that another be assigned. If survey crew personnel assigned to the project are on a leave of absence, the Project Manager or the Construction Survey Project Manager will provide an equally qualified replacement(s) until the original employee(s) returns to work. The replacement will be required to meet all the requirements of the permanently assigned employee. Prevailing Wages: All services performed under the agreement will be subject to payment of the applicable prevailing wage to the survey personnel performing the surveying. The submittal of certified payroll records will be required as work is performed or when they are needed to verify that the CONSULTANT or sub-consultant is in compliance with the State Labor Code. Certified payroll submittals will be to either the City or directly to the DIR. Task 7 - Construction Management Oversight - Post Construction Services (Close Out/Claims) The post construction services task includes project closeout after issuance of substantial completion for the construction Contractor. This task will review punch lists items of remaining work, resolve outstanding contract change orders, global settlements, dispute resolution, and arbitration meetings. The design consultant will provide project changes to original design mylars, and provide as-built plans at the project completion. Review and process the final payment. VI. MEMBER QUALIFICATIONS The quantity and qualifications of field personnel to be assigned will be determined by the scope of the Project and the degree of difficulty of required tasks to be performed. All personnel and personnel assignments are subject to approval by SANBAG. While some areas of responsibility may overlap, as a guideline, CONSULTANT personnel assigned to the Project should have the following qualifications: 1. Project Manager a. A minimum of six (6) years project management experience on similar construction projects is desired. b. Accessible to City Project Manager at all times during normal working hours. Page 14 of 39

15 c. A thorough understanding of Caltrans construction practices and procedures The Project Manager will assume the following functional responsibilities: 2. Resident Engineer 1) Review, monitor, train and provide general direction for CONSULTANT personnel. 2) Assign personal to projects on an as-needed basis. 3) Administer personal leave. 4) Prepare monthly reports for delivery to City Project Manager. a. A minimum of eight (8) years' resident engineer experience on similar construction projects is desired. b. Licensed Professional Civil Engineer in the State of California. c. Ability to work independently, makes effective decisions concerning construction work in progress, and solve field problems. d. Accessible to City Project Manager at all times including weekends and holidays. e. Certified as a Qualified SWPPP Developer or Practitioner is desired. f. A thorough understanding of Caltrans field methods, practices, and construction office procedures. The Resident Engineer will assume the following functional responsibilities: 1) Monitor and provide daily direction to CONSULTANT Resident Engineers and inspection personnel. 2) Assign field personnel to specific project tasks. 3) Monitor and track Contractor progress. 4) Prepare daily, weekly and monthly reports as required. 5) Coordinate utility relocations with appropriate agencies and the utility inspector. 6) Act as prime field contact between various project Contractors, Caltrans and City Project Manager. 3. Assistant Resident Engineer a. A minimum of four (4) years experience on similar construction projects is desired. b. Licensed Professional Civil Engineer in the State of California. Page 15 of 39

16 c. Ability to work independently, makes effective decisions concerning construction work in progress, and solves field problems. d. Accessible to City Project Manager at all times including weekends and holidays. e. Certified as a Qualified SWPPP Developer or Practitioner is desired. f. Thorough understanding of Caltrans field methods, practices, and construction office procedures. The Assistant Resident Engineer shall act under the direction of the Resident Engineer and will assume the following functional responsibilities: 4. Lead Field Inspector 1) Monitor and provide daily direction to CONSULTANT inspection personnel. 2) Assign field personnel to specific project tasks. 3) Monitor and track Contractor progress. 4) Prepare daily, weekly and monthly reports as required. 5) Coordinate utility relocations with appropriate agencies. a. A minimum of six (6) years' construction inspection experience in public works or similar projects or a four-year degree in the field of civil engineering, transportation and four years of similar construction experience is desired. b. Knowledge of Caltrans construction practices, physical characteristics and properties of highway construction materials, and approved methods and equipment used in making physical tests of construction materials. c. Ability to work independently, makes effective decisions concerning construction work in progress, and solves field problems. d. Ability to direct the efforts of subordinate inspectors. e. Ability to use an automatic level and transit for the purpose of verifying line and grade. f. Certified as a Qualified SWPPP Developer or Practitioner is desired. Page 16 of 39

17 g. Understanding of Caltrans field and construction office procedures. The Lead Field Inspector will assume the following functional responsibilities: 5. Field Inspector 1) Assist in inspections to assure compliance with contract plans, specifications, and special provisions on all phases of construction. 2) Assist in the preparation of contract change orders, contract estimates, progress pay estimates, and other documents or reports required for the Project. 3) Coordinate field testing of materials to monitor compliance with Project specifications and Caltrans Quality Assurance Program. 4) Maintain accurate and timely Project records. Perform quantity calculations for progress pay estimates. 5) Perform analytical calculations such as basic earthwork, grading, profiles, and special staking procedures. 6) Provide input for the redesign of facilities to fit existing field conditions. 7) Monitor and track Contractor progress. Prepare daily, weekly, and monthly reports as required. a. Two (2) years' construction inspection experience in public works or similar projects or a four-year degree in the field of civil transportation engineering is desired. b. Knowledge of construction practices, physical characteristics and properties of construction materials, and approved methods and equipment used in making physical tests of construction materials. c. Understanding of Caltrans construction methods and practices. The Field Inspector will assume the following functional responsibilities: 1) Assist in inspections to assure compliance with contract plans, specifications, and special provisions on all phases of project construction. 18 2) Assist in preparation of contract change orders, contract estimates, progress pay estimates, and other documents or reports required for the Project. 3) Coordinate field testing of materials to monitor compliance with Project specifications and Caltrans Quality Assurance Program. 4) Perform quantity calculations for progress pay estimates and maintain Project records. 5) Perform analytical calculations s u c h as basic earthwork, grading, Profiles and special staking procedures. 6) Provide input for the redesign of facilities to fit existing field conditions. 7) Perform construction materials sampling. 8) Perform labor compliance interviews of the Contractors personnel. 6. Structural Representative or Lead Structural Inspector a. A minimum of six (6) years' of bridge or structural construction inspection as related to major public works projects and a fouryear degree in civil engineering is desired. b. Licensed Professional Civil Engineer in the State of California. c. Knowledge of stress analysis, structural mechanics, and strength of materials. d. Knowledge of Caltrans construction practices and the physical characteristics and properties of various bridge construction materials including concrete. e. Experience in the following areas: foundations, pile driving, concrete pre-stressing, bridge deck construction, cast-in-place wall construction, false work, and shoring. f. Ability to work independently. Ability to perform duties in the construction field office and effectively make decisions concerning construction work in progress and solving field problems. g. Ability to direct the efforts of subordinate inspectors. h. Ability to use an automatic level and transit for the purpose of verifying line and grade. i. Thorough understanding of Caltrans field methods, practices, and construction office procedures. The Structural Representative shall assume the following functional responsibilities: Page 18 of 39

Structural Inspector 1) Assist in inspections to assure compliance with contract plans, specifications, and special provisions on all phases of structural construction such as bridges, foundations, walls, false work, shoring, and drainage structures. 2) Make grade, alignment, quantity, false work, and shoring calculations. 3) Assist in the preparation of contract change orders, contract estimates, progress pay estimates, and other documents or reports required for the Project. Coordinate the sampling and testing of construction materials to monitor compliance with contract specifications. 4) Monitor and track Contractor progress. Prepare daily, weekly, and monthly reports as required. 5) Coordinate with Resident Engineer, as well as the Construction Staking and Material Testing consultants. 6) Direct the daily activities of subordinate inspectors. a. A minimum of two (2) years' of bridge design or structural construction inspection as related to Caltrans or major public works projects or a four-year degree in civil or structural engineering is desired. b. Knowledge of stress analysis, structural mechanics, and strength of materials. c. Knowledge of construction practices and the physical characteristics and properties of various highway bridge construction materials including concrete. d. Experience in one or more of the following areas: pile driving, concrete pre-stressing, bridge deck construction, retaining wall construction, false work, and shoring. e. Ability to work independently and make minor decisions concerning construction work in progress and to solve field and office problems. f. Ability to use an automatic level and transit for the purpose of verifying line and grade. g. Understanding of Caltrans construction methods and practices. The Structural Inspector will assume the following functional responsibilities: 1) Assist in inspections to assure compliance with contract Page 19 of 39

Office Engineer plans, specifications, and special provisions on all phases of structural construction such as bridges, foundation, walls, false work, shoring, and drainage structures. 2) Make grade, alignment, quantity, false work, and shoring calculations. 3) Assist in the preparation of contract change orders, contract estimates, progress pay estimates, and other documents or reports required for the Project. 4) Sampling and testing of construction materials to monitor compliance with contract specifications and Caltrans Quality Assurance Program. a. A minimum of two (2) years' experience as an office engineer on similar construction projects is desired. b. Knowledge of Caltrans Office of Highway Construction forms used to administer construction projects. c. Knowledge of Caltrans system of document organization. d. Knowledge of construction records and accounting procedures. e. Knowledge of documentation, procedures, and reporting for federally funded projects. f. Knowledge of laws and regulations governing the payment of prevailing wages. g. The Office Engineer will assume the following functional responsibilities: 1) Process monthly progress pay estimates, monthly status reports, extra work reports, and weekly statements of working days. 2) Prepare and process contract change orders. 3) Monitor construction budget and schedule. 4) Prepare, maintain, and/or file project documents including labor and equipment records, correspondence, memoranda, meeting minutes, claims, personnel records, labor compliance reports, and daily, weekly, and monthly reports. 5) Perform routine calculations and checking of quantities. 6) Coordinate all office activities and functions with City Project Manager. 21 9. Materials Testing/Source Inspection Project Manager a. A minimum four (4) years' project management experience on a similar highway/bridge construction project is desired. b. Licensed Civil Engineer in the State of California. c. Ability to use typical computer programs including word processing and spreadsheets. d. The materials testing/source inspection project manager will assume the following functional responsibilities: 1) Review, monitor, train, and provide general direction for CONSULTANT s laboratory, field and source inspection personnel. 2) Assign personnel to projects on an as-needed basis in coordination with the Resident Engineer. 3) Administer personal leave, subject to approval of the Resident Engineer. 4) Prepare monthly reports for delivery to the Resident Engineer. 5) Provide direction, administration, and responsibility for Materials Certification per Caltrans Construction Manual... 6) Provide direction, administration, and responsibility for implementation of the approved SIQMP per Caltrans Construction Procedures Directive (CPD) 08-5 and FHWA Title 23 requirements. Material testing/source inspection personnel will be certified by a California Registered Civil Engineer as being experienced and competent in the test procedures required for the work involved (and possess a current certificate of proficiency (Form MR-0111) in accordance with Quality Assurance Program Manual (Section 3-5). Independent certification of Caltrans test procedures may be performed at the discretion of the City Project Manager. 10. Materials Technicians CONSULTANT personnel provided under this contract will have a variety of skills and experience appropriate for the level of tasks to be assigned. Field personnel shall be certified by Caltrans and should have a minimum of two (2) years' experience in conducting material sampling and testing of the type required for the projects involved and possesses the following additional capabilities: Page 21 of 39

22 a. Have the ability to establish specific locations for appropriate tests when construction contract administration personnel are not available. b. Be familiar with construction practices and be fully aware of construction activities at the Project site. c. Have knowledge of and comply with safety and health regulations and requirements applicable to the Project. d. Specific qualifications for technicians are as follows: 1) CONSTRUCTION TECHNICIAN I a) Performs a variety of semi-skilled activities. Examples of duties assigned to this classification are: i. Conducting quality control tests such as soil densities, sieve analysis tests, operation scales and inspecting spread operations. ii. Sampling and transporting produced construction materials from point of application or production to testing laboratory. b) Knowledge and Skills Required i. Knowledge of tools, equipment and vehicles utilized in construction. ii. Knowledge of standard equipment and materials used for the sampling and testing of construction material. iii. Knowledge of basic mathematics used in the computation of a variety of construction items. iv. Knowledge of record keeping, preparing of documents and reports. 2) CONSTRUCTION TECHNICIAN II a) Performs a variety of skilled activities. Examples of duties assigned to this classification are: i. Inspecting minor construction items, sampling and inspection of steel Page 22 of 39

23 reinforcement, sampling and inspection of concrete placing operation. ii. Collect and analyze soil samples of construction materials to determine compaction and moisture content. iii. Inspection and sampling of all phases of asphalt concrete and PCC paving operation, including plant inspection. iv. Confers with construction engineers and contractors regarding construction in progress and is conformance to specifications and construction plans. v. Answers questions and resolves problems. vi. Inspects construction in progress to ensure conformance with specification, agreements, and established requirements. vii. Keeps daily diary of work progress. viii. Prepares reports on all field inspections and submits project quantities on a daily basis. ix. Keeps accurate documentation for force accounts and possible claims. b) Knowledge and Skills Required i. All knowledge and skills required of lower classification. ii. Knowledge of currently accepted methods, procedures and techniques used in highway construction inspection, survey, materials testing, and quality control equipment. iii. Skill in interpersonal relations as applied to contact with contractors, representatives of other governmental jurisdictions, and City Project Manager/Caltrans staff. 3) CONSTRUCTION TECHNICIAN III a) Exercises considerable independent judgment within general Caltrans standards and guidelines. Examples of duties assigned to this classification are: i. Inspect Project construction on an ongoing basis to assure compliance with contract and in accordance with State and local standards. 24 ii. Perform a variety of structural material tests and inspections. iii. Reviews construction plans and verified that these are in accordance with designated specifications and other requirements. iv. Participates in the preparation of completed work estimates, to calculate compensation due contractor. v. Examines and verifies numeric data and material Specifications on project cost source documents, utilizing geometry and trigonometry calculations. vi. Supervises all work activities involved in construction projects, laboratory, and quality control work. vii. Recommends approval of proposed Project changes. b) Knowledge and Skills Required 11. Construction Surveying Project Manager i. All knowledge and skills required of lower classifications. ii. Knowledge of the principles and practices of Civil Engineering as applied to the construction of State Highways. iii. Skill in analyzing and evaluating a wide variety of highly technical engineering data, including construction plans, field survey and quality control documents. iv. Skill in interpreting and implementing Caltrans standards, policies, procedures and regulations. v. Skill in interpersonal relations, as applied to contacts with contractors, representatives of other governmental jurisdictions, and other City of Modesto/Caltrans staff. a. A minimum four (4) years project management experience on similar construction projects is desired. b. Licensed Surveyor or pre-january 1, 1982 Registered Professional Engineer in the State of California. Page 24 of 39

25 c. Accessible to the Resident Engineer and City Project Manager at all times during normal working hours as specified in this Scope of Services. d. Under the direction of the Resident Engineer, the Project Manager will be responsible for: 12. Field Party Chief(s) 1) Review, monitor, train, and provide general direction for CONSULTANT survey personnel. 2) Assign personnel to projects on an as-needed basis. 3) Administer personal leave, subject to approval of the Resident Engineer. 4) Prepare monthly reports for delivery to the Resident Engineer. a. The person(s) holding the position of Party Chief shall meet at least one of the following licensing requirements: 1) A licensed Land Surveyor in the State of California. 2) A pre-january 1, 1982, Registered Professional Civil Engineer in the State of California. 3) An experienced surveyor who serves as chief under the direction or supervision of a person who is a licensed Land Surveyor or pre-january 1, 1982 Registered Professional Civil Engineer in the State of California. The direction or supervision shall place the supervisor in responsible charge of the work. Responsible Charge is defined in Chapter 15 of the Business and Professions Code (the Land Surveyor s Act) and Title 16, Chapter 5, of the California Administrative Code (regulations adopted by the Board of Registration for Professional Engineers and Land Surveyors). b. The Party Chief(s) should have a minimum two (2) years survey experience on similar construction projects and possess the following additional capabilities: 1) Thorough knowledge of construction survey practices and the ability to read and interpret plans and specifications. 2) Ability to make effective decisions concerning field problems and work in progress. 3) Familiarity with typical coordinate geometry computer Page 25 of 39

26 programs. 4) Familiarity with safety requirements for surveying near traffic. c. The Party Chief(s) will assume the following responsibilities: 13. Survey Crews 1) Perform construction staking services for Project construction. 2) Administer day to day activities for the survey party. 3) Perform analytical survey calculations for items such as grading, horizontal and vertical control, right of way, and minor in-field design. 4) Maintain continuous communication with the Resident Engineer, field personnel, and construction administration staff. a. Qualifications for survey crew members should include the following: 1) A minimum of one (1) year of survey experience on similar construction projects is desired. 2) Fundamental knowledge of construction survey practices and the ability to read and interpret plans and specifications. 3) Ability to assist Party Chiefs and office personnel in all required surveying work. 4) One survey crew member must have the ability to assume temporary Leadership of the survey party in the absence of the Party Chief. b. Under the direction of the Resident Engineer and the Party Chief, the survey crew members will assume the following responsibilities: 1) Perform basic calculations to support construction staking. 2) Maintain continuous communication with Party Chiefs and office personnel. VII. PROPOSAL REQUIREMENTS 1. Introduction Page 26 of 39

27 The proposal shall be submitted in the following format and contain no more than the maximum number of pages indicated. Introductory Letter 3 pages Sections 3-5 Text 30 pages Figures and Illustrations 10 pages Appendix No limit Note: The 43 pages for Text, Figures, and Illustrations may be presented in any format which suits the consultant. The intent to limit pages is necessary due to reviewer time constraints. The Appendix shall include resumes of key personnel, overall firm brochures, and other information the consultant wishes to submit. Items included in the Appendix will only be evaluated as part of the overall submittal. Figures and illustrations should include items such as organization charts, schedule, estimate of hours, etc. Submit five (5) copies of the proposal and one (1) copy of the cost information required in item VIII (Cost Information). 2. Introductory Letter The introductory letter should be addressed to: Bill Sandhu, P.E, PMP City Engineer Community & Economic Development Department Land Development Engineering th Street, Suite 3100, Modesto, CA Indicate the name of the firm submitting proposal, its mailing address, address, telephone and fax number, and the name of an individual to contact if further information is desired. This letter should also contain an expression of the consultant s interest in the work, a statement regarding the qualifications of the consultant to do the work, and any summary information on the project team or the consultant that may be useful or informative to the City. The consultant should indicate his/her acceptability of the terms and conditions of the Standard Agreement for Consultant Services contained in Appendix A. Any proposed deviations and modifications to the agreement should be noted, with reasons given, in the introductory letter for review by the City. The City will not consider changes to the agreement once selection has been made. The consultant shall indicate their local experience and address of local offices, if any. Local participation is encouraged and observed as part of this RFP, but the ultimate decision to award is the City s discretion. Page 27 of 39
