GABRIEL OF THE SORROWFUL MOTHER CHURCH 06/15 NOTICE TO CONTRACTOR: ADDENDUM NO. 1 Addition to St. Gabriel of the Sorrowful Mother Church The information contained herein revises, supplements and/or supersedes the specific part of the Contract Documents referred to, and shall be attached to and become part of such Documents as if originally forming a part thereof. Except as herein modified, all other provisions of the Contract Documents shall remain in full force as originally set forth. Additional work called for herein, unless otherwise described in this Addendum shall comply with the requirements originally specified for similar work. This addendum consists of eight (8) pages. ITEM NO. 1: Pre-Bid Meeting A. Agenda of the PreBid Meeting conducted on 6/10/15 is attached and is part of the Construction Documents. See Addendum Item No. 3 for correction of sales tax issue. B. Confirmation of Discussion: The Archdiocese insists on submission of 3 hard copies. Electronic bids will not be accepted. ITEM NO. 2: Section Form of Proposal A. Clarification of Base Bid in relation to Deduct Alternate #2: Base bid includes supply and installation of insulated glass in exterior aluminum doors. B. The unit price for rock removal will be changed from L.F. to C.Y. when the bid form is reissued one week prior to the bid due date. C. Refer to Addendum Item No. 9. ITEM NO. 3: Section Notice to Bidders A. Remove Bullet Point 10 and replace with the following: 10. The church is a tax exempt organization. Contractors are responsible for any and all sales tax in accordance with state and local laws. ITEM NO. 4: Section General Conditions of the Contract for Construction A. Clarification of Section 3.7.1: GC will be reimbursed for the cost of the building permits. The cost of the permits should not be included in the bid. ITEM NO. 5: Site Drawings Sheet 2 A. Drawings show a new 15 storm pipe wrapping around the addition. Abandon (cut and cap) the portion of the existing storm pipe that runs under the addition. Connect downspouts (not shown) from the existing building to the new 15 storm pipe. ADDENDUM NO. 1 PAGE 2

Response: Although the general slopes on the grading plan would indicate that this detail would be not be needed, the bidders should review the Sediment and Erosion Control Narrative to determine if any of their means and methods would trigger the requirement for this detail. ITEM NO. 7: Bidder Question - Carpet A. Bidder Question: Please provide a carpet spec. Response: There is no carpet in project. Refer to 5/A-1 for flooring transition to existing building. ITEM NO. 8: Structural Drawings Correction/Clarification A. All roof sheathing to be 19/32 thick. B. All floor sheathing to be 3/4 thick. ITEM NO. 9: Structural Drawing S-1, Earthwork Note 7. A. The owner will provide the Geotechnical Engineer to confirm bottom of footing bearing capacity. Engineer will determine if unsuitable soil is present and must be removed. A cubic yard unit price for removal and replacement will be added to the bid form that will reissued one week prior to the bid due date. ITEM NO. 10: Structural Drawing S-2 A. Refer to attached revised Drawing P-1.2 for location of sanitary pipe that crosses the foundation at two exterior wall locations. Drop footing at these locations per Detail TD2/S-4. ITEM NO. 11: Plumbing Drawings P-1.1, P-1.2, P-2.1 A. Refer to attached revised drawings for replacement of existing sanitary pipe. Drawings have been updated on the file sharing website. ITEM NO. 12: Plumbing Drawings P-1.1 and P-1.2 A. Clarification: Work indicated in Keynotes B and 1 (excavation, pipe removal and replacement, and backfill of gas service from ROW to meter) is to be completed by the gas company and paid for by the owner. B. Clarification: Work indicated in Keynotes C and 2 (excavation, pipe removal and replacement, and backfill of gas distribution piping from meter to onsite appliances) is to be completed by the GC and is part of the GC contract. Project manual, drawings, and bidders list posted to Project File Sharing website. 2. Addenda will be posted to the website as needed. C. Project Scope of Work and Work by Others (Section ). 1. This is a Single Prime Contract (GC). Contract includes construction of an addition to the existing church, alterations to the existing building, and related sitework. 2. Work by others (outside of this bid) includes moveable furnishings, stained glass, and statues/artwork. The oak backdrop for the two statues will be supplied and installed by others. Insulated glass to be supplied and installed by GC. 3. The owner will provide the thin cut stone veneer to the GC at no cost. Installation of the stone is included in the GC scope of work. GC to supply and install all accessories (scratch coat, lath, mortar bed, etc.) D. Owner Occupancy 1. Owner will occupy the building for the duration of construction. E. Temporary Facilities 1. GC to provide sanitary facilities. 2. Owner will allow use of power and water at no charge. F. Alternate Bids (Section ) 1. Performance bond and labor and materials payment bond. 2. Site inspection: Call the church office at in advance of any visit to the site by subcontractors to confirm no there are no conflicts with funerals or other reasons that a site visit would be impractical. 2. Archdiocesan Form of Agreement, General Conditions, Insurance Requirements, and supplementary information are included in the Project Manual. Bidders must carefully review all requirements as they will all be part of the contract. 3. Project Schedule: Length of contract as determined by bidder. Contract includes Liquidated Damages of $100/day (Section ). 4. Owner is exempt from sales tax. Sales tax shall not be included in bids. 5. Insurance requirements are listed and appended in several sections in the Project Manual. 6. No Bid Bond required. Hold bids for 60 days (Section ). 7. GC must designate one person to be full time on-site superintendent for the duration of the project (General Conditions 3.9.1). 8. Special Inspections are by the Owner (Dwg S-1). Costs of reinspection and retesting for items that fail initially will be back charged to the GC via change order. 9. Utility company fees for establishing permanent services will be paid for by the owner. 10. Project requires a general two year warrantee (General Conditions 3.5.2) from the GC with several individual warrantees with longer terms. 11. Three hard copies of bids are to be delivered to the Archdiocese by Monday, June 29, 2015, 3:00PM. Bid Opening will be private.
