The report shall be read in its entirety. The reader must review the detailed information provided in the accompanying text. Any interpretation, use and conclusion resulting from the data contained in this report is the responsibility of the reader. North Tower Environmental performed a survey for asbestos-containing materials and lead paint under contract with the City and County of San Francisco Department of Public Works. The survey was conducted on April 12, 2010, by Mr. Eric Zamb, a Cal/OSHA and DHS certified professional. Diagrams indicating sample and room locations are presented in Figures 1 and 2. Table I and Appendix A contain laboratory results for suspect asbestos containing material samples collected during the survey. The results for suspect lead bulk samples and the XRF inspection are located in Appendix B and Table II. ASBESTOS The following asbestos-containing materials were identified during North Tower Environmental s inspection: Material Location Percent Asbestos Sheetrock/Joint Compound Pier 28: Walls and Ceilings Throughout 1 to 5% in Compound Second Floor Office Area. None Detect in Sheetrock
3 Material Location Percent Asbestos Pipe Run and Elbow Insulation Pier 28: Above Second Floor Ceiling 3 to 15% (including debris) (see Photo #1) LEAD The following lead-containing materials were identified during North Tower Environmental s inspection: Lead-Based Paint (1.0 mg/cm 2 lead or greater): The lead inspection revealed that the following tested surfaces at the site contain lead in the amount greater than 1.0 mg/cm 2. Pier 26: Off-White paint on original wood walls and ceilings. In many areas these walls/ceilings are covered with sheetrock. Pier 28: Tan paint on concrete ceiling in attic, and orange primmer on structural steel in attic (see Photo #2). Paint Containing No Lead: Paint containing no lead was identified in the following areas. Pier 26: White paint on exposed HVAC ductwork throughout. INTRODUCTION North Tower Environmental was requested by the City and County of San Francisco to develop a Limited Asbestos and Lead Survey Report for Piers 26 and 28 (Bulkheads) located in the city of San Francisco, California. The purpose of the Hazardous Materials Survey was to identify asbestos-containing materials (ACM) and lead paint at the site that may be impacted during upcoming HVAC renovation. This survey was conducted by North Tower Environmental in general accordance with our proposal for environmental consulting services, dated December 9, The survey and report were completed by Eric Zamb, a Cal/OSHA Certified Asbestos Consultant (# ) and Department of Health Services Accredited Lead Inspector/Assessor (#6683). The building survey and collection of bulk material samples were conducted on April 12, B. BACKGROUND 1. Site Description: The sites are located at Piers 26 and 28, in the city of San Francisco, California. Areas inspected at both piers consisted of office space located in the southwest corner of each pier. This survey consisted of inspecting areas that will be impacted during upcoming HVAC system replacement activities. Maps depicting the locations of all sample and room locations is contained on Figures 1 (Pier 26) and 2 (Pier 28). 2. Scope of Services: The scope of services used to meet the objective included: a. Visual observation of accessible building system components and materials to evaluate their potential to contain asbestos and lead; b. Collection of bulk samples of accessible construction materials and building systems that were judged to potentially contain asbestos; c. Inspected painted surfaces within the piers using bulk sampling for lead; d. This survey did not include the use of destructive surveying methods, such as breaking into wall voids, or penetrating inaccessible ceiling or floor cavities to locate suspect materials; e. Laboratory analysis of the collected asbestos bulk samples by a National Voluntary Laboratory Accreditation Program (NVLAP)-approved laboratory using polarized light microscopy (PLM) in general accordance with U. S. Environmental Protection Agency (EPA) Method No. 600/R ; Hazardous Materials Survey Report NTE Project No.: NT-3278

Laboratory analysis of the collected lead paint chip and bulk samples by a National Lead Laboratory Accreditation Program (NLLAP), Environmental Laboratory Accreditation Program (ELAP), and Environmental Lead Proficiency Analytical Testing Program (ELPAT)-approved laboratory. Samples from painted surfaces were analyzed within a flame furnace by Atomic Absorption (AA); and g. Preparation of this summary report presenting the data, findings and conclusions of the survey. C. WORK DESCRIPTION: SURVEY AND FINDINGS 1. Site Inspection/Assessment: Prior to sample collection, North Tower Environmental conducted a detailed visual assessment of the suspect asbestos-containing materials and lead-based paint at the site. a. North Tower Environmental noted that the suspect asbestos-containing materials were generally in good condition, with no damage noted during the inspection except for the following: - Pier 28: Damaged pipe insulation and pipe insulation debris were noted on the attic floor above the Second Level Office Area. b. North Tower conducted a visual assessment of painted surfaces throughout the structure, and noted the following: - Pier 28: Minor amounts of loose and peeling lead-based paint was identified on structural steel throughout the Second Level attic. It is not anticipated these areas will be disturbed during HVAC renovation activities. 2. Asbestos a. Bulk Sample Collection: Bulk samples were collected from accessible homogeneous areas of suspected ACM for subsequent laboratory analysis to determine actual asbestos content. Sampling was generally conducted in a manner that minimized damage to the building, did not leave unsightly marks, and did not create a health hazard for building occupants or inspectors. b. Sample Analysis: Bulk samples were analyzed using polarized light microscopy (PLM) in accordance with the EPA Interim Method for the determination of asbestos in bulk Insulation Samples, EPA-600/R Samples were analyzed by Analytical Labs of San Francisco, California. Analytical Labs is accredited under the National Institute of Standards and Technology-National Voluntary Laboratory Accreditation Program (NVLAP) for bulk asbestos sample analysis. 7 Pier 26 and 28, Bulkhead Survey Page Number: 3 c. Bulk Sample Analytical Results: Analytical results of the bulk samples are summarized in Table I. Laboratory analytical reports are included in Appendix A of this report. Seventeen (17) bulk samples of suspect asbestos-containing material were collected from various structural, mechanical and architectural materials observed in accessible areas throughout the site. Of the 17 samples collected, three contained detectable concentrations of asbestos ranging from 1 to 15 percent. Based upon analytical results and visual observations, the materials listed below were determined to be ACM (see Figures 1 and 2 for Sample and Area Locations). Joint Compound: The joint compound contains 1 to 5 percent asbestos and was identified in the following locations: - Pier 28: Walls and Ceilings throughout second floor office area. Pipe Insulation and Debris: The pipe insulation in the following area contains between 3 and 15 percent asbestos: - Pier 28: Above Second Floor Office Area in Attic. 3. Lead-Based Paint a. Sample Collection: A lead-based paint (LBP) survey was conducted to identify the presence of lead containing paint and LBP on building components within the facility. The purpose of the survey was to gain baseline information on the presence of lead and LBP throughout the structure. Such data will be of use in minimizing potential exposures to employees and to contract employees who may encounter lead while engaged in operations which may disturb lead in dust or paint. Mr. Eric J. Zamb (DHS #I-6683) a DHS Certified Lead Inspector/Assessors, performed the testing on April 12, The lead-based paint testing was performed using paint chip sampling. The U.S. Department of Housing and Urban Development (HUD), the agency responsible for assessing publicly owned housing for LBP hazards, has developed and defined the procedures used for measuring LBP in the residential setting. The Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing (HUD Guidelines), dated June 1995, represent the state-of-the-art procedures in LBP inspections, and are the most comprehensive methods available for assessing LBP in residential properties. Although the HUD Guidelines do not strictly apply to non- HUD facilities, they do provide an industry standard for the testing and assessment of lead in soil, dust, and paint. 8 Pier 26 and 28, Bulkhead Survey Page Number: 4 b. Paint Sample Analytical Results: Bulk sampling data for lead is located in Appendix B. A total of 7 paint chip samples were collected from surface coatings on selected building components. Lead analysis resulted in 3 of the 7 samples exceeding the U.S. Department of Housing and Urban Development (HUD) definition of lead-based paint (greater than 1.0 mg/cm² of lead). Below is a summary of the analytical results: Lead-based paint was identified in the following areas: Pier 26: Off-White paint on original wood walls and ceilings. In many areas these walls/ceilings are covered with sheetrock. Pier 28: Tan paint on concrete ceiling in attic, and orange primmer on structural steel in attic (see Photo #2). Paint containing no lead was identified in the following areas: Pier 26: White paint on exposed HVAC ductwork throughout. Pier 28: Off-White and Tan paint on sheetrock walls and ceilings throughout. c. Discussion of Results: When considering paint chip sample results, which have tested positive for lead, they may be placed in one of two categories. The sample may be Lead-Based Paint/Ceramic Tile, which is any paint/ceramic tile containing greater than or equal to 0.5 percent lead by weight. The sample may also have a lower lead content and be considered Lead-Containing Paint/Ceramic Tile, which is any paint/ceramic tile indicating detectable concentrations of lead but less than lead-based paint/ceramic tile. Lead-Containing Paint (less than 0.5 percent lead by weight): Compliance with Cal-OSHA s Lead in Construction Standard (8 CCR ) is required when an activity may disturb paint/ceramic tile with any detectable concentration of lead. The presence of lead-containing paint/ceramic tile requires that special precautions be taken during any construction or demolition work to ensure that individuals involved in the work are adequately protected against excessive exposure to lead-containing dust. Cal-OSHA regulations require employers to conduct personal air monitoring on the employees during all activities, until statistically reliable results indicate what exposure results are, and whether the Action Level (30mg/m 3 ) or Permissible Exposure Level (PEL) (50mg/m 3 ) will be exceeded. Worker training, physical exams and blood testing, engineering controls to reduce lead exposure, and hygiene practices are also required by OSHA. 9 Pier 26 and 28, Bulkhead Survey Page Number: 5 Activities strictly prohibited by OSHA include sanding, heating, grinding or other disturbance which may result in airborne lead dust levels exceeding the Cal/OSHA permissible exposure limit (PEL). Lead-Based Paint/Ceramic Tile (greater than or equal to 0.5 percent lead by weight): The use of DHS certified workers, compliance with DHS and OSHA regulations are required when disturbing surfaces considered Lead-Based Paint/Ceramic Tile. D. LIMITATIONS The reported results of the presence of ACMs and LBP presented in this report are intended for discussion and informational purposes only. These results should not be solely used in the preparation or design of specific asbestos and lead abatement response options without the supplement of additional field-specific and material-specific information. The judgments, conclusions, and recommendations described in this report pertain to the conditions judged to be present or applicable at the time the work was performed. Future conditions may differ from those described herein and this report is not intended for use in future evaluations of the facility unless an update is conducted by a Certified Asbestos Consultant and DHS certified employee familiar with currently used asbestos and lead survey practices and this subject facility. North Tower Environmental performed its services using that degree of care and skill ordinarily exercised under similar conditions by reputable members of our profession practicing in the same or similar locality. No other warranty, expressed or implied, is made or intended by our performance of consulting services or by furnishing our written report. This report has been prepared on behalf of and exclusively for the use of the City of San Francisco. RECOMMENDATIONS Asbestos Containing Material: Prior to HVAC renovation, removal of asbestos-containing construction materials (materials containing greater than 0.1% asbestos) should be conducted by a licensed, certified, and registered asbestos abatement contractor. Work involving the disturbance of materials containing asbestos should be performed in accordance with applicable federal, state and local laws and regulations. Such work should also be conducted in conformance with an Asbestos Abatement Specification developed by a Certified Asbestos Consultant. Lead-Based Paint: For all work to be performed on surfaces coated with Lead Paint, the contractor must comply with Cal/OSHA Construction Safety Orders, Lead, Section , Title 8, CCR and DHS Title 17. The work (presumably renovation, modernization or demolition work) shall be performed in compliance with applicable regulations in order to protect employee, the environment and the surrounding community from the potential hazards associated with lead.
