A.3: The construction allowance is included on the bid form, on page 2. Q.4: Section Item 1.2 C B Attached makes reference to testing. Please advise the extent of this testing. A.4: The extent of the testing will be to ensure the curtainwall system is installed as per manufacturers quality and standards requirements. Q.5: This spec indicates A report of the presence of asbestos is on file for review and use. Can you provide this report? If this is the AHERA 3 year re-inspection it will not satisfy the regulations requiring inspections for asbestos prior to renovation. The specification also assumes that all asbestos removal work will be conducted as exterior work using manual methods that do not render the asbestos materials friable or create any visible emissions during work. 7 PART 1 - GENERAL CONDITIONS 1.1 RELATED DOCUMENTS SECTION A. Review the asbestos drawings developed by Environmental Assessments & Solutions, Inc. for further information. If a conflict between the drawings and specifications exists, the more stringent of the two shall apply. B. Drawings and general provisions of the contract, including General and Supplementary Conditions and other Division Specification sections developed by the project's Architect - Fuller D'Angelo P.C. apply to the work of this section. C. The Contractor shall be responsible for reviewing all drawings and specifications. All asbestos removal required shall be included in the base bid. Contractor shall coordinate the removal of asbestos containing materials with other trades as part of this specification and base bid work, no extra charge shall apply. D. The Contractor shall be responsible for reviewing the Asbestos Pre-Renovation Inspection report which is included in the Asbestos Removal Specification. This Asbestos Pre-Renovation Inspection report must be provided to all contractors and their sub-contractors. All contractor and sub-contractor employees must be made aware of the presence and type of asbestos containing materials identified during this inspection to ensure that such materials are not accidently disturbed. 8 1.2 PRE-ABATEMENT MEETING SECTION ASBESTOS ABATEJv1ENT A. The Architect, Construction Manager, Asbestos Consultant, General Contractor, Asbestos Contractor, School District (Owner) Representative upon receipt, review and substantial approval of all pre-abatement submittals and upon verification that all material and equipment required for the project are available to the contractor, will arrange for pre-abatement meeting between the abatement contractor, construction manager, abatement design consultant and the Owner representatives. The purpose of the meeting is to discuss any aspects of the submittals needing clarification and to discuss any aspects of the project execution and the sequence of operations. The abatement contractor shall be prepared to provide any supplemental evidence and information to the Owner's Representative pertaining to any aspects of the submittals or the materials or equipment. This meeting will be held no later than 5 days prior to scheduled commencement. Upon satisfactory resolution of any outstanding items, the Owner's representative will issue a written order to proceed to the asbestos contractor. No asbestos work shall be initiated prior to the written order to proceed. The Contractor's failure to provide the submittals in accordance with the above shall not relieve the Contractor of his responsibility to complete the work in the time frame specified. 9 1.3 PRE-ABATEMENT SUBMITTALS SECTION A. Detailed site specific work plan for the entire project reflecting contract documents and the phasing and schedule requirements outlined. B. Provide evidence ofa current State of Connecticut Asbestos Contractor License and Insurance Certificate acceptable to Town of Greenwich. C..Name, location and phone number of the Asbestos Waste Hauler and Landfill. D. Provide written notification to local health, fire and / or safety authorities indicating the asbestos work to be performed and any other notifications required by the specifications. E. Notification of asbestos abatement to the Department of Public Health on the prescribed form 10 days before work commencement F. Notification of asbestos disposal to Department of Environmental Protection, if asbestos waste is to be disposed of in Connecticut. G. A site specific plan for emergencies that may require modification or abridgement of decontamination or work area isolation procedures. H. Asbestos training and certification documentation for all on-site personnel, including original and refresher and current CT State and EPA AHERA certification. I. 10 1.4 PRIOR TO COMMENCEMENT OF WORK WESTERN :MIDDLE SCHOOL CURT AINW ALL REPLACEMENT PROJECT A. Prior to beginning work, the Owner's Representative, Construction Manager, Architect, Asbestos Consultant, General Contractor and Asbestos Contractor shall make a visual inspection of each work area and list all pre-existing damage to building components. Contractor will submit a list which shall include all damaged areas not included to be repaired under this contract and include a narrated video tape. A copy of this video tape is to be furnished to the Owner's Representative. The Contractor and their subcontractors will be held responsible for any other damaged incurred within the work area during the project. The Contractor shall inform the Owner's Representative in advance as to when the inspection will take place. The videotape must be submitted to the Owner's Representative before any work activity will be permitted. B. Post signs in an around the work area to comply with OSHA. Also post one copy of OSHA Asbestos, EPA NESHAPS, CT State DPH, CT State DEP, EPA AHERA regulations. C. Post the following in the clean room of the Worker Decontamination Enclosure and Contractor staging area: 1. Copy of the Contractor's License 2. Contact list including name, address and phone numbers, emergency service providers and after hours contact information for Contractor representative and site Supervisor. 3. 11 1.5 SUBMITTALS DURlNG ABATEMENT A. The contractor shall maintain a daily log at the job site documenting the dates and times of meetings as well as the attendees and purpose for the meetings, all persons entering / exiting:asbestos work areas, daily work activity, special or unusual events such as barrier breaching, equipment failures, emergencies and any cause for stop of work, air monitoring tests, submit completed daily log which must include number of bags removed from containment each day (when applicable) or the number of waste containers leaving the property with asbestos waste and persomlel air monitoring results to Owner's Representative. Submit the logs daily and the OSHA air sample results when posted at the job site to the to the Asbestos Consultant. 1.6 SUBMITTALS AT COMPLETION OF ABATEMENT A. Submit copies of all transport manifests, trip tickets and disposal receipts for all asbestos waste materials removed from the work area during the abatement process. The Contractor shall submit to the Owner's Representative any and all documentation requested. At which time, the Owner's Representative shall assemble a project report consisting of the daily field notes and the documentation of events during abatement including the waste shipment record signed by the operator of the licensed landfill. The project report shall include a certificate of completion, dated and signed by an authorized representative of the Contractor and shall be submitted within 30 days of project completion. 15 I. PRE-RENOVATION INSPECTION REPORT 1. Executive Summary: CURTAJNW ALL REPLACEMENT PROJECT Charles Schwartz of Environmental Assessments & Solutions, Inc. conducted a pre-renovation asbestos inspection at the Western Middle School on 12/15/12, 01105/13 and 01107/13. The purpose of the inspection was to determine whether building materials, contractors might come in contact with, disturb or remove during the course of the curtainwall replacement project, are asbestos containing materials (ACMs). This report provides information concerning asbestos containing materials identified at this school and the procedures used to identify these materials. The inspection also involved the collection of curtainwall caulk materials for PCB analysis. The scope of the pre-renovation inspection included the curtainwall system and materials at the curtainwall including panels, windows, frames, doors and miscellaneous interior materials such as pipe elbow insulation, ceiling tiles and music room transite wall panels at the curtainwall from the following school building areas: 1. The Administration Wing 2. The Classroom M & U Wing 3. The Gymnasium & Auditorium Wing 4. The Bridge Our scope of work included a visual inspection, review of available asbestos inspection reports and other documents as well as bulk sampling for asbestos and PCBs and laboratory analysis of collected samples. This asbestos report contains the results of all samples which were analyzed. This inspection involved the collection and laboratory analysis of samples to supplement the school's Asbestos Inspection and Management Plan. Other hazardous materials including but not limited to lead in paint were not included in tills inspection. Furthermore, the investigation was limited to building materials that might be directly damaged during this renovation project only. Section EAS,Inc.

Executive Summary (continued): Asbestos - Containing Material (ACM) means material composed of asbestos of any type and in an amount greater than one (1) percent (%) by weight, either alone or mixed with other fibrous or non-fibrous material. State and Federal Regulations require asbestos containing materials (ACMs) be removed in compliance with specific control measures prior to demolition, renovation or other activities which may disturb them. The following materials were sampled and determined to be asbestos - containing materials (ACMs): A. Administration Wing: 1. Curtainwall System Asbestos - Containing Materials: a. Caulk (beige and soft) associated with metal frames at mullions under metal covers (visible from inside the building after the mullion covers are removed) b. Tar sealant (black, soft and sticky) associated between panels and metal frames (visible from inside the building) 2. Non-Curtainwall System Asbestos - Containing Materials: 1. Caulk materials on the curtain wall system are presumed asbestos - containing materials (ACM) unless tested and proven negative by laboratory analysis. 2. 19 1. Executive Summary (continued): SECTION WESTERN IvIIDDLE SCHOOL CURT AlNW ALL REPLACEMENT PROJECT The following materials were determined to be non asbestos - containing materials (non ACMs): A. Ceiling tiles which were tested during this inspection. B. Floortiles (all replacement including mastic following historical asbestos abatement). C. Other visible exposed caulk materials not listed above which were tested during this inspection (refer to the Asbestos Inspection Report Bulk Sample Results Tables contained herein). However, if previously inaccessible suspect asbestos materials such as caulks or adhesives become exposed during renovation, they must be assumed asbestos or be tested by the Asbestos Consultant to rule out asbestos. The following asbestos - containing materials (ACMs) will be abated by Greenwich School District prior to the commencement of the curtainwall replacement work as the project may cause disturbance to these asbestos - containing materials: A. Pipe elbow cementitious packing insulation between the radiators and curtainwall in the Administration Wing B. Pipe elbow cementitious packing insulation above the hung ceiling near the curtainwall in the Classroom M & U Wing C. Pipe elbow cementitious packing insulation located near the curtainwall in the Gymnasium / Auditorium Wing D. Transite acoustical wall panels located on or adjoining the curtainwall in the Gymnasium / Auditorium Wing in the two (2) music classrooms and office. Section EAS, Inc.

Asbestos Inspection Procedures: Work conducted by EAS, Inc. was performed in accordance with industry standard of care and in compliance with applicable regulations and guidelines including Environmental Protection Agency Asbestos Hazard Emergency Response Act (EPA AHERA) and Connecticut State Department of Public Health (CT DPH). The asbestos survey procedure consists of a visual inspection of building materials to locate and identify suspect ACBM which may be impacted by the renovation project. Bulk samples are collected at EAS' s discretion and submitted for analysis. Sampling was core to the substrate. Each sample area was cleaned following the sampling. Photographs were taken of representative sample locations and building material conditions. The investigation, was however limited and therefore performed in a representative manner. No representation is made that the areas inspected do not contain concealed asbestos containing materials. In general 2-3 samples of each homogenous material were collected unless materials were patches or in very small quantities. Samples were analyzed by EMSL Analytical, Inc.. - NY, NY. All samples were submitted directly to an independent accredited laboratory with the sample chain of custody forms. All samples were analyzed by PLM via the EPA 600 / R -93/116 method. Some samples, if inconclusive by PLM, may also have been analyzed by the TEM Gravimetric Reduction method. Representative caulk samples were also subjected to PCB analysis. Samples were submitted in glass jars on blue ice. Samples were prepped by EPA method 3550B and analyzed in accordance with Reference Method EPA SW by York Analytical Laboratories, Inc. - Stratford, CT Section EAS, Inc.

21 3. Asbestos Removal Considerations: A. An asbestos removal project is required to address caulk and sealant materials associated with the curtainwall system in the Administration Wing, Classroom M & U Wing and Bridge as well as caulk and glaze materials associated with the Gymnasium & Auditorium Wing This work must be conducted by a licensed asbestos abatement contractor. All panels, windows and frames containing or contaminated by asbestos - containing materials (ACMs) are to be disposed of as asbestos - containing material (ACM) waste. Additionally, any caulk remaining on brick after the curtainwall is removed at the Gymnasium & Auditorium Wing must be cleaned off as an asbestos containing material. B. Prior to commencement of the curtainwall replacement project, Greenwich School District shall conduct asbestos abatement of pipe elbow cementitious packing insulation and transite acoustical wall panels, as needed, to prevent disturbance to these asbestos - containing materials. C. Our pre-renovation inspection was limited to the curtainwall replacement project and associated scope. Suspect building materials which may become exposed during the replacement project that were not tested must be assmned to be asbestos - containing materials (ACMs) and handled accordingly or tested to prove that they do not contain asbestos. D. Any field conditions which differ from this Inspection Report or Asbestos Material Removal Specification should immediately be brought to the attention of the Asbestos Consultant, LEA Designated Person and I or Construction Manager. E. Close coordination will be required between the asbestos removal contractor and curtainwall replacement contractor as this project involves asbestos material removal. F. CT State DPH must be notified on the proscribed form by the Asbestos Contractor prior to commencement of asbestos activities. Section EAS,Inc.

22 3. Asbestos Removal Considerations (continued): G. F or the purposes of the Inspection Report and Removal Specification, it is assumed that there will be NO children present in the building for the duration of asbestos activities. Ifthis changes, approval must be granted by CT State DPH prior to asbestos work commencement and additional work procedures will apply. H. All asbestos removal and related asbestos work must be conducted by a licensed asbestos contractor following proper project control and cleanup procedures. I. A licensed Asbestos Project Monitor / Inspector will be retained by the Greenwich School District to conduct visual inspection, ambient air sampling, clearance inspections and documentation activities as part of this asbestos project. J. Prior to this project's commencement, the contractor will be provided with a copy of this Asbestos Pre-Renovation Inspection Report and the Asbestos Removal Specification. The contractor must also attend a walkthrough meeting where the project is discussed in detail. The school district's LEA Designated Person and the school district's Asbestos Consultant will conduct inspections to ensure compliance with the asbestos specification and applicable regulations. K. Under no circumstance should asbestos be disturbed without proper notification, isolation, containment and clean-up procedures. If unplanned disturbance does occur, asbestos debris must be properly cleaned by an asbestos contractor following appropriate notifications and using appropriate controls as specified in the Asbestos Removal Specification. L. Manual removal methods will be required to keep asbestos material nonfriable during removal and prevent visible emissions. Section EAS,Inc.

All asbestos material waste and materials contaminated by asbestos are to be disposed of as asbestos waste with an Asbestos Waste Manifest for each shipment to prove proper hauling and disposal. N. Lead paint testing was not conducted. The contractor is responsible for proper worker protection, clean-up and waste disposal. O. PCBs were not detected in bulk sampling conducted at this School. 24 4. Qualifications: Charles Schwartz is licensed as an Asbestos Consultant - Inspector / Management Planner ( expo 3/ ), Project Designer ( expo ) and Project Monitor ( expo 3/ ) by CT State DPH. Charles Schwartz attended his most recent Asbestos Refresher training for Inspector / Management Planner on 1/13/2011 (certificate # ), Project Monitor on 2116/2011 (certificate # ) and Project Designer on 1117/2011 (certificate # ) at QuES&T in Wappingers Falls, NY. EMSL Analytical is accredited as An Approved Environmental Laboratory - Asbestos Bulk by CT State DPH (PH-0170 expo 9/30/13), US NIST NVLAP ( expo 6/30/13) and AIHA llilap (102581). 33 2.2 SCOPE OF WORK A. The work specified herein includes all labor, materials and equipment needed to complete all demolition, removal, cleaning and disposal of all non-asbestos materials and debris as required for gaining access to asbestos containing materials indicated on drawings, addendum's and / or herein called for in this Asbestos Removal Specification. The demolition of non-asbestos materials shall be treated, handled, removed and disposed of as asbestos regulated materials whenever there is evidence or concern that such materials may be contaminated by asbestos or the removal of such materials would cause vibration or disturbance of asbestos - containing materials. B. The Asbestos Consultant shall determine whether work causes asbestos, assumed asbestos or asbestos debris to be disturbed. If so, work shall be conducted as asbestos work not demolition. C. The Contractor shall be responsible for the following general requirements: 1. Obtain and post all permits as required. 2. Provide, erect, and maintain all planking, scaffolding, shoring, barricades, and warning signs as requested to complete the scope of work for this project, including hard wall tunnels to allow building occupants to safely exit the building 3. Unless otherwise specified, all equipment, fixtures, piping and debris resulting from demolition and not scheduled to be saved and reinstalled shall become the property of the Contractor and removed from the premises and properly disposed of. 4. Material or equipment to be reset shall be removed with the utmost care to prevent damage of any kind. All material to be reused shall be stored in the existing building, but not in the asbestos work area. Any such items deemed contaminated with asbestos shall be cleaned by the Contractor if not porous until acceptable to the Owner's Representative. Section EAS,Inc.

34 2.2 SCOPE OF WORK (continued) SECTION D. It shall be the responsibility ofthe Contractor to protect and preserve in operating condition all utilities traversing the building and site. Damage to any utility due to work under this contract shall be repaired to the satisfaction of the Owner at no cost to the Owner. E. It shall be the responsibility of the Contractor to protect landscaping plants, trees and flowers from damage caused by contractor equipment, materials and work methods. Damage to landscaping due to work under this contract shall be repaired or replaced to the satisfaction of the Owner at no cost to the Owner unless the owner calls for the removal oflandscaping plants, trees and flowers. F. The Owner will retain an Asbestos Consultant - Project Monitor for the School District's own interests. The Project Monitor shall be on site during asbestos work as directed by the Owner. The Consultant shall not conduct any testing for the Contractor. G. Deviations from this Specification or accompanying Drawings require the Owner's written approval. H. The Contractor shall supply all labor, materials, equipment services, insurance (including asbestos insurance) and incidentals which are necessary to perform the work in accordance with applicable governmental regulations and these specifications. I. The Contractor shall provide and install fire extinguishers at each decontamination enclosure and in each work area. J. During the progress of work, various areas of the building may be occupied by others. The Abatement Contractor will be responsible for verifying interior conditions prior to the commencement of any work and documenting any damage or previous condition that may affect new work. Any additional damage, which occurs during construction, will be the responsibility of the Contractor to be corrected at no charge to the Owner. Section EAS, Inc.

35 2.2 SCOPE OF WORK (continued) SECTION K. The Contractor is responsible for posting proper notifications, barriers and signs that may be required to prevent access of unauthorized persons to areas undergoing asbestos removal activities. L. The Contractor shall be responsible for all isolation barriers to separate the exterior work area from the interior of the building for the duration of asbestos work. The isolation barriers may be constructed out of plastic sheeting sealed in place from the underside of the deck above the ceiling tiles to the floor surface including walls in a manner that is air-tight and prevents dust intrusion. To allow access onto the work area side of the isolation barrier from the inside of the building, zippers or other equivalent methods may be used. Zippers may be reused from room to room. M. There is no Alternative Work Practice (A WP) or Occupancy Variance required for this asbestos abatement project in order for compliance with regulations. Convenience A WPs intended to save the Contractor money but which are not needed to comply with regulations, are the Contractor's responsibility. Convenience A WPs must still be prepared by a CT State DPH licensed Asbestos Project Designer and be reviewed and approved by the Asbestos Consultant before their approval for use at this asbestos abatement project. N. If it is determined that an Occupancy Variance is required for this project because of child occupancy in the school building during the asbestos project, it is the Contractor's responsibility to comply with ALL conditions of the Approved Occupancy Variance as part of their base bid price. All asbestos work must be conducted as exterior work with air-tight and dust isolation -tight barriers in place separating the work area from the interior of the school building. The isolation barrier must be sealed from the underside of the roof deck to the floor and to walls. Ceiling tiles must be removed to gain access to the underside of the roof. The ceiling tiles may be removed as non-asbestos. P. All asbestos work must be conducted by hand tools as apposed to power tools to ensure they remain non-friable. The use of power tools to cut or grind asbestos will render the asbestos material friable and increase the likeli1lood of visible emissions. Visible emissions will require the use of full negative pressure containments with HEP A equipped negative air units to contain asbestos debris and dust to prevent visible emissions with manometers to verify the negative pressure reading. 39 2.3 DESCRIPTION OF WORK (continued) B. The following materials were determined to be non asbestos - containing materials (non ACMs) and therefore need not be removed or disposed of as asbestos: 1. Ceiling tiles which were tested during this inspection. 2. Floor tiles (all replacement including mastic following historical asbestos abatement) 3. Other visible exposed caulk materials not listed above which were tested during this inspection (refer to the Asbestos Inspection Report Bulk Sample Results Tables contained herein). However, if previously inaccessible suspect asbestos materials such as caulks or adhesives become exposed during renovation, they must be assumed asbestos or be tested by the Asbestos Consultant to rule out asbestos. C. The following asbestos - containing materials (ACMs) will be abated (removed) by Greenwich School District prior to the commencement of the curtainwall replacement work as the project may cause disturbance to these asbestos - containing materials: 1. Pipe elbow cementitious packing insulation between the radiators and curtainwall in the Administration Wing 2. Pipe elbow cementitious packing insulation above the hung ceiling near the curtainwall in the Classroom M & U Wing 3. Pipe elbow cementitious packing insulation located near the curtainwall in the Gymnasium / Auditorium Wing 4. The Architect's curtainwall replacement specifications and drawings should be carefully consulted for a specific listing of existing work areas for curtainwall removal. If there is conflict between the Asbestos Consultant and Architects drawings or specifications regarding component removal - the Architects drawings and specifications take precedence. E. All curtainwall components including windows, frames and panels containing asbestos or contaminated by asbestos shall be disposed of as asbestos regulated waste. No open waste containers are permitted for storage on site. Open waste containers must be sealed end of shift with hard covering wrapped with 2 layers of plastic sheeting or tarps to render air tight, water tight and secure. F. No asbestos debris in soil was observed during the inspection. If soil, walkways, building interiors are not effectively isolated and protected from asbestos debris and asbestos contamination is confmned by visual inspection and / or testing during or following work, the Contractor shall be responsible for all associated clean-up / decontamination / replacement costs. G. Work areas where asbestos components are being removed without abrasive abatement / cleaning methods that would render the asbestos materials friable may do so without negative pressure containments as long as visible emissions or uncontrolled dust / debris is not generated. H. Work areas where asbestos components are not removed in a non-friable condition and where abrasive abatement / cleaning methods such as but not limited to grinding must do so with negative pressure containments. 1. Any loss of negative pressure / containment breech into the interior of the school shall be handled as a fiber release episode and containment, pressurization, cleanup and testing shall be conducted in accordance with EPA AHERA, CT State DPH and the Asbestos Consultant's directives. Air testing, bulk testing, dust testing and / or visual inspection may be conducted by the Asbestos Consultant to determine whether the fiber release episode occurred. Section EAS, Inc.
