1 0.0 Separation of work Types and Cost Estimate Exhibit A Scope of Work 0.1 Prepare scope of work and cost estimate for MnDOT efforts relating to all items numbered less than Prepare scope of work and cost estimate for City of White Bear Lake efforts relating to all items numbered starting at Administration and Project Management 1.1 Prepare for and attend monthly progress meetings. Provide meeting minutes and Gantt chart updates. This task assumes eight Contractor meetings at Waters Edge, four Contractor internal coordination meetings, four Subcontractor meetings at Waters Edge and four Subcontractor internal coordination meetings 1.2 Provide daily instruction and superintendence of work. This task assumes the design portion of the project will last four months. 1.3 Provide a schedule of work using a Gantt chart. 1.4 Coordinate and obtain project permits from the City of White Bear Lake, Ramsey County and State. 1.5 Collect information needed for plans from State functional units and outside agencies. 1.6 Organize, publicize and provide graphics and handouts for one public open house, and up to three additional meetings. 1.7 Provide graphic and written content for State's project website. 1.8 Provide written updates for constant contact list. 2.0 Plan Preparation 2.1 Construction Plans Follow State Metro District's Sample Plan Follow Level 2 Computer-Aided Drafting and Design (CADD) standards Follow State s manuals and technical memoranda Comply with environmental documents Comply with State s Materials Design Recommendations Field verify condition of all storm drain components 2.2 Field verify condition of all signs. 2.3 Field verify existing guardrail for damage and deterioration; analyze need for additional guardrail. Field review will be performed in conjunction with task Include shoulder aggregate where overlay raises profile or other areas where shoulder aggregate is deficient. 2.5 Include corrections to clear zone deficiencies. 2.6 Provide miscellaneous surveys-topography at intersections, sidewalk inventory (1/2 inch or greater). 2.7 Include improvements to meet Americans with Disabilities Act (ADA) standards Conduct field investigation to verify State s recommendations are consistent with ADA standards. Make recommendations regarding sufficiency of facilities Conduct additional survey work to verify ADA compliance. 3.0 Final Plan Sheets Develop and provide to State the following: 3.1 Title Sheet - 1 sheet 3.2 General Layout - 4 sheets Page 1 of 6

State will provide MicroStation files of Standard Plans Staging Plan - 20 sheets 3.13 Inplace Utility, Topography and Removal Plan - 5 sheets 3.14 Construction Plan - 5 sheets 3.15 Construction Plan Details including ADA design plans - 6 sheets, 2 hours per corner of intersection, 12 corners 3.16 Drainage Plan 4 sheets 3.17 Storm Water Pollution Prevention Plan (SWPPP) and Water Resources Engineering (WRE) Notes - 3 sheets 3.18 Turf Restoration and Erosion Control Plan - 5 sheets 3.19 Drainage Profiles and Tabulations - 3 sheets 3.20 Concrete Pavement Rehabilitation Details - 1 sheet. State will provide MicroStation files of available details as needed Traffic Control Plan and Details - included on staging plan sheets 3.22 Striping Plan and Details - 6 sheets 3.23 Signing Plans - 5 sheets 3.24 Cross Sections - Cross sections will not be prepared 3.25 Cost Estimate - State will provide unit prices for recent similar construction 3.26 Special Provisions - Assumes minimal editing of Special Provisions that are available on State's web site 4.0 Construction Plan Review 4.1 Submit 60% plans and review checklist. 4.2 Submit 90% plans (considered 100% complete), review checklist and review comment form. 4.3 Submit 100% plans for review and comment. 5.0 Construction Cost Estimate and Special Provisions 5.1 Submit 60% Cost Estimate. 5.2 Submit special provisions and cost estimate with 90% plans. 5.3 Submit 100% complete special provisions. 6.0 Quality Management of Final Plan 6.1 Follow and provide documentation as outlined in State s Quality Management Process 6.2 Provide quality control checks and documentation at 60%, 90% and 100% plan submittals. 7.0 Utility Coordination 7.1 Coordinate project development with utility owners in accordance with State s Utilities Manual. 7.2 Blank Page 2 of 6

3 7.3 Field review visible utilities to check what is shown on plans and make appropriate corrections. 7.4 Address utilities, design to avoid/minimize conflicts. 7.5 Submit utility easement or other documentation of reimbursement eligibility gathered from utility owners. 7.6 During bid or proposal period, provide plan interpretation and information to provide understanding of project; attend preconstruction meeting and provide utility coordination information. Assumes 1 meeting at 4 hours; 4 hours of interpretations during proposal period. 7.7 Be available during construction to assist in design changes due to utility issues - assume 2 changes. 7.8 Where pipelines are present, include note on plans "WARNING: Petroleum Products Pipeline". 8.0 Transportation Management Plan (TMP) Assumes using State s TMP template with minimal modifications, and coordination and support provided by State. 8.1 Set up and attend meetings with State to go through and revise the TMP checklist (up to 2 meetings). 8.2 Document project issues, considerations and decisions during design related to safety and mobility. 8.3 Complete traffic analysis as necessary to estimated traffic delays during construction (HCM or Synchro). 8.4 Complete the TMP report Deliverable Standards The following standards apply to all deliverables in this Contract. See the individual deliverable description for more specific deliverable standard requirements. 9.1 Software The software version Contractor will use for deliverables are as follow: Microsoft (MS) Word 2007 Adobe Acrobat 8.0 Microsoft (MS) Excel 2007 MicroStation Version Paper copies of reports and technical memoranda will be printed as two-sided copies to the extent possible. 9.3 Paper copies of drawings will be printed, single-sided, on 11 x pound white bond paper. 9.4 Electronic documents will be delivered via compact disc (CD) whenever practicable. 9.5 A final Material Design Recommendation (MDR) will be provided. 9.6 Final signal plans will be provided by MnDOT Administration and Project Management 51.1 In addition to MnDOT monthly meetings, three additional design meetings to specifically discuss City items (primarily median enhancements). These meetings would be with City staff Provide daily instruction and superintendence of work. This task assumes the design portion of the project will last eight months Provide a schedule of work using a Gantt chart Coordinate and obtain project permits from the City of White Bear Lake, Ramsey County and State. A preliminary concept design will be provided by the City Final design of center median. Final design of center median pedestrian refuges at White Bear Avenue, Whitaker Street, 2nd Street, 4th Street, 7th Street, 8th Street and CSAH Design of median landscape elements. Landscape elements may include vegetation, lighting, and pavement treatments. This design will include soils preparation and accommodations for watering and maintenance Design of water main replacement from Lake Avenue North to Veterans Park will include reconnection of existing services Design installation of overhead, lighted street name signs at White Bear Avenue, CSAH 96, 2 nd Street, 4 th Street, 7th Street and TH Design work for signal poles, mast arms and associated equipment at the same five intersections as Design corridor lighting both in the median and behind curb.
