1 Site survey for SMART Room System (SRS) with Skype for Business Page 1 Purpose: Collect room specific information needed for hardware installation Highlight any potential challenges or additional work to be performed by all parties Capture any non-standard installation requirements Responsible for completion: IOP installation technician Customer site contact Site survey technician requirements: Meet and confirm the local persons responsible for the installation Complete this form in full by following the guidelines below: o Document the structural, power and network data details as completely as possible o Document any installation concerns in this form o If a facilities representative is available to discuss the installation wall and any other concerns, document your findings below in the applicable section o Include room drawing showing any obstructions and location of important items like power outlets and network interface o Take clear pictures of the room features described on page 9 and include then with the completed form **Revisit may be required if any fields are left blank**!

Is there an elevator to transport the equipment if needed? Is a store room available to safely store all packages until installation date? Will all equipment be carried to the room / nearby room before installation? Is this a unionized labor site? (Is union labor required to perform the drilling of holes, etc.?) Is asbestos a concern when drilling into wall surfaces? Is garbage and recycling disposal available onsite? Are there any general safety concerns? 4. If this is necessary due to room or customer requirements, please flag and discuss with SMART Authorized reseller and SMART ICS Agent. Standard Lengths: HDMI 6.3M (20 8 ) USB with active extension 6.7m (22 ) Describe the installation wall (concrete, brick, single/dual layer plaster, wood/metal studs, reinforcement). Are there any obstructions that could impact the installation? (Light switches, thermostats, doors, windows, glass, etc.) Is additional wall evaluation or assessment needed by building facilities? (YES / NO) Confirm if there are any known obstructions inside the wall at the mounting location (water pipes, electrical conduit, HVAC, etc.)

Describe the floor (concrete/raised floor, carpeted, wood/concrete tiles Are there any direct sources of incandescent light or sunlight? (Pot lights near the installation wall, windows with no blinds?) Is there excessive background noise in the room that could cause problems with audio quality? Are there any obstructions that could cause issues with heat dissipation from the interactive flat panels? Minimum clearance is +4" (10cm) around all sides.
