(Freestanding or attached to a structure) Required Documents: Application for permit filled out in its entirety with correct parcel number and original notarized signature of license-holder or owner-builder Proof of Ownership (warranty deed, tax bill or Property Appraiser printout.) Letter of Authorization (a notarized letter of authorization from the owner is required giving the applicant permission to obtain the permit on the property.)
