2 SCHOOL OF BUSINESS PARKING LOT AND VARIOUS PARKING LOT REPAVING PROJECT FOR Alabama A & M University Huntsville, Alabama ADDENDUM NUMBER ONE Page 1 of 2 This addendum shall forma part of the contract documents for The School of Business Parking Lot and Various Parking Lot Repaving Project on the Campus of Alabama A & M University, Huntsville, Alabama Modifications to the contract documents; dated 03 January 2018, made by this addendum, follow: REFER TO THE PROJECT MANUAL: 1. ABC FORM C-3, ACCOUNTING OF SALES TAX, the following clarification is offered: a. See Attachment Number One Alabama Department of Revenue Notice, Tax Guidance for Contractor, Subcontractors and Alabama Governmental Entities Regarding Construction Related Contracts for Clarification on this issue. b. See Attachment Number Two ACT , CERTIFICATE OF EXEMPTION FROM SALES AND USE TAX FOR GOVERNMENTAL ENTITIES, dated 28 October 2013, for clarification on this issue. 2. SPECIFICATION SECTION ALLOWANCES, make the following change: a. In Section 3.3 SCHEDULE OF ALLOWANCES, add the following Allowance Number Three: C. ALLOWANCE No. THREE: 1. The Contractor shall include in his bid all labor and material to repair existing potholes and damaged paving in the existing parking lot adjacent to the existing Carver Building and Carver Annex as shown on drawing sheet C-5 SITE LAYOUT PLAN REPAVING OF PARKING LOT 2. Repair work shall be constructed in accordance with civil engineering detail 6 / C-6. The Allowance amount shall be 150 cubic yards. The Geotechnical Engineer shall determine the suitability of earth pad under the pavement by use of a loaded dump truck across the pad. Final price will be adjusted by the difference in material removed / replaced from 150 cubic yards as confirmed by the Geotechnical Engineer and / or the Surveyor. This allowance includes material cost, receiving, handling, and installation, and Contractor overhead and profit. Coordinate quantity allowance adjustment with corresponding unit-price requirements in Section "Unit Prices." 3. SPECIFICATION SECTION UNIT PRICES, make the following change: a. In Section 3.1 SCHEDULE OF UNIT PRICES, add the following Unit Price Number Three:

UNIT PRICE No. THREE DAMAGED PAVEMENT AT PARKING LOT 2: 1. Description: Provide a unit price quote for the removal of deteriorated existing pavement and repair of potholes at the existing parking lot adjacent to the Carver Building and Carver Annex shown on drawing Sheet C-5. Repair shall be in accordance with Detail 6 / C PROPOSAL FORM, make the following change: a. Replace the existing Proposal Form with the attached form labeled ADDENDUM NUMBER ONE. REFER TO THE DRAWINGS: 1. ON SHEET C - 2 SITE LAYOUT PLAN, make the following change: a. Replace the existing drawing sheet with the attached revised sheet C ON SHEET C 4, STOREM SEWER PLAN & PROFILE, the following clarification is offered: a. The headwall should be a Flared End Precast Concrete Headwall as shown on the plans. 3. ON SHEET C - 5 SITE LAYOUT PLAN REPAVING OF PARKING LOT 2, make the following change: a. Replace the existing drawing sheet with the attached revised Sheet C - 5. b. Pavement thickness for parking lot 2 shall be one inch. 4. ON SHEET C - 6 SITE DETAILS SHEET 1, make the following change: a. Refer to Detail 6 / C-6; Delete the 1 requirement for the Tack Coat. 5. ON SHEET E -1 PARKING LOT 1 LIGHTING PLAN, make the following change: a. Replace the existing drawing sheet with the attached revised Sheet E-1. ATTACHMENTS: 1.
