1 TH 25 Bituminous Mill, Full Depth Reclamation and Overlay SP & Contract No Project Overview is in need of professional and technical assistance to provide detail design for State Project (SP) , a pavement resurfacing project on TH 25 from Benton/Morrison County line to 123 rd Street in Genola in Morrison County, and State Project , a pavement resurface and rehabilitation project on TH 25 located in Benton County from the Jct. of TH 95 to Jct. of TH 23 in Foley. SP is scheduled for a letting date of November 18, SP is scheduled for a letting date of December 16, This work requires the development of separate plans and specifications for each project. SP This project consists of bituminous milling, bituminous surfacing, culvert repairs, and the following additional work: Americans with Disabilities Act (ADA) improvements within the city limits of Buckman, which include concrete curb & gutter, ADA ramps, sidewalk and driveway pavement construction. There is a vertical curve correction to address a sight distance deficiency at 83rd Street on the south end of the project. This section will be reconstructed with a 950 foot vertical curve to correct this deficiency. SP This project consists of bituminous milling, full depth reclamation, bituminous surfacing, and culvert repairs. The project is divided into two distinct sections: From R.P to R.P In this pavement section the final proposed roadway will have 10 shoulders, of which 6 will be paved. From R.P to R.P In this pavement section the final proposed roadway will have 8 paved shoulders. Final project plans and specifications will be prepared in accordance with Manuals, Standards and Work Types included in the project. The will perform tasks necessary to prepare and receive approval for final plans. The s tasks will include, but are not limited to: Project Management Utility Identification and Coordination Construction Limits Detail Roadway Design Consultation During Construction 1. PROJECT MANAGEMENT [Source Type 1010] Project management will include work necessary for communication and completion of the project tasks on time and within budget. The s Project Manager or their primary duties will not be reassigned without the written consent of the Project Manager. The s staff will have the training and expertise necessary for the work tasks to which they are assigned. 1.1 Project Management and Administration Monthly Budget and Billing (Invoice and Status Report) Include a cover letter with each Invoice that explains the general status of the project, including at a minimum the work completed to date, the anticipated remaining efforts and required schedule changes; progress report form; supporting data for direct expenses; and an updated project status 1

2 report reflecting Project and Primavera 6 (P6) activities identified by the Project Manager Weekly Update to s Project Manager Update the Project Manager with regards to the status of the project schedule, budget and general status/progress on a weekly basis (as defined by the Project Manager). 1.2 Work Plan and Schedule Develop a refined work plan (scope of work, budget, and schedule) based on discussions with the Project Manager Submit the refined work plan to the Project Manager for review and approval Develop and maintain the project schedule. 1.3 Kick-off Meeting Schedule and facilitate a kick-off meeting to establish communications protocol, discuss known issues and review the project schedule Coordinate the scheduling of the Design Team Meetings. The timeline for the meetings will likely be after the review of submittals and at a time appropriate for traffic staging meetings Receive available existing information from including existing horizontal alignment, right of way data, and other required design data. 1.4 Design Team Meetings Attend up to 6 team meetings during detail design Prepare agenda and facilitate each Design Team meetings. Agenda items should include, but not be limited to, the following: Review the project schedule and budget status with the s Project Manager and task leaders. Address and discuss issues that may affect the schedule, budget, or work quality Prepare and distribute draft minutes within 2 business days after each Design Team meeting and final minutes within 3 business days after each Design Team meeting. 1.5 Survey Coordination Make an analysis of existing survey data and information If any data is found to be incorrect or incomplete, bring this finding to the attention of the Project Manager immediately with a survey request, such that district surveys can collect the additional field data needed for a quality design. 1.6 Quality Control Management Assign a key team member the title of quality control manager. Quality Control Manager will: Review key submittals. Key submittals include draft plan set submittals. Use the following basic format for quality certification: checking, back checking, and verifying format in accordance with s Design-Bid-Build Quality Management Process. Refer to for further information Develop a TH 25 specific Quality Management Plan (QMP) detailing the quality process to be used Submit the TH 25 QMP to for review and approval. 1.7 Deliverables Schedule and attend meetings Hold weekly update meeting or conference call with s Project Manager Prepare meeting agendas, displays, and minutes Prepare and submit monthly progress and status reports Maintain project schedule Provide timely copies of critical correspondences and 2 Review and respond to submittals Coordinate internal reviews Provide copies of project correspondences and project information Attend meetings Approve deliverables Monitor quality control

3 project issue data Submit refined Work Plan Submit electronic project files at the project completion TH 25 Quality Management Plan (QMP) 2 UTILITY IDENTIFICATION AND COORDINATION (Source Type 1195) For the purpose of this contract utility facilities means and includes all privately, publicly, or cooperatively owned communication lines and facilities; and any system, lines, or facilities for the distribution or transmission of electrical energy, gasoline, oil, gas, water, steam, or for the exclusive collection of sewage. 2.1 Utility Identification and Coordination Utility identification and coordination will include all tasks necessary to comply with the current Utilities Manual and all applicable State Statutes. The utility identification and coordination services set forth herein must be performed by individuals with sufficient skill and experience Complete Gopher State-One Call, design locate request and field locate, as necessary Prepare and distribute utility identification, verification and relocation correspondence to utility owners Identify possible construction impacts Facilitate at least 1 utility information meeting and 1 utility design meeting Complete project utility certification Incorporate this information into the Detail Design Plans, according to s design standards. 2.2 Deliverables Provide utility coordination and plan content per current State Utilities Manual Prepare and distribute utility verification and relocation correspondence to utility owners Obtain and submit follow-up responses to the utility verification and information correspondence Facilitate at least 1 utility information meeting and 1 utility design meeting, if necessary Project utility certification Provide templates for utility coordination correspondence (e.g., Utility Coordination Letter) Approve all correspondence prior to delivery to utility owners. 3 CONSTRUCTION LIMITS (Source Type 1200) Establish preliminary cross-sections and grading plans to determine limits of construction. 3.1 Cross Sections and Earthwork Develop cross sections and earthwork sufficient for the preparation of a Section 404 wetland permit. 3.2 Plan / Layout Complete plan view of construction limits and wetland impacts sufficient for preparation of a Section 404 wetland permit. 3.3 Deliverables Construction Limits Determination of wetland impacts Review construction limits and submit comments Wetland Delineations 4 DETAIL ROADWAY DESIGN (Activity Code 1250) 4.1 Detail Roadways Design Prepare portions of the construction plans for the proposed roadway improvements that are consistent with horizontal and vertical alignments, typical sections, and construction limits identified in the 3

4 approved Design Memorandum. The portions of the road plans prepared under this Contract will also be consistent with findings and recommendations identified in the CATEX and Material Design Recommendation (MDR), and ADA recommendations Perform work in accordance with s Highway Project Development Process (HPDP), Computer Aided Drafting and Design (CADD) standards, and Technical Memoranda. Work will be completed using English units Ensure that the format of the draft construction plans comply with the District provided sample plan and s CADD Standards Manual Prepare the following MicroStation plan sheets. Sheets will be combined with the prior consent of the Project Manager: 1. Title Sheet 2. General Layout 3. Statement of Estimated Quantities (SEQ) 4. Soils & Construction Notes 5. Standard Plates 6. Alignment Tabulations 7. Typical Sections 8. Quantity Tabulations 9. Utility Tabulations 10. Underground Utilities Plan 11. Removal Plans 12. Miscellaneous Details 13. Standard Plan Sheets 14. Construction Staging Plans & Details 15. Traffic Control Plans, detour plans & tabulations 16. In-place Topography, Utility, and Right-of-Way 17. Construction Plan Sheets 18. Roadway Profile Sheets 19. Grading Plans 20. TPAR Plans 21. ADA Intersection Detail Sheets (1:20 Scale) 22. ADA Driveway Elevation Table 23. Storm Water Pollution Prevention Plan (SWPPP) 24. Staking Tabulations and Plan Sheets with coordinate point locations and elevations at all critical points. 25. Drainage Plans, and Profiles 26. Earthwork Tabulations and Summary 27. Temporary Erosion/Sediment Control Plans 28. Turf Establishment and Permanent Erosion/Sediment Control Plans 29. Striping Plan 30. Signing Plan 31. Cross-Section Sheets 4.2 Hydraulic Analysis and Design Complete all activities necessary to analyze the hydraulic system in the projects area. The plan will also show the appropriate labels, direction of surface flow, and direction of water flow into the drainage structures. will provide a hydraulics recommendation including sizes and locations of culverts within the project Culvert Repair and Replacements: Include design culvert extensions and drainage modifications as needed. Prepare the analysis and design in accordance with s Drainage Manual, Technical Memoranda, Design Manuals and the Standard Specifications for Highway Construction utilizing HydroCAD, HY8 or CulvertMaster and Power Geopak V8i. 4

5 Prepare related notes, tabulations and specifications. 4.3 Traffic Control Design Prepare traffic control and detour plans for this project, consistent with the Minnesota Manual of Uniform Traffic Control Devices (MMUTCD), Manual for Temporary Traffic Control Zone Layouts, and District 3 practices, including the following details: Confer with regarding the staging, schedule, and detours Prepare Transportation Management Plan (TMP), in coordination with the District 3 Traffic Engineer, as guiding document for design and resource for implementation during construction Prepare traffic control and detour plans that will correspond with each construction stage. The plans will include notes, symbols and abbreviations, layout plans for each stage showing signs, pavement markings, channelizing devices, traffic barriers, delineators, temporary curbs, attenuators, and pavement marking removals as appropriate Prepare tabulations that will summarize the markings, signs, and other traffic control devices and their locations Maintain a minimum of one lane in each direction, at all times, during construction. 4.4 Special Provisions Produce the project Special Provisions. Deletions from, and additions to Standard Specifications will be written and included as necessary Develop construction contract time and traffic provisions (timeline and bar chart) with input from Submit Special Provisions to for review along with the 90%, and 100% Construction Plan submittals Submit electronic copies of the final Special Provisions in Microsoft Word format with the Final Construction Plans. 4.5 Engineer s Construction Cost Estimate Submit the Engineer s Construction Cost Estimate based on quantities and information at hand, starting with the Intermediate Design (60%), and all submittals thereafter Utilize the latest cost data available for the cost estimates Submit an electronic copy of the cost estimates in Excel format. 4.6 Plan Format Utilize the sample plan provided, to develop the Construction Plans to comply with s current design concepts and practices Submit all sheets contained in the Construction Plans and cross sections to in Power Geopak V8i formats. The Plans and cross sections will be in compliance with CADD Standards Manual Deliver plans submitted for reviews in PDF format created using Adobe Acrobat 8.0 or later Submit the final detail design plan on 11 x17 bond and only the title sheets will be on an 11 x 17 sheet of vellum. 4.7 Plan Review and Approval Submit Construction Plans and documents for review and approval at the following milestones: Initial Design (30% Complete) Submit one PDF copy of the following elements of the Construction Plan. staff will complete review and comment on this submittal within 10 business days of the submittal date. District 3 staff will complete review and comment on this submittal within 10 business days of the submittal date Make all necessary revisions required by the District 3 staff The will conduct a constructability review in cooperation with the District 3 staff to confirm the feasibility of staging and plan implementation Detail Design (90% Complete) Consider the 90% complete plans to be 100% complete Submit one Power Geopak V8i copy and one PDF copy of the proposed Construction Plan, one electronic Microsoft Word copy of the Project Special Provisions, and one electronic Microsoft Excel copy of the Engineer s Construction Cost Estimate for s review and comment. The District 3 staff will complete review and comment on this submittal within 10 business days of the submittal date. Selected sheets will be sent to s Central Office for preparation of any necessary Agreements Make all necessary revisions required by the District 3 staff Construction Plan (100% Complete) Upon making the revisions to the 90% Construction Plan submittal, the will submit two signed and certified set of bond prints with vellum title sheet, one Power Geopak V8i copy and one PDF copy of the Construction Plan. Electronic signature is acceptable for signing the plans, except for the title sheet which must have wet signatures Submit one electronic Microsoft Word copy of the Project Special Provisions; and one electronic Microsoft Excel copy of the Engineer s Construction Cost Estimate Submit original prints to s Central Office Design Liaison Unit for review and approval. s Central Office Design Liaison Unit will make a final review and comment on the certified Construction Plan Construction Plan (Revisions for Bid Letting) Make the revisions requested by s Central Office Design Liaison Unit, the will submit new signed and certified sheets, as necessary. An electronic copy of the project s Geopak design files and each sheet in Power Geopak V8i format and PDF format will be submitted. One copy of the design computations and quantity calculations will also be submitted. 4.8 Deliverables Submit all deliverables specified in this Exhibit to Complete review and comments in a timely manner 6
