The Saint Paul Legislative code requires that no building shall be occupied without a Certificate of Occupancy and a Fire Certificate of Occupancy. Neither of these certificates will be issued unless all work required to be done under permit is inspected and approved by the appropriate inspector. Your Certificate of Occupancy and Fire Certificate of Occupancy will be granted upon demonstration of compliance with the following deficiency list and payment of required fees. A re-inspection will be made on or after June 15, YOU WILL BE RESPONSIBLE FOR NOTIFYING TENANTS IF ANY OF THE FOLLOWING LIST OF DEFICIENCIES ARE THEIR RESPONSIBILITY. DEFICIENCY LIST 1. Bldg Throughout - MSFC Provide, repair or replace the fire rated door and assembly. The minimum rating must be: 20 minute.-repair and maintain the door closers, the door latches and the door frames. 2. Bldg Unit 1 South Sleeping Room Egress Window - MSFC Provide and maintain an approved escape window from each sleeping room. The minimum size must be 5 square feet of glazed area with a minimum of 24 inches of openable height and 20 inches of openable width. With a finished sill height not more than 48 inches. This work may require permit(s). Call DSI at (651) Refer to the Escape Windows for Residential Occupanices handout for more information.-replace broken egress window in south bedroom. 11. Bldgs 652 and Appliances Throughout - SPLC (2), (1) - Repair and maintain all required and supplied equipment in an operative and safe condition.-missing kitchen appliances throughout all units. 12. Bldgs 652 and Exterminator Report - SPLC (6), (5) - Exterminate and control insects, rodents or other pests. Provide documentation of extermination.- Evidence of bugs and roaches found in lower units. Have a licensed exterminator inspect and treat building for bugs and roaches. 13. Bldgs 652 and Throughout - SPLC (3), (3) - Repair and maintain the door in good condition.-replace or repair all broken doors, door latches, strike plates, door frames and door locks throughout building. 14. Bldgs 652 and Throughout - SPLC (3), (3) - Repair and maintain the door frame.-replace or repair all broken door frames throughout the property. 15. Bldgs 652 and Throughout - MSFC All light fixtures shall be maintained with protective globes if originally equipped.-replace or repair all missing light fixture covers.

Bldgs 652 and Throughout - SPLC Provide and maintain light and ventilation in all habitable rooms in accordance with the Saint Paul Legislative Code.- Install missing light bulbs throughout interior and exterior of property. 17. Bldgs 652 and Throughout - SPLC Provide address numbers on the garage facing the alley.-the address number to be posted is: 652 and Bldgs 652 and Throughout - SPLC (3), 34.33(2) - Repair or replace the damaged handrail in an approved manner.-install missing runs on ends of staircase handrails throughout. 24. Bldgs 652 and Throughout - SPLC (3), 34.33(2) - Repair or replace the unsafe stairway in an approved manner.-repair and replace all loose and damaged staircase steps in building and basement. 25. Bldgs 652 and Throughout - MSFC Repair or replace damaged electrical fixtures. This work may require a permit(s). Call DSI at (651) Replace and repair all loose and damaged electrical throughout building. 26. Bldgs 652 and Unit Utilities - SPLC (5), (4), Provide and maintain interior in a clean and sanitary condition.-gas and water shut off. Missing 3 boilers/furnaces. Bldg. 652 Unit 1 furnace found not in operation during inspection. 27. Bldgs 652 and Units and Basement - MN Rules Immediately discontinue use of unsafe heating appliance until repaired or replaced by a licensed contractor. This work may require a permit(s). Call DSI at (651) Red tags may not be removed except by fire department inspector.-multiple uncapped gas and plumbing lines throughout property. Multiple gas pipes cut throughout basement. 28. Bldgs 652 and SPLC 34.23, MSFC This occupancy is condemned as unfit for human habitation. This occupancy must not be used until re-inspected and approved by this office.-no water, heat, electricity or gas.

4 29. Exterior - House - SPLC (1) b,c, (1) b,c - Provide and maintain all exterior walls free from holes and deterioration. All wood exterior unprotected surfaces must be painted or protected from the elements and maintained in a professional manner free from chipped or peeling paint.-repair or replace all missing and damaged soffits, fascia and eaves throughout property. 30. Exterior - House - SPLC (1) e, (1) d - Provide and maintained the roof weather tight and free from defects.-replace deteriorated west roof. 31. Exterior - House - SPLC (2), 34.32(2) - Repair or replace the unsafe stairways, porch, decks or railings in an approved manner. This work may require a permit(s). Call DSI at (651) Repair cracks in west staircase. 32. Exterior - West Porch - SPLC (5), (3) - Repair, replace and maintain all exterior surfaces on fences, sheds, garages and other accessory structures free from holes and deterioration. Provide and maintain exterior unprotected surfaces painted or protected from the elements.-repair ceiling boards on 654 side. 33. Interior - Throughout - MN Stat. 299F Immediately provide and maintain a smoke detector located outside each sleeping area.-replace all missing hard wired smoke detectors throughout the building. 34. Interior - Throughout - MN State Statute 299F.50 Immediately provide and maintain an approved Carbon Monoxide Alarm in a location within ten (10) feet of each sleeping area. Installation shall be in accordance with manufacturers instructions.-replace all missing and broken CO alarms throughout the units and basement. 35. Plumbing Basement - Fixture: Standpipe - SPLC 34.11, SBC , SPLC 34.17, MPC condition.-(mpc 0200 E) - Unvented. 36. Plumbing Basement - Gas Piping - SPLC 34.11, SBC , SPLC 34.17, MPC condition.-(ifgc 402.1) - Dryer gas shutoff; connector or piping incorrect. (IFGC IMC 604.1) - Run dryer vent to code. 37. Plumbing Basement - Soil and Waste Piping - SPLC 34.11, SBC , SPLC 34.17, MPC condition.-(mpc 1000) - No front sewer clean out; no soil stack base clean out. (MPC 2420) - Improper connections, transitions, fittings or pipe usage. (MPC 1430 Subp. 4) - Add appropriate hangers. 38. Plumbing Basement - Water Heater - SPLC 34.11, SBC , SPLC 34.17, MPC condition.-(mpc 2210 Subp. 4) - T and P relief discharge piping incorrect on all five water heaters. (MPC 1730 Subp. 1) - Water piping incorrect on all five water heaters.

(MPC 2100) - Boiler fill water line requires backflow assembly or device. (MPC 1430 Subp. 4) - Add appropriate hangers. 41. Provide and maintain exterior unprotected surfaces painted or protected from the elements.-repair and seal cracks in retaining walls. 55. Electrical MSFC Close openings in service panel/junction boxes with knockout seals, and/or junction box covers. 56. MSFC Properly strap cables and conduits in basement/service conduit on the exterior of the house. 57. MSFC Install globe-type enclosed light fixtures in all closet lights. 652 Oakdale Ave. Apt. 1) 58. MSFC 605.1, Repair or replace all broken, missing, or loose light fixtures, switches & outlets, covers, and plates. 59. MSFC Check all outlets for proper polarity and verify ground on 3-prong outlets. Repair or replace to 2011 NEC (No power at time of inspection) 60. MSFC Replace all painted-over receptacles. 61. MSFC Wire electric water heaters to code. A 30 amp circuit with 30 amp wiring is required. 62. MSFC Remove or repair electrical in kitchen pantry. (654 Oakdale Ave. Apt. 1) 63. NEC - All added receptacles must be grounded, and in dwelling units must also be tamper-resistant and be on an Arc-Fault Circuit Interrupter protected circuit. 64. NEC - Any open walls or walls opened as part of this project must be wired to the standards of the current NEC. (654 Oakdale Ave. Apt. 2)

7 65. Minnesota Electrical Act - All electrical work must be done by a Minnesota-licensed electrical contractor under an electrical permit. 66. MNFGC Install approved lever handle manual gas shutoff valve on all gas appliances and remove unapproved valves. 67. MNFGC Install approved lever handle manual building shutoff gas valve in an accessible location ahead of the first branch tee. 68. MNFGC All gas piping from gas meters to gas appliances to be tested according to code 69. HEATING REPORT - SPLC (6) - Clean and Orsat test boiler burner. Check all controls for proper operation. Check furnace heat exchanger for leaks; provide documentation from a licensed contractor that the heating unit is safe. 70. MNFGC Install approved metal chimney liner. 71. MNFGC Replace furnace/boiler flue venting to code. 72. MNFGC Connect furnace/boiler and water heater venting into chimney liner. 73. MNFGC & MNMC Vent clothes dryer to code. 74. MNFGC Provide adequate combustion air and support duct to code. 75. MNFGC Provide support for gas lines to code. MNMC Plug, cap and/or remove all disconnected gas lines and unapproved valves. 76. MNRC R Bathroom exhaust system vented to the outside with approved material according to code. A mechanical ventilation permit is required if an exhaust system is installed. 77. SPLC 34.11(6) - Install a heating system for each dwelling unit according to code. 78. SPLC 34.11(6) - Provide heat in every habitable room and bathrooms. 79. MNMC Attach metal tag to expansion tank valve stating that this valve must be OPEN at all times except when draining the expansion tank. 80. MNMC Support supply and return piping for heating system according to code. 81. MNMC Conduct witnessed pressure test on hot water heating system and check for leaks. 82. MNMC Install boiler pressure relief valve and pipe discharge to within eighteen inches of the floor.

8 83. MNMC & MNPC Install back flow preventer on city water fill line to hot water heating system and pipe vent as required. 84. MNMC Repair or replace radiator valves as needed. 85. MN RULES mechanical gas, warm air/ventilation permits are required for the above work. 86. SPLC (6), (3) - Provide service of heating facility by a licensed contractor which must include a carbon monoxide test. Submit a completed copy of the Saint Paul Fire Marshal's Existing Fuel Burning Equipment Safety Test Report to this office. 87. SPLC 39.02(c) - Complete and sign the smoke detector affidavit and return it to this office.
