2 SUMMARY The Unit-1, factory is in a seven storied building with a shed. The building was constructed in 2001, and the factory started the production in the same year. The building was approved for commercial purpose and the factory during survey, had about 190 workers working on regular basis. The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The purpose of the survey was to identify significant electrical safety issues and to provide recommendations for remediation based on applicable standards specified by the Accord. The scope of this initial electrical safety inspection was limited to the review and identification of major electrical safety issues. The inspection did not include identification of minor deficiencies, which will be further addressed as part of follow-up inspections. Table below summarizes the major electrical safety issues identified during the inspection. Recommendations have been provided to address each issue. An implementation schedule shall be developed by the factory to remediate each of the findings. The specific timing of improvements, including any requested extensions due to design / installation constraints, shall be submitted to the Accord for approval. Page 2 of 14

3 FINDINGS AND RECOMMENDATIONS FINDING NO: E- 1 CATEGORY: DRAWINGS The factory does not have electrical Single Line Diagram. N/A Factory must have as-built electrical single line diagram for proper maintenance of their electrical system. REMEDIATION TIME FRAME: 12 WEEKS FINDING NO: E- 2 CATEGORY: OPERATION & MAINTENANCE The following programs and test results are not conducted and kept. 1. Thermographic scanning inspection report of electrical equipment (tri-annual base). 2. Insulation resistance test of electrical equipment (5 year cycle). 3. Electrical safety program (training on electrical safety). N/A Set up the above program and keep the test result on site. REMEDIATION TIME FRAME: 25 WEEKS Page 3 of 14

4 FINDING NO: E- 3 CATEGORY: GENERATOR ROOM The size of generator room does not have adequate to perform routine maintenance activities. Ensure appropriate size for the generator room in order to properly access the generator to perform routine maintenance activities. REMEDIATION TIME FRAME: 1 WEEK Inadequate generator room size. FINDING NO: E- 4 CATEGORY: GENERATOR ROOM Combustible material present inside the generator room. Need to remove combustible material from the generator room. 6 FINDING NO: E- 7 CATEGORY: EQUIPEMENT & MACHINE Exhaust fan hanged from ceiling without proper foundation inside the compressor room. Also, no mechanical on rotating part. The exhaust fan should be properly mounted on the wall with proper foundation and provide mechanical guard. Exhaust fan inside the compressor room. FINDING NO: E- 8 CATEGORY: WIRINGS Wires on wall without mechanical guard. The PVC/ rigid pipe must be used for surface wiring which must be continuous through-out its length and properly supported. 7 FINDING NO: E- 9 Distribution board is not provided with adequate clearance in front (typical). Provide clearance of at least 1 m (39 in) in front of distribution board. PRIORITY: P3 Inadequate clearance in front of distribution boards. FINDING NO: E- 10 Panel doors not connected to earth bond. Provide earth connection for the body and doors of metallic distribution boards using green cables preferably braid so that the metallic door remains at zero potential all the time. Replace the wooden distribution board by a distribution board made of noncombustible material (using sheet steel fabrication of a minimum thickness of 20 SWG). REMEDIATION TIME FRAME: 5 WEEKS Distribution board made by wooden board. FINDING NO: E- 12 MCCB and fuse are installed on wooden board. Wooden boards shall be avoided in factories for mounting the MCCB and fuse. Install these protective devices into an enclosure made of non-combustible material. MCCB and fuse mounted on wooden board. 9 FINDING NO: E- 13 Control components (DOL starter) are mounted on the wooden board and wires connecting to the starter terminals are not protected. Wooden boards shall be avoided in factories for mounting the DOL starter. Install these protective devices on non-combustible material. Wires entering and exiting the DOL starter panels should be protect in conduit till its connection end to prevent from physical damaging of cable insulation. DOL starter mounted on wooden board. FINDING NO: E- 14 CATEGORY: CABLE SUPPORT Remaining gap around the cables passing through wall is not sealed. 11 FINDING NO: E- 16 CATEGORY: WIRINGS Wires exposed while extending concealed wiring point. Use rigid PVC pipe for conceals wiring and cable should be fully encased in the pipe for protection. The conduit shall run vertically or horizontally, shall never at angle. Wires exposed while extending concealed wiring point. FINDING NO: E- 17 Bunch of cables is connected without lug in a single point of bus-bar. (Typical) Need to connect a single cable in a single port with the proper size of cable lug.
