2 SOUTH GRANVILLE WATER AND SEWER AUTHORITY H STREET BY-PASS WATERLINE INSTALLATION CONTRACT NO GENERAL DESCRIPTION OF COST ITEMS FOR H STREET BY-PASS WATERLINE INSTALLATION GENERAL: The South Granville Water and Sewer Authority (SGWASA) proposes to construct a waterline in Butner, North Carolina from the 12 th Street / G Street Intersection to a point of connection to an existing 12 inch ductile iron waterline on H Street. The waterline will primarily be 16 inches in diameter installed in easements across property owned by the Town of Butner. A section of 12 inch ductile iron waterline will be installed connecting to the 16 inch waterline before it crosses H Street to connect to the existing 12 inch ductile iron waterline in H Street. The overall length of ductile iron water pipe to be installed is approximately 2658 feet of 16 inch ductile iron piping and 116 feet of 12 inch ductile iron piping. All work required is shown on Drawings S-1.0 through 3.2; S-5.0 through S-5.2 and S-8.0 through S-8.1 and S-8.0. Drawing S-0.0 is the Cover Sheet which provides a vicinity map. All work performed on this Project will be in accordance with the Supplemental Technical Specifications and SGWASA s Standard Water Specifications. All work shown on the Drawings and outlined in these Specifications will include furnishing all materials, labor, equipment, appurtenances and construction necessary for a complete and satisfactory installation of the proposed ductile iron waterlines. Payment for all work will be according to the Items provided. The cost for the work performed will be used to determine the low, responsive, responsible bidder for the Project. The cost for work required for which there is no ITEM in the Bid will be distributed among the various prices submitted in the ITEMS. The following is a brief description of required work and means for payment for each individual line ITEM as listed for the work required for the H Street By-Pass Waterline: ITEM 1: MOBILIZATION: A lump sum price will be provided for the assembling of all equipment, material and labor on-site as necessary to commence construction of the Waterline. The price named for Mobilization will also provide for obtaining a Performance and Indemnity Bond with the North Carolina Department of Transportation as well as all other bonds and permits required for construction. The price named will not exceed seven (7) percent of the subtotal cost provided for work. The price named for Mobilization will also cover the cost of materials stored at the Site for the duration of the Project. Payment for Mobilization will be made according to the following schedule: 60% on the first monthly Estimate after trenching begins according to the Construction Schedule. Remaining 40% on the second monthly estimate provided sufficient progress has been made with an adequate labor force working at the Site installing pipe. ITEM 2: MEETINGS AND INSPECTIONS: A lump sum price will be provided for the attendance of the Contractor at a preconstruction meeting and required monthly construction meetings. Also included in this Item will be the cost associated with participating in final inspections as the Project is substantially completed by the Contractor. Payment for this Item will be prorated with each monthly estimate according to the Schedule. ITEM 3: CONSTRUCTION STAKING: A lump sum price for Construction Staking as specified. Reference Sections I and III in the Specifications for required construction staking and documentation to be provided to the Engineer. ITEM 4: CLEARING AND GRUBBING AND REMOVAL OF DEBRIS: A lump sum price for Clearing and Grubbing and the removal of all trash and debris within temporary and permanent easements. All clearing, grubbing and debris will be removed and properly disposed off-site according to state and federal laws. No burning will be allowed. 44

3 ITEM 5: 16 INCH WATERLINE FROM STATION 0+00 TO STATION : A lump sum price for installing a 16 inch ductile iron waterline from the Tapping Sleeve and Valve located at Station 0+00 to the reducer located at Station as shown on Engineers Drawings S-1.0, S-2.0 and S-3.0. Stone bedding, geotextile fabric, undercutting, Select Backfill, valving and fittings will be paid separately under the appropriate Items as specified. The cost associated with trenching, pipe laying and backfilling with acceptable excavated material will be included in the cost named for installing the 16 inch waterline. Payment for this Item will be prorated on each monthly estimate based upon the length of ductile iron waterline laid as a percentage of the overall length of waterline to be installed under this Item. ITEM 6: 12 INCH WATERLINE FROM STATION TO STATION : A lump sum price for installing a 12 inch ductile iron waterline from the reducer located at Station to the tee at Station and all of the ductile iron piping parallel H Street as shown on Engineer s Drawing S-3.2 necessary to make the piping connection to the existing 12 inch ductile iron waterline on H Street. The price named for 12 inch ductile iron piping will include all stainless steel rodding as shown on Engineer s Drawings S-1.2 and S-8.2 and the installation of a 4-1/2 inch fire hose as a jumper to provide water from the newly installed fire hydrant at Station to the existing fire hydrant located on the north side of H Street where shown on Drawing S-3.0. Stone bedding, geotextile fabric, undercutting, Select Backfill, valving, fittings and hydrants will be paid separately under the appropriate Items as specified. The cost for installing Concrete Collars will be included in the cost named for Class AA Concrete. Payment for this Item will be prorated on each monthly Estimate based upon the length of ductile iron waterline laid as a percentage of the overall length of waterline to be installed under this Item. ITEM 7: BORING OF 30 INCH DIAMETER STEEL ENCASEMENT PIPE: A unit price per foot for boring 30 inch diameter steel encasement pipe, inches thick, under 12 th Street as a dirt bore. The cost provided for this Item will include the installation of the carrier pipe along with other appurtenances as shown by the Boring Assembly Detail on Engineer s Drawing S-8.1. ITEM 8: PRESSURE TESTING NEW WATERLINE FROM STATION 0+00 TO STATION : A lump price for pressure testing new ductile iron waterline from the Tapping Sleeve and Valve at Station 0+00 to the 12 inch gate valve at Station according to SGWASA Standard Specifications. The cost named for this Item will also include flow testing of newly installed hydrant as specified. ITEM 9: PRESSURE TESTING NEW WATERLINE FROM STATION THROUGH 12 INCH DUCTILE IRON PIPING INSTALLED ALONG H STREET: A lump sum priced for pressure testing new ductile iron waterline from 12 inch gate valve at Station through all 12 inch ductile iron piping shown on Engineer s Drawings S-3.2 along H Street according to SGWASA Standard Specifications. The cost named for this Item will also include flow testing of newly installed hydrant as specified. ITEM 10: DISINFECTION TESTING OF NEW WATERLINE FROM STATION 0+00 TO STATION : A lump sum price for disinfecting and sterilizing new ductile iron waterline from the Tapping Sleeve and Valve at Station 0+00 to the 12 inch gate valve at Station according to SGWASA Standard Specifications. ITEM 11: DISINFECTION TESTING OF NEW WATERLINE FROM STATION THROUGH 12 INCH DUCTILE IRON PIPING INSTALLED ALONG H STREET: A lump sum price for disinfecting and sterilizing new ductile iron from the 12 gate valve at Station through 12 inch ductile iron piping installed along H Street as shown on Engineering Drawing S-3.2 according to SGWASA Standard Specifications. ITEM 12: 6 INCH DUCTILE IRON WATERLINES TO HYDRANTS: Unit price per foot for installing 6 inch ductile iron waterlines associated with fire hydrant assemblies. Length of piping will be determined by measuring from centerline of connecting tee at feeder main, through in-line valves and fittings, to center of hydrant. Valves, hydrants, fittings, rock excavation, undercutting, stone bedding, and select backfill will be paid separately under the appropriate Items as specified. Costs associated with trenching, pipe laying, rodding and backfilling with acceptable excavated material will be included in the unit cost named for this Item. ITEM 13: ¼ DEGREE BENDS: A unit price each for installing ¼ degree ductile iron bends as shown on Drawings or required by the Engineer. Unit price to include restraining the fitting as specified. Cost associated with concrete blocking will be paid separately under the appropriate Item. 45

4 ITEM 14: ½ DEGREE BENDS: A unit price each for installing ½ degree ductile iron bends as shown on Drawings or required by the Engineer. Unit price to include restraining the fitting as specified. Cost associated with concrete blocking will be paid separately under the appropriate Item. ITEM 15: DEGREE BENDS: A unit price each for installing degree ductile iron bends as shown on Drawings or required by the Engineer. Unit price to include restraining the fitting as specified. Cost associated with concrete blocking will be paid separately under the appropriate Item. ITEM 16: 12 X16 INCREASER: A unit price each for installing a 12 x16 increaser as shown on Drawings or required by the Engineer. Unit price to include costs associated with installing mechanical joint retainer glands. ITEM 17: 12 X6 TEES: Unit price each for installing 12 x6 tees as shown on Drawing or required by Engineer. Unit price to include costs associated with mechanically restraining the tee as specified. Cost associated with concrete blocking will be paid separately under the appropriate Item. ITEM 18: 12 X12 TEES: Unit price each for installing 12 x12 tee as shown on Drawing or required by Engineer. Unit price to include costs associated with mechanically restraining the tee as specified. Cost associated with concrete blocking will be paid separately under the appropriate Item. ITEM 19: 6 GATE VALVES / VALVE BOXES: Unit price each for installing 6 gate valves as shown on Drawings. Unit price to include costs associated with installing a valve box over the operator of the gate valves along with concrete base around valve box casting. ITEM 20: 12 GATE VALVES / VALVE BOXES: Unit price each for installing 12 gate valves as shown on Drawings. Unit price to include costs associated with installing a valve box over the operator of the gate valves along with concrete base around valve box casting. ITEM 21: TAPPING SLEEVE AND VALVE ASSEMBLY / PRECAST MANHOLE: Lump sum price for installing 18 inch Tapping Sleeve with 16 inch outlet and valve including 6 foot diameter precast manhole and appurtenances as shown on Engineer s Drawing S-8.0. Costs associated with stone bedding / foundation support, rock excavation, undercutting, Select Backfill, poured in place concrete will be paid separately under the appropriate Items as specified. All other costs associated with this Assembly will be included in the cost named for this Item. ITEM 22: 12 INCH CUT-IN VALVE TO EXISTING 12 INCH DUCTILE IRON WATERLINE IN H STREET: Lump sum price for installing a 12 inch cut-in valve on the existing 12 inch ductile iron waterline as shown on Engineer s Drawing S-3.2. The cost for traffic control will be included in the price named for installing this valve. ITEM 23: FIRE HYDRANT ASSEMBLIES: Unit price each for installing fire hydrant assemblies as shown on the Drawings. Cost provided for hydrant assemblies to include installation of hydrant only along with tie rods and concrete base around valve box casting. Cost associated with stone bedding / foundation support, rock excavation, undercutting and Select Backfill will be paid separately under the appropriate Items as hereinafter specified. ITEM 24: 12 DUCTILE IRON SLEEVES: Unit price each for installing 12 ductile iron sleeves with appropriate gaskets to connect to existing ductile iron waterline along H Street. Ductile iron sleeves will be long body, mechanical joint, minimum 12 inches in length, mechanically restrained as specified. ITEM 25: H STREET ASPHALT ROAD BASE AND SURFACE COURSE REPLACEMENT: Unit price per ton for removing existing asphalt surfaces across H Street and after trenching and pipe laying operations with CABC stone backfill, re-surfacing H Street with 3 inches of S9.5B asphalt in two lifts, each lift being 1.5 inches thick. The first lift will be considered the base coarse. The second lift the surface coarse. End joints for trenching operations will be established by the Engineer. All end joints will be saw-cut prior to trenching operations. All debris and removed asphalt will be properly disposed off-site. The cost for debris removal will be included in the price named for this Item. ITEM 26: EROSION CONTROL BLANKETS: Unit price per square yard for installing erosion control blankets as specified. Installation will be required at locations shown on the Drawings or as directed by the Engineer. 46

5 ITEM 27: CLASS B RIP-RAP: Unit price per ton for installing Class B rip-rap at locations shown on the Drawings and directed by the Engineer. Payment of rip-rap quantities will be verified by tickets delivered to the site clearly marked SGWASA, H Street By-Pass Waterline. The price named for Class B Rip-Rap will also include the cost for installing a filter fabric as specified prior to installing the rip-rap. ITEM 28: CLASS 1 RIP-RAP: Unit price per ton for installing Class 1 rip-rap at locations shown on the Drawings and directed by the Engineer. Payment of rip-rap quantities will be verified by tickets delivered to the site clearly marked SGWASA, H Street By-Pass Waterline. The price named for Class 1 Rip-Rap will also include the cost for installing a filter fabric as specified prior to installing the rip-rap. ITEM 29: SILT FENCE: Unit price per linear foot for installing silt fence as specified and shown on the Drawings. Installation will be required at locations shown or as directed by the Engineer. The cost for trench stone will be paid separately under Item 31. ITEM 30: ROCK CHECK DAMS: Unit price each for installing rock check dams as shown in detail on the Drawings. Location of check dams to be as shown on the Drawings or as directed by the Engineer. Temporary rock check dams installed at the end of the waterline at the end of each work day will not be considered for payment. ITEM 31: #57 CRUSHED STONE: Unit price per ton for crushed stone used for hydrant drains, foundation support, trench stabilization in unstable areas, pipe bedding, resurfacing gravel drives, and areas shown on the Drawings or directed by the Engineer etc... All stone used will be clean and washed, ¾ in diameter. Payment of stone quantities will be verified by tickets delivered to the site clearly marked SGWASA, H Street By-Pass Waterline. ITEM 32: #67 CRUSHED STONE: Unit price per ton for crushed stone used for hydrant drains, foundation support, trench stabilization in unstable areas, pipe bedding, resurfacing gravel drives, and areas shown on the Drawings or directed by the Engineer etc... All stone used will be clean and washed, ½ in diameter. Payment of stone quantities will be verified by tickets delivered to the site clearly marked SGWASA, H Street By-Pass Waterline. ITEM 33: CABC STONE: Unit price per ton for the use of crusher run type stone for pipe cushioning or support as the waterline crosses storm culverts, resurfacing gravel drives where appropriate, as a backfill of trench excavation where shown on the Drawings and as a stone subgrade for asphalt paving. Payment of stone quantities will be verified by tickets delivered to the site clearly marked SGWASA, H Street By-Pass Waterline. ITEM 34: SELECT BACKFILL FOR WATERLINE TRENCHES: Unit price per cubic yard for the use of any select backfill required from off-site locations necessary for backfilling trenches with suitable material. Select Backfill material will be used only with approval from the Engineer and only in conditions where unsuitable material is present for proper compaction or trench rock is present during pipe laying operations. Payment for Select Backfill will be verified by tickets delivered to the site clearly marked SGWASA, H Street By-Pass Waterline. Payment for the use of Select Backfill generated on-site will be made only with the approval of the Engineer. Quantities will be verified by the Engineer by truck count if Select Backfill is used whereby existing contours do not change after construction by more than 6 inches. ITEM 35: SELECT BACKFILL FOR EMBANKMENT: Unit price per cubic yard for the use of Select Backfill required from off-site locations properly tested for embankment and the backfill around the Elliptical Pipe and other areas stipulated by the Engineer. Payment for Select Backfill will be verified by tickets delivered to the site clearly marked SGWASA, H Street By-Pass Waterline or by Average End Methods to calculate the volume of placed material. ITEM 36: UNDERCUTTING: Unit price per cubic yard for the removal of unsuitable or unstable earth material that is necessary to provide a satisfactory foundation for structures or piping. Undercutting is further defined as that material that must be removed below designed subgrade elevations shown on the Drawings. Undercutting will be measured by geometric formula for the material removed. Undercutting will only be paid for after the Engineer has been notified and inspected the foundation conditions and approved the removal of additional earth material for disposal. All undercut material will be properly disposed off-site. 47

6 ITEM 37: 15 REINFORCED CONCRETE PIPE: Unit price per linear foot for installing new 15 inch reinforced concrete pipes as specified for new stormwater culvert piping where shown on Drawings or as directed by the Engineer. ITEM 38: 15 INCH FLARED END OUTLET: Unit price each for installing 15 inch reinforced concrete flared end outlet pipe where shown on the Drawings. The price named for the flared end outlet will include excavation required for the energy dissipater at the end of the flared end outlet. Rip-rap installed for the dissipater will be paid under the appropriate Item. ITEM 39: 34 x53 CONCRETE ELLIPTICAL PIPE: A lump sum price for installing 34 x53 concrete elliptical pipe as specified and shown on the Drawings. All excavation, trenching, removal of topsoil, constructing slopes will be included in the cost named for installing the concrete elliptical pipe. The cost named for installing the concrete elliptical pipe will include the cost associated with installing a flared end section at the outlet and inlet ends. Payment for this Item will be prorated on each monthly estimate based upon the length of pipe laid as a percentage of the overall length of piping to be installed under this Item. The price named for this Item will include excavation for the inlet / outlet rip-rap energy dissipaters. Rip-Rap and Select Backfill For Embankment will be paid under the appropriate Items as specified. ITEM 40: FLOWABLE FILL: Unit price per cubic yard for pouring a grout mix around grout mix each side of the Elliptical Piping as shown on Drawing S-3.1. ITEM 41: CLASS AA CONCRETE: Unit price per cubic yard for the pouring of all Class AA Concrete at locations shown on the Drawings, directed by the Engineer and where steel reinforcement is required. Payment for Class AA Concrete will be verified by tickets delivered to the Site clearly marked SGWASA, H Street By-Pass Waterline. ITEM 42: CLASS A CONCRETE: Unit price per cubic yard for the pouring of all Class A Concrete at locations shown on the Drawings, directed by the Engineer and where concrete blocking is installed at all waterline fitings. Payment for Class A Concrete will be verified by tickets delivered to the Site clearly marked SGWASA, H Street By-Pass Waterline. ITEM 43: 4 FOOT DIAMETER MANHOLE: A lump sum price for installing a 4 foot diameter manhole to extend a 15 inch storm drain line across the easement as shown on Drawing S-2.0 and in detail on Drawing S-8.1. ITEM 44: TOPSOIL: Unit price per cubic yard for providing a topsoil as specified to mix with on-site earth material over areas impacted by trenching or other construction areas cleared of vegetation and trees within the easement in order to provide a satisfactory seedbed for stabilization or areas that need to be fine graded to lawn standards. The price named for this Item will include mixing and blending the topsoil as part of fine grading operations in order to help establish a permanent cover of grass over disturbed areas. Topsoil will be used only after approval has been granted by the Engineer. Payment for topsoil will be verified by tickets delivered to the Site clearly marked SGWASA, H Street By-Pass Waterline. ITEM 45: TREE PROTECTION FENCING: Unit price per linear foot for installing tree protection fencing. Installation will be required at locations shown on Drawings or as directed by the Engineer. ITEM 46: NON-WOVEN GEOTEXTILE FABRIC FOR TRENCHES: Unit price per linear foot for installing non-woven geotextile fabric in trenches for stabilization as specified. Payment by the linear foot will be based upon a roll width of twelve (12) feet. ITEM 47: GRAVEL CONSTRUCTION ENTRANCE: Lump sum price for constructing a gravel construction entrance off H Street as shown in Detail on Drawing S-5.2. ITEM 48: SILT FENCE OUTLET: Unit price each for installing silt fence outlets along the waterline easement as shown in Detail on Drawing S-5.2. ITEM 49: ROCK AND GRAVEL FILTER: Lump sum price for shaping the subgrade for the rock and gravel filter shown on Drawing S-5.1 and in Detail on Drawing S-5.2. The cost named for this Item will include all earth excavation and shaping and the installation of the filter fabric. The cost for stone will be paid under other Items as specified for Class 1 Rip-Rap and No. 57 Stone. 48

7 ITEM 50: FINE GRADING, SEEDING, AND MULCHING: Lump sum price for repairing all disturbed areas except for streets and drives. All disturbed areas will be prepared and properly seeded within seven (7) days after construction in any particular area. The Contractor will obtain a full stand of grass and any areas which show erosion or settle will be filled with suitable topsoil and reseeded as necessary. Payment for this Item will be prorated on each monthly estimate based upon Stationing along the new waterline as a percentage of the overall length of new waterline Stationing. ITEM 51: CLEAN-UP AND DEMOBILIZATION: Lump sum price for the removal and disposal of any and all equipment, debris, trash, and materials after construction and temporary sediment control measures such as silt fencing after work areas have stabilized from seeding. Rock check dams are to be removed. Disturbed areas where check dams were installed will be seeded and mulched. 49

8 BID FORMS TO SOUTH GRANVILLE WATER AND SEWER AUTHORITY PROPOSAL FOR H STREET BY-PASS WATERLINE INSTALLATION Proposal of (hereafter called BIDDER ), organized and existing under the laws of the State of North Carolina doing business as (insert a corporation, a partnership, or an individual as applicable). TO SOUTH GRANVILLE WATER AND SEWER AUTHORITY (hereinafter called OWNER ). In compliance with your Invitation for Bids, Bidder hereby proposes to furnish all materials, tools, machinery, equipment, apparatus, labor, and all means necessary to perform all work in connection with constructing the H Street Waterline Improvements in strict accordance with the Contract Documents, within the time set forth therein, and at the prices stated hereinafter. By submission of this Bid, each Bidder certifies that this Bid has been arrived at independently, without consultation, communication or agreement as to any matter relating to this Bid with any other Bidder or with any competitor. Bidder hereby agrees to commence work under this Contract on or before a date to be specified in the Notice to Proceed and to complete the Project within 240 CALENDAR DAYS. Bidder further agrees to pay as liquidated damages, the sum of $ for each consecutive calendar day which elapses between the agreed completion date and the date of actual completion. Bidder acknowledges receipt of the following Addenda: The undersigned has carefully examined the sites of this work, and has informed himself fully in respect of the conditions of the place where the work is to be performed, and has examined the Drawings and Specifications for the work, the Proposed Contract, and the Contract Documents relating thereto, has examined all special provisions, labor requirements, and other matters relating thereto. It is understood that the South Granville Water and Sewer Authority reserves the right to reject all bids or to award the Contract to the low, responsive, responsible Bidder taking into consideration quality, performance and time specified in the bid. Award will be on the basis of the lowest responsive proposal for ALL OF THE BID ITEMS. If the proposal is not accepted by the South Granville Water and Sewer Authority, the bid bond or cash or check deposited in lieu thereof will be returned to the undersigned. On being awarded a Construction Contract, the undersigned will execute a Contract for the construction of the work described in conformity with the Contract Documents in the form hereto attached. A 100% Performance Bond and a 100% Payment Bond in the amount of the Contract with acceptable Corporate Surety, will be delivered to the South Granville Water and Sewer Authority. Performance and Payment Bonds will be valid for one year beyond the date of acceptance of completed Project. 50

9 The undersigned agrees, that if awarded the Contract, he will commence work on the Project and within 240 CALENDAR DAYS from the date stipulated on the Notice-To-Proceed, complete the work and agrees to pay as liquidated damages the sum of $ for each consecutive calendar day that the work remains incomplete and not ready for continuous use by the public and accepted by the South Granville Water and Sewer Authority. Bidder agrees to furnish all labor, materials, tools, equipment, fees, bonds and services and do all things necessary to perform the work described in the Contract Documents for constructing the H STREET BY-PASS WATERLINE at the following unit and lump sum prices: I.
